Coronavirus disease-2019 (COVID-19) is associated with severe inflammation in mainly the lung, and kidney. Reports suggest a beneficial effect of the use of heparin/low molecular weight heparin (LMWH) on mortality in COVID-19. In part, this beneficial effect could be explained by the anticoagulant properties of heparin/ LMWH. Here, we summarise potential beneficial, non-anticoagulant mechanisms underlying treatment of COVID-19 patients with heparin/LMWH, which include: (i) Inhibition of heparanase activity, responsible for endothelial leakage; (ii) Neutralisation of chemokines, and cytokines; (iii) Interference with leukocyte trafficking; (iv) Reducing viral cellular entry, and (v) Neutralisation of extracellular cytotoxic histones. There were several suggestions which I am following up, namely: 1) Reducing pH to ,4 to ppt DNA (or using PEI, protamine sulfate, etc...) 2) Trying Exclusion on Q-media first then running through S-media 3) Dilution of detergent to levels below CMC 4) Alternative buffer systems 5) Checking pI of the protein (sorry if I excluded any others, but I took them all into consideration!) OK, I tried diluting out the detergent to below CMC and running on S-Sepharose, but still got flat line after the void on the chart. I also tried reducing the pH to below 4 but this didnt seem to work either. I couldnt try the precipitations because we didnt have any PEI or protamine sulfate at the time! I jumped on these questions right away.. I found no instrument or QC problems. Specimens were rechecked for clot, hemolysis and tube type, pre-analytical variables could affect results. All 4 specimens were good. To rule out possible intravenous fluids or heparin contamination in the specimens, we contacted the patients nurse who confirmed that the last specimen was drawn from a peripheral stick. Therefore, at least results from the last specimen were reliable. Then what was wrong with the patient? Although PT and aPTT were the only two tests ordered by the doctor, I added D-dimer, heparin activity level by anti-Xa method (HAL), fibrinogen (FIB) and thrombin time (TT) on these 4 specimens and re-tested PT and aPTT at the same time. The four specimens had identical results: D-dimer >20000 ng/ml FEU and no clot on PT, PTT, fibrinogen and thrombin time tests. HAL results were zero indicating no heparin contamination. It was mediated by a heat-stable nondialyzable factor which separated with C-reactive protein (CRP) during fractionation and purification, correlated with serum CRP levels, and, like other known reactivities of CRP, was inhibited by phosphoryl choline. Preparations of CRP purified either from serum or ascites resulted in consumption of large amounts of C1, C4, and C2 when preincubated with normal serum and protamine.
