Patients with mesothelioma who are fit enough to be treated with chemotherapy and enough tumor material was available are asked for participation in this study. After 4 cycles of Alimta chemotherapy, a leukapheresis is performed of which the monocytes are used for differentiation to DCs using different cytokines. The procedure to grow DCs in vitro and pulse them with tumor lysate is performed according to our earlier performed phase I study that was approved by our local ethics committee. Three doses of properly pulsed autologous DCs (MesoCancerVac) are then re-injected every two weeks. Patients will be treated with a low dose of CTX for seven day in a row the week before the 1st vaccination, the weeks in between the 2nd, and for one week after the 3rd vaccination ...
We report 34 patients (aged 5-18 years) with acute (n = 26) or chronic (n = 1) leukemia, non-Hodgkins lymphoma (n = 3) or severe aplastic anemia (n = 4) evaluated for pancreatic beta-cell function 9 months to 10.2 years after autologous (n = 19) or allogeneic (n = 15) BMT. Before BMT, all patients received cytotoxic drugs, combined with total body irradiation (TBI) in 24 cases or thoracoabdominal irradiation (TAI) in 4 children. Patients were investigated for fasting blood glucose (FBG), HbA1C, anti-insulin (IAA) and islet cell antibodies (ICA), first-phase insulin response (FPIR) and insulinemia/glycemia (I/G) ratio on i.v. glucose tolerance test (GTT) and C-peptide response after glucagon 1 mg i.v. Results were compared with those obtained in 21 age- and sex-matched controls. None of the patients or controls had IAA and/or ICA. FBG and HbA1C were normal in all children. In the patients, glycemia on i.v. GTT was similar to controls whereas insulin levels I/G ratio and FPIR were significantly ...

The participants were contacted by phone, and demographic information, medical history, and quality-of-life index were obtained. The next contact was in three to four weeks for a brief physical examination, blood draw, and electrocardiography. Information about previous stroke or TIA was obtained using the Questionnaire for Verifying Stroke-Free Status (see accompanying table). The stroke risk burden for each participant was evaluated using the Framingham Stroke Risk Score. The main outcome measure was stroke symptoms.. Results: The following stroke symptoms were found in the study group: sudden hemibody numbness (8.5 percent); sudden painless hemibody weakness (5.8 percent); sudden painless loss of vision in one or both eyes (4.6 percent); sudden inability of linguistic expression (3.8 percent); sudden hemifield visual loss (3.1 percent); and sudden inability to understand speech (2.7 percent). One or more of these symptoms was present in 17.8 percent of the study population. For each increase ...

We next used a ErbB2/HER-2-transduced MDA-MB-231 variant, termed 231-H2N. The 231-H2N line, which we previously described (12), was chosen for analysis for a number of reasons. Previously, we used this line to show the efficacy of combining the anti-Her-2 antibody trastuzumab (Herceptin) with metronomic low-dose cyclophosphamide to suppress the growth of orthotopically implanted 231-H2N primary tumors (12). The derivation of a metastatic variant of 231-H2N would therefore allow us to test if the same therapy is effective when treating metastatic disease. In addition, a metastatic model responsive to trastuzumab whose growth kinetics could be monitored (i.e., via β-hCG readings) would allow us to study conditions where resistance to trastuzumab-based regimens eventually develops. This, in turn, would permit proactive initiation of second-line therapies (12) when the first-line therapy begins to fail.. H2N.hCG cells were implanted in the mammary fat pad of female SCID mice. Primary tumors were ...

By Amy Alkon. This girl I met on a dating site attends another college, three hours away, so weve only talked on the phone. Ive experienced the online/in-person divergence before, so Im worried. Ive seen photos, but theyre blurry face shots or half-body shots. My friends and family say Im being shallow, that I should focus on how good a person she is. Im not bad looking, but I could lose 10 pounds, so I see their point. But Im a smart guy with a promising career ahead, and I really desire an attractive woman (at least better than average).. Fearing Big Bertha. Careful what you wish for. If this girls true to her pictures, shell show up on your date with a blurry face and a body that ends where they cropped the photo.. Ignore your friends and family, who wont be the ones sleeping with your girlfriend (well, presumably). Its anything but shallow to make sure a woman has the looks you need to be hot for her. In fact, one of the unintentionally crueler things people do is tell themselves ...

