N2 - Introduction: Severe sepsis and septic shock are common problems in the intensive care unit and carry a high mortality. Endotoxin, one of the principal components on the outer membrane of gram-negative bacteria, is considered important to their pathogenesis. Polymyxin B bound and immobilized to polystyrene fibers (PMX-F) is a medical device that aims to remove circulating endotoxin by adsorption, theoretically preventing the progression of the biological cascade of sepsis. We performed a systematic review to describe the effect in septic patients of direct hemoperfusion with PMX-F on outcomes of blood pressure, ...
In this study, we measured plasma endotoxin concentrations of blood drawn from the radial artery and the outlet circuit of the PMX column after 24 hours duration of PMX treatment in septic shock patients. The assay for endotoxin was performed with separated plasma from heparinized whole blood samples centrifuged at 3,000 rpm for 40 seconds. The high-sensitivity assay was performed by kinetic turbidimetric Limulus assay using a MT-358 Toxinometer (Wako Pure Chemical Industries, Ltd, Japan). This Limulus assay test is specific to endotoxin and has no cross-reaction to β-glucan. The endotoxin removal rate was defined by the equation: ((radial artery endotoxin concentration - outlet circuit of PMX column endotoxin concentration)/radial artery endotoxin concentration) × 100%. The endotoxin removal rate represents endotoxin adsorption ability. Five patients with septic shock were studied. ...

The mortality rate due to peritonitis associated to a severe sepsis or a septic shock remains high (between 40 and 60% as per the studies). The recent complementary therapies for severe sepsis have been reassessed (strict glycaemic control, substitutive corticotherapy, activated protein C). Early neutralisation of the endotoxaemia related to gram-negative bacilli sepsis in contact with hemoperfusion membrane covered with polymyxin B (Toraymyxin™) may enable reduction of the inflammatory reaction caused by sepsis and improve its prognosis. 30 studies, including 10 randomized studies, have compared hemoperfusion with Toraymyxin™ to the standard treatment, showing an improvement in the patients haemodynamic state, oxygenation conditions and reduction in mortality. This treatment is commonly used in Japan. However, the studies conducted either include only a limited number of patients or are not randomized prospective studies. The post-hoc analysis of a recent randomized study conducted on a ...

Direct hemoperfusion therapy with polymyxin B immobilized fiber cartridge (PMX-DHP) is an established strategy in the treatment of septic shock in Japan and parts of Western Europe. PMX-DHP is currently the subject of a pivotal North American randomized controlled trial (EUPHRATES) in patients with septic shock and confirmed endotoxemia, as measured by the endotoxin activity assay. The major mechanism of action of this therapy is the removal of circulating endotoxin. High affinity binding of circulating endotoxin by the PMX-DHP column may decrease circulating endotoxin levels by up to 90% after two standard treatments. Basic research has shown reductions in circulating cytokine levels and in renal tubular apoptosis. Clinical research has shown that PMX-DHP therapy results in hemodynamic improvements, improvements in oxygenation, renal function, and reductions in mortality. Further research is needed to further define additional patient populations with endotoxemia that may benefit from PMX-DHP therapy
This clinical study designed as a prospective, open labelled, multi-centre, RCT will be carried out to evaluate if direct hemoperfusion with polymyxin B immobilized fiber column (PMX) is superior to conventional medical therapy for sepsis, for patients with sepsis arising from abdominal cavity infection, accompanied by the failure of one or more organs. 120 patients (60 treatment/60 control) will be considered in this study. Those patients fulfilling inclusion criteria and not having exclusion criteria will be randomly allocated to one of two study groups. One group will be treated with PMX (PMX group) and the other will receive a standard therapy for sepsis (control group). All patients will receive full intensive care management, including fluid resuscitation, vasopressors, antimicrobial chemotherapy, ventilatory support, and renal replacement therapy, if required. Each patient will be followed up for 28 days after study entry ...
Thirty-six sepsis patients were the subjects, consisting of 21 men and 15 women aged 62 ± 18.5 years. A thermodilution catheter was inserted in each patient, and the mixed venous oxygen saturation, oxygen delivery index, oxygen consumption index, and oxygen extraction ratio were measured. After it was confirmed that systemic oxygen metabolism was not impaired, PMX-DHP was started. Patients with uncontrolled infection were excluded from the study. The Acute Physiology and Chronic Health Evaluation (APACHE II) score was used as the index of severity, and survival of the patients was assessed after 1 month. The humoral mediators measured were IL-8 as a chemokine, plasminogen activator inhibitor-I (PAI-1) as an index of vascular endothelial cell activation, and polymorphonuclear neutrophil elastase (PMN-E) as an index of neutrophil activation. These mediators were measured before the start of PMX-DHP, and at 24, 48, and 78 hours after the start. The PaO2/FiO2 (P/F) ratio was used as an index of ...

