An operator system and related methods for automatically controlling access barriers including a controller associated with at least one access barrier and a transceiver associated with the controller for transmitting and receiving operational signals. The system also includes at least one proximity device capable of communicating operational signals with the transceiver based upon a position of the proximity device with respect to the barrier and/or the operational status of a vehicle carrying the proximity device, wherein the controller monitors the operational signals and controls the position of the access barrier based upon the operation signals. Such a system allows for hands-free operation of the access barrier.
Brussels, 16 October 2014. On 16 October 2014, EULAR organised the conference Analysing how to reduce the access barriers to health care for people with chronic diseases. The event was hosted by Member of the European Parliament Takis Hadjigeorgiou (Cyprus), being part of the conference held in the European Parliament. The main goal of the event was to discuss on main issues affecting access to health care as well as to develop policy recommendations to EU and national policy makers.. Access to quality health care is one of the main concerns of people with rheumatic and musculoskeletal diseases (RMDs) and other chronic conditions. They often have to cope with a number of access barriers, such as the absence of science-based standards of care; the late referral to specialists (and often the insufficient number of specialised medical facilities and personnel); the long waiting lists; the high cost of therapies; or the lack of sufficient and adequate information for patients, to mention only few ...

Over the past four decades, the Indonesian health care system has greatly expanded and the health of Indonesian people has improved although the rich-poor gap in health status and service access remains an issue. The government has been trying to address these gaps and intensify efforts to improve the health of the poor following the economic crisis in 1998. This paper examines trends and levels in socio-economic inequity of health and identifies critical factors constraining efforts to improve the health of the poor. Quantitative data were taken from the Indonesian Demographic Health Surveys and the National Socio-Economic Surveys, and qualitative data were obtained from interviews with individuals and groups representing relevant stakeholders. The health of the population has improved as indicated by child mortality decline and the increase in community access to health services. However, the continuing prevalence of malnourished children and the persisting socio-economic inequity of health suggest
KIWAYUU, Coastal Kenya - The story of mother-of-six Aisha Tiro Bahero, 30, is not only one of resilience and determination, but also of how access to maternal and child health services is critical in saving womens lives and empowering them to lead productive and healthy lives.. Aisha lives in Kiwayuu, a small island located in the eastern part of the Lamu Archipelago in Coastal Kenya. The only health facility on the island was established two years ago by the Lamu County Government, and it serves 600 inhabitants. Prior to its inception, patients, including expectant mothers would travel to Lamu Island, a seven-hour trip by dhow or an hours trip by motorboat.. I got married when I was barely 18 years old and I delivered my first child at home, says Aisha. Her three subsequent deliveries also took place at home.. As a marginalized community, we were ignorant about the importance of antenatal care, largely because we didnt have a health facility in Kiwayuu.. The community relied heavily on ...

Equity in access to health care services is a central policy goal in most publicly funded health care systems. Access is a complex concept that can be understood as a fit between the demand for health care services among the population and the supply of care by providers. To be accessible, health care services need to be affordable, contain the appropriate forms of treatment in relation to medical needs and be physically situated so that patients can get to them [1, 2]. An important component of access is geographical accessibility, which can be understood as the distance, or travel time, between patients and health care providers. Equity in geographical accessibility in its simplest form can be understood as all citizens having a similar, or minimum, travel distance to the nearest health care provider [3].. Inequity in geographic access to health care is a well-known problem in many developing countries, where health care services are often underdeveloped, especially in rural areas, and ...

Using the framework of multidisciplinary best practice recommendations promoted by the National Collegiate Athletic Association, this study used a mixed-methods approach to investigate Division I student-athletes perceived access to and satisfaction with mental health service availability. Participants were asked about their satisfaction with direct (e.g., counseling, psychiatry, assessment) and indirect (e.g., mental health outreach, educational workshops) service availability, both on campus and within athletics. Results from a researcher-generated survey indicated that participants were moderately satisfied with service availability in each of the four contexts. Hierarchical multiple regressions revealed that student-athletes satisfaction was predicted by different factors for each service type-location combination. Qualitative data contained requests for more athlete-centered mental health services as well as more preventative outreach in general. These data provide a foundation for ...

