This project represents the inauguration of a National Patient Registry for Lymphatic Diseases. This project will be completed through an affiliation with the Lymphatic Research Foundation, a non-profit organization whose mission is to promote research and the development of new therapies for patients with lymphatic diseases, including lymphedema. The registry will provide researchers with much-needed clinical data to study the impact of diseases of the lymphatic system, in order to develop improved treatments and find a cure for lymphatic diseases, lymphedema, and related disorders. The establishment of this initiative is a major step forward in research for direct study of groups of patients with lymphatic disease. In the future, this project will be able to be linked with a tissue bank, in which tissue and blood samples derived from patients will be made available for research into human disease states. The age-adjusted incidence of first lifetime thrombosis is 1.92 per 1000 person-years, which translates into nearly 190,000 noted thromboses each year in persons ages 45 years or older in the United States.1 DVT is especially common in hospitalized patients and may constitute the greatest mortality risk in some inpatient populations. The overall incidence of a first VTE seems to be similar among men and women, but the risk is higher among women of childbearing age than among men in the same age group. This gender difference likely relates to the association of VTE with pregnancy or the use of oral contraceptives. In contrast, the risk of thromboembolism among older women is substantially lower than that among men in the same age group.2,3 The 28-day fatality rate after a first thrombosis is 11%, with cancer conferring an increased mortality risk. The leg to be examined is elevated to drain the blood from the superficial venous system. It is convenient to rest the patients heel on the examiners shoulder. A narrow tourniquet is placed around the thigh as high as possible. A narrow tourniquet is used because it occludes the superficial veins but does not affect the deep veins. A length of rubber tubing makes a good tourniquet. With the tourniquet in place, the patient is asked to stand. The leg is inspected to see if the veins that had been seen previously are full or empty. The veins will fill slowly (30-60 seconds) because of arterial inflow or rapidly because of venous reflux. October 24, 2017-The Society for Vascular Surgery (SVS) announced the publication of a study showing that the results of endovascular treatment of iliocaval and infrainguinal postthrombotic venous obstruction do not appear to be adversely affected by the extension of iliac vein stents into the femoral venous system.. According to the SVS, venous stenting for symptomatic patients has increased significantly in this setting. Several large series have demonstrated safety, efficacy, and good durability of this technique in iliocaval obstruction. However, questions remain as to the outcomes with regard to etiology (ie, thrombotic versus nonthrombotic occlusion) and the extent of stenting (ie, extension below the inguinal ligament). Musculoskeletal tuberculosis (TB) accounts for approximately 10% of all extrapulmonary TB cases in the United States and is the third most common type of extrapulmonary TB after pleural and lymphatic disease. Vertebral involvement (tuberculous spondylitis, or Potts disease) is the most common type of skeletal TB, accounting for about half of all cases of musculoskeletal TB. The presentation of musculoskeletal TB may be insidious over a long period and the diagnosis may be elusive and delayed, as TB may not be the initial consideration in the differential diagnosis. The diagnosis is often confused with malignancy. Concomitant pulmonary involvement may not be present, thus confusing the diagnosis even further.