Serum alpha-fetoprotein (AFP) levels have been measured sequentially by a radio-immunoassay method in 64 patients with fulminant hepatic failure. In 15 of the 64 patients (23%) AFP levels were raised but in only two did they exceed 500 ng/ml. Of the 23 survivors 11 (48%) had raised AFP levels compared with four of the 41 (9-8%) fatal cases (P less than 0-005). This rise in AFP levels was found early after the development of grade IV coma and constitutes an encouraging prognostic sign at a time when the liver function tests and EEG are unhelpful. A radioimmunoassay must be used if these small but significant rises in plasma concentration are to be detected. Twelve patients survived without showing a rise in plasma AFP at anytime during the illness. The four fatal cases who had raised AFP levels all had serious complications of fulminant hepatic failure. Charcoal haemoperfusion did not seem to increase the survival of AFP negative patients.. ...

The treatment of porphyria cutanea tarda (PCT) in patients with chronic renal failure poses a therapeutic challenge. In the absence of renal failure, phlebotomy and oral antimalarials have been the standard of care for PCT. However, in the presence of renal failure, associated chronic anemia often precludes the use of phlebotomy, and oral antimalarials are usually ineffective. We describe a patient with severe symptomatic PCT and chronic renal failure whose disease was successfully managed with a combination of high-dose erythropoietin and small volume phlebotomy. We also review several previously reported approaches to management of PCT in the setting of renal failure, which include small repeated phlebotomy, erythropoietin, deferoxamine, chloroquine, plasma exchange, high-efficiency/high-flux hemodialysis, cholestyramine, charcoal hemoperfusion, and kidney transplantation. An algorithm for the management of these patients is proposed ...

Enhancing the elimination of a xenobiotic from a poisoned patient is a logical step after techniques to inhibit absorption such as orogastric lavage, activated charcoal, or whole-bowel irrigation have been considered. Table 10-1 lists methods that might be used to enhance elimination. Some of these techniques are described in more detail in chapters that deal with specific xenobiotics. In this chapter, hemodialysis, hemoperfusion, and hemofiltration are considered extracorporeal treatments because xenobiotic removal occurs in a blood circuit outside the body. Currently, these methods are used infrequently because most poisonings are not amenable to removal by these methods. In addition, because these elimination techniques have associated adverse effects and complications, the risk-benefit analysis suggests a benefit in a relatively small proportion of patients. ...
Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. Cytokine hemoadsorption is found to be effective in the management of septic shock, by reducing cytokines and other inflammatory mediators, thereby improving hemodynamics and preventing multiple organ dysfunction syndrome. Here, we report a case of 22-year-old male patient, presented with fever, vomiting, loose stools, and altered mental status since 2 weeks, was provisionally diagnosed with severe sepsis/septic shock - salmonella-induced, hepatitis B-induced acute liver failure, hemophagocytic lymphohistiocytosis, and disseminated intravascular coagulopathy. The patient was admitted in intensive care unit and started on cytokine hemoperfusion therapy for 12 h. Before cytokine hemoadsorption, he was on three inotropic supports, IV antibiotics, and other supportive measures. The patient improved symptomatically and hemodynamically within 8 h of cytokine therapy, and inotropic supports were weaned ...