The adsorption of S2 on the Si(1 1 1)-(7 × 7) surface and the interaction of copper and sulfur on this sulfur-terminated Si(1 1 1) surface have been studied using synchrotron irradiation photoemission spectroscopy and scanning tunneling microscopy. The adsorption of S2 at room temperature results in the passivation of silicon dangling bonds of Si(1 1 1)-(7 × 7) surface. Excessive sulfur forms Sn species on the surface. Copper atoms deposited at room temperature directly interact with S-adatoms through the formations of Cu-S bonds. Upon annealing the sample at 300 °C, CuSx nanocrystals were produced on the sulfur-terminated Si(1 1 1) surface. © 2010 Elsevier B.V. All rights reserved ...
Method: A randomized controlled trial. Subjects are psycho-geriatric nursing home residents (N=80) admitted in two Dutch nursing homes. The intervention consists of half-body UVB irradiation, two times a week, over 6 months, with 0.5 minimal erythemal dose (MED), compared to oral supplementation with 5600 IE cholecalciferol once a week. Main outcome measure is Well-being, monitored by asking the nursing staff to complete the CMAI (Cohen-Mansfield Agitation Inventory) and the Cornell scale for depression in dementia at 0, 3 and 6 months ...
Lymphoma is one of the most common neoplasms in canine companion animals accounting for upwards of 25% of all canine cancer. Chemotherapy results in high remissions rates but poor overall survival even with aggressive therapy. Lymphoma is extremely radiosensitive; however, incorporating full and half-body radiation therapy results in harmful toxicity to health tissue. In human medicine, full and half-body radiation has been abandoned for advanced Involved-Field Radiotherapy (IFRT) techniques utilizing advanced radiotherapy systems designed to kill diseased tissue while sparing normal tissues. As the technology becomes available in veterinary medicine, this treatment capability will also become available; however, there are no studies in the veterinary literature specifically interrogating this strategy. While demonstrable benefit is the ultimate clinical endpoint, it is critical to ensure safe implementation of IFRT for use in canine patients. To test feasibility and safety, Dr. Deveau will
Abstract: The dual-specificity protein kinase monopolar spindle 1 (Mps1) is a central component of the mitotic spindle assembly checkpoint (SAC), a sensing mechanism that prevents anaphase until all chromosomes are bioriented on the metaphase plate. Partial depletion of Mps1 protein levels sensitizes transformed, but not untransformed, human cells to therapeutic doses of the anticancer agent Taxol, making it an attractive novel therapeutic cancer target. We have previously determined the X-ray structure of the catalytic domain of human Mps1 in complex with the anthrapyrazolone kinase inhibitor SP600125. In order to validate distinct inhibitors that target this enzyme and improve our understanding of nucleotide binding site architecture, we now report a biophysical and structural evaluation of the Mps1 catalytic domain in the presence of ATP and the aspecific model kinase inhibitor staurosporine. Collective in silico, enzymatic, and fluorescent screens also identified several new lead quinazoline ...
Overall survival was improved in metastatic pancreatic cancer patients through an innovative immunotherapy strategy in a multicenter study reported at the 2014 Gastrointestinal Cancers Symposium.. This is the first time a randomized study has shown that immunotherapy is effective in pancreatic cancer, said Dung T. Le, MD, Assistant Professor of Medicine at the Johns Hopkins University Sidney Kimmel Comprehensive Cancer Center, Baltimore. This is just a first step, and we believe well be able to take this approach further. . The novel treatment, which may be better tolerated than standard chemotherapy, involves two different anticancer vaccines: GVAX Pancreas followed by CRS-207. GVAX is composed of pancreatic cancer cells that have been genetically modified to secrete granulocyte-macrophage colony-stimulating factor (GM-CSF), which stimulates the immune system. GVAX is given with low-dose cyclophosphamide to inhibit regulatory T cells and boost the vaccines efficacy. The second ...

