CORRESPONDENCE. Clinical haematology training in South Africa for fellows with the primary specialty of internal medicine - whats in a name? To the Editor: Clarity with regard to what training in clinical haematology in South Africa involves and aims to achieve can be elusive. Clinical haematology trainees can have primary specialties of Internal Medicine or Paediatrics or Pathology (Haematology). I discuss the situation of those with the primary specialty of Internal Medicine. In South Africa, clinical haematology combines clinical and laboratory haematology into one subspecialty. Regulation 11(1) of Government Notice No. R.590 of 29 June 2001 (Government Notice)1 states that a clinical haematologist shall have completed 4 years of training as a registrar in Medicine and then 2 years in Pathology (Haematological). In practice, interpretations vary. The Clinical Haematology regulations of the College of Physicians2 agree with the Government Notice1 on who is a clinical haematologist (CH). ...

Physicians who specialize in hematology are known as Hematologists. Their routine work mainly includes the care and treatment of patients with hematological diseases, although some may also work at the hematology laboratory viewing blood films and bone marrow slides under the microscope, interpreting various hematological test results and blood clotting test results. In some institutions, hematologists also manage the hematology laboratory. Physicians who work in hematology laboratories, and most commonly manage them, are pathologists specialized in the diagnosis of hematological diseases, referred to as hematopathologists. Hematologists and hematopathologists generally work in conjunction to formulate a diagnosis and deliver the most appropriate therapy if needed. Hematology is a distinct subspecialty of internal medicine, separate from but overlapping with the subspecialty of medical oncology. Hematologists may specialize further or have special interests, such as few choose to work in field ...

On Sunday, I took Yirmiyahu to the eye doctor for an exam and was happy to hear that his eyes look great! I dont have to go back for another year.. Then this morning I took Yirmiyahu to the pediatric hematologist for a follow-up visit.. A number of people have asked me why he needs to go to a hematologist (a hematologist is a blood doctor). The reason is that when he was born, he had an extremely high leukocyte count - 95,000. About 10% of babies with Trisomy 21 are born with this condition, known as transient leukemia. The hematologist told me, after we got the results of the genetic testing, that there had never been a question in his mind that Yirmiyahu had T21 due to his elevated leukocytes - he said its a medical phenomenon seen only in the Down syndrome population. Yirmiyahu received very strong antibiotics to counter this within a short time of being born, and we were very happy to see his leukocyte count go down to the normal range of 20,000 within a few days.. However, children who ...

The purpose of this study was to compare the charges and payments associated with bone marrow aspiration and biopsies performed by hematology/oncology specialists versus interventional radiology specialists at Bassett Medical Center located in a rural area of New York State. Charges pertained to what the hospital charged for the procedure and payment refers to the reimbursement the hospital received. Our secondary objectives were to compare specimen quality by procedure and to determine whether body mass index was associated with which specialist performed the procedure. The median charge was significantly higher in the interventional radiology group ($5254 USD) compared to the hematology/oncology group ($413 USD), p | 0.0001. Median payments were also higher in the interventional radiology ($1392 USD) compared to the hematology/oncology group ($1109 USD), p | 0.0001. Adequacy of the samples obtained by either profession was similar. Disease process was not associated with adequacy of the sample. There

Background: Various types of automated hematology analyzers are used in clinical laboratories. Here, we performed a side-by-side comparison of five current top of the range routine hematology analyzers in the setting of a university hospital central laboratory.. Methods: Complete blood counts (CBC), differentials, reticulocyte and nucleated red blood cell (NRBC) counts of 349 patient samples, randomly taken out of routine diagnostics, were analyzed with Cell-Dyn Sapphire (Abbott), DxH 800 (Beckman Coulter), Advia 2120i (Siemens), XE-5000 and XN-2000 (Sysmex). Inter-instrument comparison of CBCs including reticulocyte and NRBC counts and investigation of flagging quality in relation to microscopy were performed with the complete set of samples. Inter-instrument comparison of five-part differential was performed using samples without atypical cells in blood smear (n=292). Automated five-part differentials and NRBCs were additionally compared with microscopy.. Results: The five analyzers showed a ...

