We describe a 56-year-old woman with histiocytic sarcoma involving the bone marrow. The neoplastic cells proliferated diffusely and showed occasional erythrophagocytosis. Immunohistochemically, the neoplastic cells were positive for CD68, lysozyme, CD4 and CD163, but negative for B- and T-cell markers, S100 protein and epithelial markers. The patient received multi-agent chemotherapy and is living at 22 months after diagnosis without recurrence. Histiocytic sarcoma is an exceedingly rare hematopoietic neoplasm and the prognosis is poor due to its rapid progression, widespread disease and poor response to therapy. The doctors were stumped when nothing would come up. All organs were fine they called it a strange case. He had no red blood cells and they didnt know what was destroying them. Finally they did a bone marrow test and Andy had histiocytic sarcoma. This liquid in his stomach was from the proteins not getting to his body he had a very very low red blood cell count. They told us Andy had 3 to 4 weeks to live when he left the hospital that Friday and he died 30 hours later. Thank you. They were amazing months and with the cancer coming back so quickly, we wondered if we wouldnt have had three weeks only without neoplasene? We just needed to be on a higher dose. The vet in Value told me this would happen if we didnt increase it. We tried to heed his warning, but the pancreatitus stopped us from being able to react fast enough. He was on just enough to be barely holding it off. He went from clear and clean to gone in 3 weeks! Its just so hard to believe. I really pray that someone learns from our story. We got some amazing photos and some great days and appreciated every extra moment we got and that was all possible because of you and Addiss journal. Thank you from the very bottom of our hearts for giving us the extra days. His quality of life was better in the last 7 months than it had been in the last 2 years. Resistance to the Brutons tyrosine kinase (BTK) inhibitor ibrutinib has been attributed solely to mutations in BTK and related pathway molecules. Using whole-exome and deep-targeted sequencing, we dissect evolution of ibrutinib resistance in serial samples from five chronic lymphocytic leukaemia patients. In two patients, we detect BTK-C481S mutation or multiple PLCG2 mutations. The other three patients exhibit an expansion of clones harbouring del(8p) with additional driver mutations (EP300, MLL2 and EIF2A), with one patient developing trans-differentiation into CD19-negative histiocytic sarcoma. Using droplet-microfluidic technology and growth kinetic analyses, we demonstrate the presence of ibrutinib-resistant subclones and estimate subclone size before treatment initiation. The genes of interest are those that code for proteins known as growth factor receptors. These proteins are present on the surface of the cell and when stimulated by growth factors, signal into the cell promoting cell survival and growth.