INTRODUCTION: Patients with malignant pleural mesothelioma (MPM) may develop painful procedure tract metastasis (PTM) at the site of previous pleural interventions. Prophylactic radiotherapy has been used to minimise this complication; however, three small randomised trials have shown conflicting results regarding its effectiveness. The surgical and large bore procedures in malignant pleural mesothelioma and radiotherapy trial (SMART Trial) is a suitably powered, multicentre, randomised controlled trial, designed to evaluate the efficacy of prophylactic radiotherapy within 42 days of pleural instrumentation in preventing the development of PTM in MPM. METHODS AND ANALYSIS: 203 patients with a histocytologically proven diagnosis of MPM, who have undergone a large bore pleural intervention (thoracic surgery, large bore chest drain, indwelling pleural catheter or local anaesthetic thoracoscopy) in the previous 35 days, will be recruited from UK hospitals. Patients will be randomised (1:1) to receive
A considerable body of evidence suggests that Fc-dependent functions improve the capacity of broadly neutralizing antibodies (BnAbs) to protect against and control HIV-1 infection. This phenomenon, however, has not been formally tested in robust cell-associated macaque simian-human immunodeficiency virus (SHIV) models with newer-generation BnAbs. We studied both the WT BnAb PGT121 and a LALA mutant of PGT121 (which has impaired Fc-dependent functions) for their ability to protect pigtail macaques from an i.v. high-dose cell-associated SHIVSF162P3 challenge. We found that both WT and LALA PGT121 completely protected all 12 macaques studied. Further, partial depletion of NK cells, key mediators of Fc-dependent functions, did not abrogate the protective efficacy of PGT121 in 6 macaques. Additionally, in animals with established SHIVSF162P3 infection, SHIV viremia levels were equally rapidly reduced by LALA and WT PGT121. Our studies suggest that the potent neutralizing capacity of PGT121 renders ...
One of the first studies describing the use of Theratope in clinical practice had been published in 1996 by MacLean and colleagues.2 They described how Theratope produced clinically significant antibody titers and a marked increase in survival in patients with breast cancer.2 Vaccine activity was also associated with expression of CD69+ lymphocytes and CA27.29 (MUC-1), a high-molecular-weight glycoprotein rich in serine and threonine residues.3 Two phase II trials compared the use of low-dose cyclophosphamide with and without Theratope and found that there was a statistically significant increase in survival among patients treated with the STn-KLH vaccine (overall median survival, 19.1 months; n=50), compared with those patients not treated with the vaccine (overall median survival, 9.2 months; n=104).4,5 The trials reported minimal toxic effects with mild injection-site reactions and flulike symptoms.4,5 Sandmaier et al6 tested Theratope in vitro in 33 high-risk patients with breast cancer ...
These absurd misbeliefs are generally expressed without particular concern, but, less frequently, the patient may be annoyed or demand insistently the removal of the limb (misoplegia). 69-72 These processes include inattention to the left hemibody, reduced sensory feedback, impaired attention resources, defect in body schemata or in selfawareness, internal representation, mental flexibility, and diffuse mental deterioration. 68 Furthermore, none of these theories-either psychological, or denial or emotional changes-adequately explains the more common occurrence of anosognosia with right hemisphere stroke. Empathy loss is probably at the origin of emotional and personality disorders due to frontal lesions. ACUTE BEHAVIORAL CHANGES RELATED TO IMPAIRMENT OF INSTRUMENTAL FUNCTIONS 37 Emotional and personality disorders in patients with frontal lobe lesions vary from the absence of tact and inhibition, inappropriate familiarity, childish behavior, and sexual disinhibition, to lack of initiative and ...

I was a stroke victim and paralyzed for more than 3 years now (more reason was stress due to studies and my blood pressure went up). Based on me , I am more or less 80%-85% well because I can walk now and do some light work at home, go to the grocery store on my own, etc. than when I started as a bed-ridden patient. I am half-body paralyzed, right side, with little movement in the arms and hands but not opening the fingers (I can close them though). I can talk with a slur but still understandable. ...

