Angus Mac MacGyver was a secret agent, hero of an ABC action-adventure television series that ran from 1985-1992. MacGyver was a troubleshooter who used his scientific training and existing resources in a creative way, to create simple albeit ingenious solutions to overcome unexpected problems or to resolve difficult situations. His trademark was to be resourceful and innovative. In one episode the hero described his drive for resourcefulness in the following quote "...the tighter your plan the more likely you are to run into something... unpredictable"[1]. The eponymous MacGyver Effect has come into common parlance since the 1990s as a result of the popular television series. The MacGyver Effect is the ability to apply scientific principles and use everyday things in an innovative way to create what is needed to overcome obstacles thrown into the path by another party. This paper sets out to show how the MacGyver Effect operates within health services research.. Health services research is ...

After a brief introduction into the general field of health services research, a large section deals with the specific issues arising when epidemiological or statistical methods are used to study health services. This is followed by sections describing the main fields of investigation which are usually thought of as pertaining to the wider realm of health services research. These are studies of demand, need, utilization, and access to health services which have the interface between the patient and health services in common. The next section describes the importance of financial resources, structure, and organization for the delivery of effective and efficient health care. This is followed by a description of the processes and outcomes of health care, including concepts such as effectiveness and appropriateness of care and their use, for example, in physician profiling or in hospital rankings. In the section on outcomes, special emphasis is put on health status measurement and the evaluation of ...

The "era of health systems strengthening" can be dated to a ministerial summit held in Mexico City in 2004, which resulted in the Mexico Statement on Health Research and signaled the emergence a new era for global health. It called on national governments to commit more funding to research on health systems and other funders of health research to "support a substantive and sustainable program of health systems research aligned with priority country needs" [1].. This call came shortly after the launch of large-scale disease-specific initiatives to address HIV, malaria, and tuberculosis, and just as global mobilization began in earnest to address the Millennium Development Goals (MDGs). At least three major factors aligned to drive a shift in attention and resources to health systems strengthening: realization that weak health systems impede the achievement of health MDGs, growing evidence on the adverse effects of global health initiatives on national health systems, and the recognition by global ...
Health services researchers support the identification, implementation and uptake of the most appropriate health care that is acceptable, accessible, safe, effective and value for money. To identify appropriate health care, health services researchers gather evidence to inform questions of who should deliver what health care in what settings to which patients. Such evidence includes data from clinical trials to determine what health care delivers the best patient outcomes, but also qualitative research to better understand why different ways of delivering health care produce different outcomes. As an example, Jodi Gray and colleagues interviewed consumers to assess the acceptability of a nurse-led obesity management program, which highlighted the benefits of a "non-judgemental, non-prescriptive way to learn about weight management in an environment that was convenient, accessible and trusted".. The identification of what is appropriate health care does not necessarily mean appropriate health ...

Topics addressed in this report were suggested at a meeting of staff from the National Center for Health Services Research and Health Care Technology Assessment (NCHSR) and other Public Health Service representatives held in 1988 to update the Public Health Services plan for the prevention and control of human immunodeficiency virus (HIV). Current epidemiologic and economic information about Acquired Immune Deficiency Syndrome (AIDS) and other HIV-related illnesses is reviewed, highlighting the complexity of issues and gaps in knowledge that should be addressed in grant research. Priority research issues and examples of substantive research questions are presented. Issues are identified that extend beyond health economics and the costs and financing of services. Researchable topics are outlined with respect to health status measures and information systems, access and barriers to care, quality of care, health systems analysis, cost effectiveness of alternative service and

Despite growing recognition of the need for qualitative methods in health services research, there have been few attempts to define quality standards for assessing the results. This article acknowledges the desirability of a plurality of standards. However, it is argued that three interrelated criteria can be identified as the foundation of good qualitative health research: interpretation of subjective meaning, description of social context, and attention to lay knowledge. These criteria can be examined in relation to different dimensions of any research report, including theoretical basis, sampling strategy, scope of data collection, description of data collected, and concern with generalizability or typicality. But if the concern is with the appropriateness of care and with understanding the factors that shape lay and clinical behavior, then these criteria must form the basis of a hierarchy of qualitative research evidence.. ...

Webster et al. [14] review the literature evaluating service delivery systems for malaria related interventions, including insecticide-treated nets and intermittent preventive treatment in pregnancy. They reflect on the complexity of these systems and indicate that evaluation needs to look comprehensively at the multiple steps in the delivery process and at the multiple factors influencing it. Their proposed method for evaluating service delivery arrangements could be achieved through the insertion of questions in routine household surveys about the location and mechanism of service provision to aid in attribution of coverage outcomes to specific delivery systems.. The collection as a whole also points to the need to develop and refine the methodology of health policy and systems research for addressing scaling up. The papers adopt a range of study designs and analytical approaches - cross-sectional surveys, Markov and financial modelling techniques, case studies using a combination of document ...

Population health research seeks to improve peoples health through a better understanding of the ways in which social, environmental, occupational and economic factors can influence health status and ultimately reduce the numbers seeking acute medical interventions or requiring care for chronic conditions. Population health research is a relatively new term that is considered to include, but be distinct from, traditional definitions of public health, health promotion and social epidemiology. In general, it can be viewed as a field which analyses health outcomes, patterns of health determinants and policy interventions that link them ...

There is good evidence showing that cognitive behavioural therapy (CBT) can and does help many people with depression. However, it does not help everyone all the time. For people experiencing moderate or severe depression, there is an alternative treatment, called Counselling for Depression (CfD). Service evaluation evidence indicates that CfD is as effective as CBT but we need to test whether this is really the case by conducting a scientifically rigorous trial. The PRaCTICED trial has therefore being undertaken to determine the clinical and cost-effectiveness of CfD compared with CBT as delivered in primary care for clients with moderate or severe depression. It is a pragmatic trial as it is being carried out within the routine setting of the Sheffield IAPT service. Patients selected for the trial will stand an equal chance of receiving either CfD or CBT and focuses on patients in the IAPT service who meet a specific threshold of either moderate or severe depression ...

The prevalence of hepatitis C virus (HCV) infection among VA patients is 3 times higher than in the general population. Recent VA data indicate that only about 14% of all HCV-infected VA patients have ever received antiviral therapy. Barriers to receiving antiviral treatment include factors such as pre-existing psychiatric illness, ongoing substance abuse, and other medical co-morbidity. Using a new protocol-based model of care that centers around integrating psychiatric care and case management into VA HCV clinics may increase treatment rates, improve rates of sustained viral response (SVR), and improve health outcomes for veterans with hepatitis The HCV study aims to determine the effectiveness of this protocol-based integrated care model for increasing treatment rates and the number of patients who receive successful antiviral treatment. The study will also assess the effects of an integrated care model on patient involvement in care.. The HCV study uses a prospective patient level randomized ...