Hi everyone,We are asking for help to pay for my fathers medical bills. He has been battling CKD (Chronic Kidney Disease) and now relies on going to Dialysis three times a week to survive until he is able to receive a kidney transplant. We have not yet recovered from the financial toll due to his open heart surgery and complications last year. Unfortunately, he has recently contracted COVID-19 and is currently being treated in the hospital but the medical bills are quickly becoming overwhelming. Due to his positive COVID-19 status, he is now required to complete his dialysis in isolation which costs so much more. He is also undergoing hemoperfusion. He is now on oxygen support due to inadequate oxygenation. He is also in need of extra treatment to survive the virus along with his future kidney transplant.Anything that you are willing to donate for my father will be immensely appreciated. We understand that the worlds current economic status is not ideal, so we graciously thank everyone who can donate
Nanotechnology has provided the possibility of delivering drugs to specific cells using nanoparticles.[9] The overall drug consumption and side-effects may be lowered significantly by depositing the active agent in the morbid region only and in no higher dose than needed. Targeted drug delivery is intended to reduce the side effects of drugs with concomitant decreases in consumption and treatment expenses. Drug delivery focuses on maximizing bioavailability both at specific places in the body and over a period of time. This can potentially be achieved by molecular targeting by nanoengineered devices.[10][11] A benefit of using nanoscale for medical technologies is that smaller devices are less invasive and can possibly be implanted inside the body, plus biochemical reaction times are much shorter. These devices are faster and more sensitive than typical drug delivery.[12] The efficacy of drug delivery through nanomedicine is largely based upon: a) efficient encapsulation of the drugs, b) ...

A double lumen cannula for hemodialysis and similar applications such as haemoperfusion comprises elongated flexible tubular members, disposed one within the other, for semi-permanent insertion into the sub-clavian vein of a patient, leaving semi-permanent access thereto for repeated hemodialysis treatments. The cannula is formed of an elongated flexible outer member, having a smooth exterior surface, and made up of an elongated main portion, a convergent, constricting portion and a narrower distal end portion which is tapered at the tip to fit over a Seldinger guide wire. There is also an inner member having a diameter substantially the same as that of the distal end portion. The inner member is insertable into the outer member to come into sealable engagement with the interior walls of the constricting portion, so as to form a continuous inner lumen with the distal end portion of the outer member, and is removable therefrom for removal of clot or replacement purposes. Both members are sufficiently
Quality by Design demands can be fulfilled by mechanistic process understanding and/or a massive expansion of the data basis - for example by high throughput techniques such as batch chromatography. Mechanistic models strongly depend on model parameters, which are fitted from experimental data, which are typically taken from column runs at lab-scale.. Quantification of adsorber characteristics: column scale vs. batch systems. To improve the implementation of a process at different stages during its development, the same model should be applied for all scales and formats. To allow for a model-based scale comparison, precise and uniform measurements of different column characteristics such as ionic capacity are then necessary. The standard technique to determine the ionic capacity in column scale is the acid-base-titration. As this approach requires in-line conductivity trace, the method is not applicable for high throughput experimental systems like batch chromatography.. Measuring histidine ...
In the past, there have been several attempts to get at the SIRS by pharmaceutical or medical therapies. But success remained modest or it lacked completely. Thus, for example, it was not possible to achieve enough clinical benefits with the reduction of cytokines and other inflammatory mediators by means of ultra filtration. However, the team led by Born was more successful. There were significant differences in the inflammatory parameters between the two study groups immediately after surgery. The cytokine IL-6 differed significantly throughout the post-operative course between the control and study group, fibrinogen responded with significantly less activation. The leukocytes showed a positive trend in the CytoSorb group and the CRP showed a lower increase in the CytoSorb group and returned to normal more quickly. The procalcitonin was significantly increased in the control group. Born´s conclusion: By using a cytokine adsorber the physician now has an option available to reduce the ...

|i|Introduction|/i|. Cytokines are basic targets that have to be removed effectively in order to improve the patient’s health status in treating severe inflammation, sepsis, and septic shock. Although there are different adsorbents commercially available, the success of their clinical use is limited. Here, we tested different adsorbents for their effective removal of cytokines from plasma and the resulting effect on endothelial cell activation.|i| Methods|/i|. The three polystyrene divinylbenzene (PS-DVB) based adsorbents Amberchrom CG161c and CG300m and a clinically approved haemoperfusion adsorbent (HAC) were studied with regard to cytokine removal in human blood. To induce cytokine release from leucocytes, human blood cells were stimulated with 1 ng/ml LPS for 4 hours. Plasma was separated and adsorption experiments in a dynamic model were performed. The effect of cytokine removal on endothelial cell activation was evaluated using a HUVEC-based cell culture model. The beneficial outcome was

Blood is the most important and inevitable part of the body. The working of our body depends on how well is the condition of the blood. For good functioning of the blood, our blood should be free from unwanted toxins and impurities. Liver, kidneys and lymphatic vessels tend to carry out the function of cleansing the blood. However sometimes the toxins tend to build up in the blood and the organs may not be able to cope up with them.