A 62-year-old right-handed woman of normal psychiatric history presented a simple focal epileptic seizure including a complex sensation characterized by feeling the presence of several members of her family in the immediate environment, associated with paresthesia of the right hemibody (excluding the face). The feeling of presences and the paresthesia (numbness) appeared concomitantly and lasted in total several minutes. The episode occurred while she was sitting alone on the sofa of her living room and immediately felt the presence of four persons in her frontal space. She did not see or hear these persons (no visual or auditory hallucinations), but felt vividly their presence in her peripersonal [within reach] and near extrapersonal space [just outside of reach]. She recognized them as close family members. Closest was her grand-daughter who was sitting on the floor immediately in front of her, without any left or right lateralization in relation to her body, whereas the three other persons, ...
article{74c6c5e2-5f8f-4bce-afbd-04daa1c85807, abstract = {Background/Objectives: In previous studies we observed a high incidence of elevated plasma homocysteine (tHcy) concentrations in psychogeriatric patients. Plasma tHcy is increased in folate deficiency. Folates are sensitive to oxidative stress. Oxidative stress, caused by inflammatory processes, could represent an endogenous reason for folate deficiency, even when the dietary intake of the vitamin is within the recommended range. It has been suggested that oxidative stress rather than insufficient folate intake causes hyperhomocysteinemia in different forms of psychogeriatric diseases. In the present study we investigated the association between plasma tHcy and C-reactive protein (CRP), a sensitive marker for inflammation, in psychogeriatric patients. Methods: Plasma tHcy, its main determinants, and CRP were measured in plasma and serum of 132 psychogeriatric patients (53 males and 79 females, mean age 75 12 years). Results. In the ...
Heat shock of dormant spores of Bacillus stearothermophilus ATCC 7953 at 100 or 80 degrees C for short times, the so-called activation or breaking of dormancy, was investigated by separating the resulting spores by buoyant density centrifugation into a band at 1.240 g/ml that was distinct from another band at 1.340 g/ml, the same density as the original spores. The proportion of spores at 1.240 g/ml became larger when the original dormant spores were heated for a longer period of time, but integument-stripped dormant spores were quickly and completely converted to spores with a band at 1.240 g/ml. The spores with bands at both 1.240 and 1.340 g/ml were germinable faster than the original dormant spores and thus were considered to be activated. The spores with a band at 1.240 g/ml, which were considered to be fully activated, were apparently permeabilized, with a resulting complete depletion of dipicolinic acid, partial depletion of minerals, susceptibility to lysozyme action, permeation of the ...
Spontaneous fluctuations in functional magnetic resonance imaging (fMRI) signals of the brain have repeatedly been observed when no task or external stimulation is present. These fluctuations likely reflect baseline neuronal activity of the brain and correspond to functionally relevant resting-state networks (RSN). It is not known however, whether intrinsically organized and spatially circumscribed RSNs also exist in the spinal cord, the brains principal sensorimotor interface with the body. Here, we use recent advances in spinal fMRI methodology and independent component analysis to answer this question in healthy human volunteers. We identified spatially distinct RSNs in the human spinal cord that were clearly separated into dorsal and ventral components, mirroring the functional neuroanatomy of the spinal cord and likely reflecting sensory and motor processing. Interestingly, dorsal (sensory) RSNs were separated into right and left components, presumably related to ongoing hemibody processing of
INTRODUCTION & OBJECTIVES: Adaptive deep brain stimulation (aDBS) uses feedback from brain signals to guide stimulation. A recent acute trial of unilateral aDBS showed that aDBS can lead to substantial improvements in contralateral hemibody Unified Parkinsons Disease Rating Scale (UPDRS) motor scores and may be superior to conventional continuous DBS in Parkinsons disease (PD). We test whether potential benefits are retained with bilateral aDBS and in the face of concurrent medication. METHODS: We applied bilateral aDBS in 4 patients with PD undergoing DBS of the subthalamic nucleus. aDBS was delivered bilaterally with independent triggering of stimulation according to the amplitude of β activity at the corresponding electrode. Mean stimulation voltage was 3.0±0.1 volts. Motor assessments consisted of double-blinded video-taped motor UPDRS scores that included both limb and axial features. RESULTS: UPDRS scores were 43% (p=0.04; Cohens d=1.62) better with aDBS than without stimulation. Motor

Review provided by Dr. Brian Draper, MBBS MD FRANZCP. This 3-day meeting was a joint meeting of the RANZCP Faculty of Psychiatry of Old Age (FPOA) and the International Psychogeriatric Association (IPA). IPA ran four pre-conference workshops and FPOA organised the following 2-day meeting with a theme of Capacity, Creativity and Ageing in Clinical Practice. Overall the meeting was a great success with 356 people attending from 15 countries; indeed the FPOA part of the meeting was sold out with over 300 registrants while the IPA workshops attracted 252 registrants.. Queenstown is located on New Zealands South Island on the shores of Lake Wakatipu and surrounded by mountains. In winter it is the hub of the New Zealand ski fields. Overnight spring snow before the workshops turned the mountains into a picture postcard backdrop to the meeting with extensive snow-capped peaks framing the lake.. The IPA pre-conference workshop day had four workshops. There were two full day workshops: one on ...
Subacute care is specialised multidisciplinary care in which the primary need for care is optimisation of the patients functioning and quality of life. A persons functioning may relate to their whole body or a body part, the whole person, or the whole person in a social context, and to impairment of a body function or structure, activity limitation and/or participation restriction.. Subacute care comprises the defined care types of rehabilitation, palliative care, geriatric evaluation and management and psychogeriatric care.. A multidisciplinary management plan comprises a series of documented and agreed initiatives or treatments (specifying program goals, actions and timeframes) which has been established through multidisciplinary consultation and consultation with the patient and/or carers.. While psychogeriatric care is a subspecialty of mental health, it is an established component of subacute care. Therefore, if a patient meets the definition of psychogeriatric care, then the ...