Vietnamese patients are disproportionately represented in type 2 diabetes mellitus statistics and also incur high rates of diabetes complications. This situation is compounded by limited access to health care. The aim of this project was to gain a deeper understanding of the difficulties Vietnamese patients experience when accessing services and managing their type 2 diabetes mellitus, and to identify factors that are important in promoting health service use. Three focus groups with 15 Vietnamese participants with type 2 diabetes mellitus, 60 to |70 years of age, were conducted in Vietnamese. Open-ended questions were used and focussed on experiences of living with diabetes and access to healthcare services in the Inner Northwest Melbourne region. Audio recordings were transcribed and then translated into English. Data were analysed using a thematic analysis framework. Findings indicate four main themes, which together provide some insight into the experiences of living with diabetes and accessing

Develop a multi-lingual community services workforce. Having staff able to communicate in community languages is one important way of improving access for people from diverse language backgrounds. For instance, traineeships and scholarships can encourage people with English as a second language to work in community services.. Embed culturally appropriate practice and strengthen language services. People from culturally diverse communities are more likely to access services if staff have been trained in cultural safety and trauma-focused responses. The Victorian Government can fund affordable training opportunities, improve assistance to organisations to provide materials in community languages (including Auslan), and expand access to interpreter and translation services, to improve service access.. Embed co-design and participatory approaches. Involving service users in program design and delivery is critical to developing person-centred services. Consumer participation needs to be resourced and ...
For a station in a data communication network having a predefined limit of congestion for handling data immediately undergoing transmission through the station, a system for improving service given to network users requesting new access services to support new transmissions through the same station while the predefined congestion limit is exceeded. This system contains: a first element for determining when the predefined limit is exceeded by existing data transmissions through the station; and a second element, responsive to requests for new access services that are received while the predefined limit is being exceeded for: concurrently processing the request for new service and reducing the quality of service (QoS) to selected users. The reductions in QoS generally have the effects of reducing the speed/priority of handling of data being transmitted to or from the selected users, and of freeing up of sufficient bandwidth to accommodate the requested new access services. The system includes a third

In general, many users experienced barriers with regards to the acceptability of services, particularly in the form of provider attitudes and the impact of diagnoses-based organisation of services on confidentiality and poor communication. Acceptability of service delivery was compromised for service providers through negative user behaviour, language as barriers, short contact sessions and fragmentation of services.. Discussion. Considering the historical context of a fragmented and inefficient healthcare system with poor capacity,8 remarkable achievements in health care service delivery were observed in the PHC health care facilities of Gugulethu. From a service providers perspective the results demonstrate available, accessible, affordable and adequate services in the study setting through efficient organisation of services according to the DHS guidelines and policies. However, user perspectives differed. Although services were accessible, challenges were experienced with regard to ...

Weve seen solid investment in Queensland feedlot capacity as supply chains have moved to secure cattle supply to meet their brand commitments and customers demand for high quality beef 365 days a year, irrespective of environmental conditions.. To attract future investment and enable the feedlot sector to expand in the face of climate change predictions the proposed GABORA Water Plan must give feedlot operators options for better access to water resources, Mr Mulders said.. At the moment the proposed plan effectively restricts lot feeders to access water from the general reserve allocation.. The proposed allocation for the general reserve has been significantly reduced and some of the allocations are in deep aquifers requiring very expensive bores to be drilled.. ...

The care of children matters to all of us. But today there are millions of children around the world that are not cared for adequately. It is a global crisis and one that will only get worse. There are an estimated 151 million children worldwide with either one or both parents dead, with at least 13 million of these children having lost both parents (UNICEF 2013). Adequate care means that children grow up in a permanent, safe and caring family. Many children are separated from their families because they cannot afford to care for them. Other move to attend school or access health services unavailable to them at home. Children that arent cared for adequately are less likely to attend or do well in school, they are less able to access health and other basic services, and they are more likely to engage in anti-social and criminal behaviours. The lack of care affects their physical, emotional and cognitive development and can sometimes even be life-threatening. This in turn has an impact on society
Central government and departments now believe that delivering public services digitally could save billions of pounds annually and make the services better and more flexible for customers. Delivering services digitally means much more than using websites to access services. Each step of the service process needs to take advantage of ways technology can replace manual processes, and information from various sources can be used to inform decision-making. Government needs to be able to get and make use of more comprehensive information, organisations need to work together, and systems have to be able to respond to changes in the needs and behaviours of both customers and staff, and ever changing technologies.. As well as all the wider issues to be addressed when transforming public services, delivering services digitally means managing specific challenges and risks, from integrating legacy IT systems to protecting service continuity and avoiding cyber crime. Achieving such a scale of change ...