This study has several limitations. First, it is a cross-sectional study with self-reported data, including several questions that ask fellows to recall experiences or perceptions from earlier stages in their training. This introduces the possibility that fellows could experience recall bias or project their current interests or plans onto past experiences. We sought to reduce the effect of recall bias by including questions about medical school and residency experiences only for first- and second-year fellows whose memories are more recent, but it is impossible to eliminate it entirely. Also, this study examines associations between fellows reported training and mentorship experiences and their career plans only. It cannot provide definitive evidence of a causal relationship between training and mentorship experiences and plans to enter hematology-only careers-or fully account for the possibility that certain training experiences are associated with career plans because fellows choose them ...
Prognostic markers, such as NPM1, Flt3-ITD, and cytogenetic abnormalities have made it possible to formulate aggressive treatment plans for unfavorable acute myeloid leukemia (AML). However, the long-term survival of AML with unfavorable factors remains unsatisfactory. The latest data indicate that the standard dose of daunorubicin (DNR) at 45 mg/m2 is inferior to high dose 90 mg/m2 for induction therapy. The rates of complete remission and overall survival are significantly better in the high dose induction regimen. New regimens exploring the new liposomal encapsulation of Ara-C and DNR as well as addition of gemtuzumab ozogamicin monoclonal antibody have been studied. New agents, including the nucleoside analogues (clofarabine, sapacitabine, elacytarabine), FLT3 inhibitor (sorafenib), farnesyl-transferase inhibitor (tipifarnib), histone deacetylase inhibitor (vorinostat), lenalidomide, as well as DNA methyltransferase inhibitors (decitabine, azacitidine), were recently reported for AML treatment in

H&O What have studies shown about direct-to-consumer advertising of medical treatments?. LS A few studies have evaluated direct-to-consumer advertising of medical treatments, but they are not the most authoritative because they are not easy to conduct. Gregory A. Abel, MD, MPH, has published studies on patient awareness and prescribing habits.1,2 For the study on patient awareness, Dr Abel mailed questionnaires to patients who were receiving cancer treatments to assess their awareness of advertising. The study found that patients were highly aware of this type of advertising, and that it prompted a modest amount of discussion with their healthcare provider. Few patients, however, reported changes in therapy. The study on prescribing patterns found that for every million dollars spent on direct-to-consumer advertising for aromatase inhibitors, there was an associated increase 3 months later in appropriate prescriptions, but no significant change for inappropriate prescriptions.. In the oncology ...
Hodgkin lymphoma (HL) is a cancer of the lymph system and is diagnosed by the identification of a characteristic cell under the microscope (the Reed- Sternberg cell). Hodgkin lymphoma typically begins in the lymph nodes in one region of the body and then spreads through the lymph system in a predictable manner. Fortunately, if treated appropriately HL is a curable cancer for the majority of patients. More than 75% of all newly diagnosed patients can be cured with combination chemotherapy, precision cancer medicines and/or radiation therapy. (1) Mortality has fallen more rapidly than for any other cancer over the last 5 decades. (2). ...

Since its discovery, improvements in treating Castleman disease and its variants have centered on interleukin-6 (IL-6). IL-6 was discovered from T-cell factors (BCDF or BSF-2), which induced B-cell maturation. Most symptoms of the plasma cell variant of Castleman disease are linked to the hyperfunction of IL-6, constitutively produced in the affected lymph nodes (1989), suggesting IL-6 is key in the pathogenesis of multicentric Castleman disease (MCD). Scientists recommend testing for Pearson syndrome in patients with congenital anemia. Some babies diagnosed with and treated for a bone marrow failure disorder, called Diamond Blackfan Anemia, may actually be affected by a very rare anemia syndrome that has a different disease course and treatment, say scientists from Dana-Farber/Boston Childrens Cancer and Blood Disorders Center.. Genetic analysis of DNA from 175 patients believed to have Diamond Blackfan Anemia, identified eight that showed hallmarks of Pearson Marrow Pancreas syndrome, according to research presented at the 55th annual meeting of the American Society of Hematology.. The treatment choices are difficult in both syndromes, but getting the diagnosis correct is crucial, said Suneet Agarwal, MD, PhD, a pediatric hematologist/oncologist at Dana-Farber/Boston Childrens. Some patients with Diamond Blackfan will respond to steroids, but theres no reason to give steroids to someone with Pearson Syndrome -- and they could make things ...

Transfusion medicine involves many hot-seat decisions that span a wide patient spectrum-from surgery and hematology/oncology to obstetrics and neonatology. Transfusion Medicine in the Hot Seat is a useful text for pathology trainees and an educational resource for pathologists who cover transfusion medicine. The text is divided into three sections: antibodies, blood components, and complications. Each section presents numerous short cases that illustrate real-world transfusion scenarios, with 26 cases total. The short-case format makes the information easily accessible and can serve as the basis for a transfusion medicine curriculum in clinical pathology.