Bhavesh Jain, who leads International knowledge linkage programme at ACCESS Health International, and Nehal Jain, Regional Director, Foundation for Research in Health Systems analyse the allocation for health in the current budget and provide some alternatives to finance healthcare.. Despite slowing global economy, Indian economy is growing at an accelerated rate of 7.6 per cent in 2015-16. The Finance Minister, Arun Jaitley in his budget speech, mentioned that healthcare is one of the nine pillars for the development of the country. The budget allocation of Rs 38,892 crores to the health sector for the year 2016-2017 is a welcome move for the country. There has been a 15 per cent increase as compared to last years allocation yet it is not enough.. The Indian healthcare system is characterised by low public spending on health and high out-of-pocket payment by the people at the point of availing healthcare services. Individual households contribute about 70 per cent of the total healthcare costs ...
As with other coronavirus-affected countries, Nepalese medical fraternity also expressed concerns regarding public health strategies of government and hospital readiness in response to upgoing case surge. To gauge such response, we assessed service availability and Infection prevention and control (IPC) status in 110 hospitals situated across seven provinces. An electronic survey was sent out to the frontline clinicians working on those hospitals between 24th March and 7th April 2020; one response per hospital was analyzed. Hospitals were divided into small, medium, and large based on the total number of beds (small:|=15; medium:16-50; large:|50), and further categorized into public, private, and mixed based on the ownership. Out of 110 hospitals, 81% (22/27) of small, 39% (11/28) of medium, and 33% (18/55) of large hospitals had not allocated isolation beds for COVID-19 suspects. All small, majority of medium (89%; 25/28), and 50% of large hospitals did not have a functional intensive care unit (ICU)

The impacts of globalisation and rural restructuring on health service delivery in rural Australia have been significant. In the present paper, it is argued that declining health service access represents a failure of policy. Rural communities across the world are in a state of flux, and Australia is no different: rural communities are ageing at faster rates than urban communities and young people are out-migrating in large numbers. During the past 5 years, rural Australia has also experienced a severe and widespread drought that has exacerbated rural poverty, and impacted on the health and well-being of rural Australians. Australian governments have responded to globalising forces by introducing neoliberal policy initiatives favouring market solutions and championing the need for self-reliance among citizens. The result for rural Australia has been a withdrawal of services at a time of increased need. This paper addresses the social work response to these changes ...

This study, conducted in five rural districts in Afghanistan, used qualitative methods to explore traditional practices of women, families and communities related to maternal and newborn care, and sociocultural and health system issues that create access barriers. The traditional practices discussed include delayed bathing of mothers and delayed breastfeeding of infants, seclusion of women after childbirth, restricted maternal diet, and use of traditional home remedies and self-medication instead of care in health facilities to treat maternal and newborn conditions. This study also looked at community support structures, transportation and care-seeking behaviour for maternal and newborn problems which create access barriers. Sociocultural barriers to better maternal-newborn health include shame about utilisation of maternal and neonatal services, womens inability to seek care without being accompanied by a male relative, and care-seeking from mullahs for serious health concerns. This study also ...
Abstract. Health planners commonly use geographic proximity to define access to health services. However, effective access to case management requires reliable access to a trained, supplied provider. We defined effective access as the proportion of the study population with geographic access, corrected for other barriers, staffing patterns, and medicine availability. We measured effective access through a cross-sectional survey of 32 health facilities in Malawi, Mali, and Zambia and modeled the potential contribution of community case management (CCM). The population living within Ministry of Health (MOH)-defined geographic access was 43% overall (range = 18-52%), but effective access was only 14% overall (range = 9-17%). Implementing CCM as per MOH plans increased geographic access to 63-90% and effective access to 30-57%. Access to case management is much worse than typically estimated by distance. The CCM increases access dramatically, again if providers are available and supplied, and should be

INTRODUCTION. The reasons that lead people to visit a doctor come from a complex interaction of different factors such as demographic, socioeconomic, and psychological aspects, morbidity profiles, and health services availability.2,5 The effect and relative importance of each factor are affected by cultural background, health policies and health care system available.. As lower socioeconomic groups have a higher burden of disease and therefore need more health services, equity is at the heart of the entire health care issue. In Brazil, a universal decentralized and free of charge health care system was created, the Unified Health System (SUS), following the promulgation of the 1988 Federal Constitution. Reforms have occurred since then, but there is evidence that, despite intended universal provision, health services utilization among social groups remains unequal.1 Furthermore, there is limited information about how demographic, socioeconomic, health needs and other factors affect health ...

Law professor and cybercrime expert Orin Kerr published a proposal to amend the Computer Fraud and Abuse Act (CFAA) to address the overcriminalization that he has been at the forefront of identifying and combatting. His current proposal, which very simply but comprehensively addresses a number of problems with the CFAA, is here. By focusing purely on whether the service operator implements technological access barriers, the proposal risks a similar problem to the one that the current statute has, giving server owners plenary authority to criminalize the way members of the public interact with information made available online, but through technological access barriers rather than merely terms of service and employee agreements. ...
BACKGROUND: Unmet health care needs are associated with negative health outcomes, yet there is a paucity of data on this problem among older people. OBJECTIVE: To identify unmet health care needs and associated factors among older people in France. METHODS: This is a cross-sectional population study of people aged 70 years or older in which 2350 respondents were interviewed in 2008-10. During a standardized interview, a nurse examined health problems, functional abilities and use of health care resources. Unmet health care needs were defined as situations in which a participant needed health care and did not receive it. RESULTS: The mean age was 83.2 ± 7.4 years. Almost all participants reporting a chronic disease (98.6%) had consulted a physician in the previous 6 months. Unmet health care needs were found in 23.0% of the sample and mainly consisted of lack of dental care (prevalence of 17.7%), followed by lack of management of visual or hearing impairments (prevalence of 4.4% and 3.1%,

Abstract: BACKGROUND: Threats by enforced Intellectual Property (IP) rights to equitable HIV treatment access by poor populations are impending. India and Chinas policy directions in the field will be crucial in ultimately affecting the affordability and accessibility of antiretroviral (ARV) therapy in the under-served markets. These directions, together with the exploitation level of IP-bound flexibilities and the evolutionary modelling in partnerships and trade agreements between research-based and generic pharmaceutical industry, will also ...
There is considerable inequity in access to high quality outpatient services. Health service utilization data has consistently demonstrated an association between patient characteristics and access barriers for individuals with chronic diseases. For example, ethnic minorities within the United States have been found to be significantly less likely to access outpatient services for asthma, hypertension, diabetes mellitus or congestive heart failure as compared to Caucasians [18]. This trend has also been identified in access to oncology services [19].. The proportion of unmet needs reported by patients is significantly higher for those with chronic diseases and increases with comorbidities [20]. Results from the Canadian Community Health Survey and national hospitalisation data report that unmet needs in samples of people with chronic diseases remain disproportionally high even after controlling for socio-demographic characteristics [20]. Yes, totally agree. This would be big plus for us. But in order for this to happen the ADA needs to lobby for things that will make these people comply with accessibility by making the fines bigger and the slap much harder. They also need to take that word reasonable out of the ADA and make it say it has to meet accessibility and define some rules for accessibility. Till this happens this is going to keep going on. When it comes to public education that is funded by tax dollars this should be something they must do and it be done correctly. We too are tax payers and there forth in this matter should have the same right as our visual counter parts does. JMT ! A growing share of countries across the globe are declaring a commitment to pursuing Universal Health Coverage (UHC) and introducing policies and approaches to advance toward that goal. International donors and multilateral organizations are supporting these initiatives, with UHC under serious consideration for the post-2015 development agenda [1]. Such attention raises the issue of the most appropriate metrics for progress towards universal health coverage. Individual indicators currently used to capture aspects of UHC are myriad. There is however no existing measure that captures multiple dimensions of UHC. Even the composite indicators that have been developed in this area are focused on service coverage. Such composite measures are limited in how they are constructed and what health services are covered. Therefore, existing approaches do not appear to meet the current and future needs of policy makers, who need concise metrics to monitor whether countries are advancing in covering their ...
Children and adolescents in the U.S. have made substantial oral health gains in recent years while the prevalence of dental disease in adults has remained fairly constant, with an increase in the number of seniors with decay, according to a September 2019 report by the Centers for Disease Control and Prevention (CDC).

More than half the population of rural areas worldwide do not have access to basic health care, with four in five rural Africans lacking services, the UN said Monday.. In the first report of its kind, the UNs International Labour Organisation found that the rural-urban divide was omnipresent from the richest countries down to the poorest.. Fifty-six percent of those living in rural areas worldwide were not covered by basic health care against 22% in cities and towns, according to the report, with data from 174 countries.. The results that we found are really shocking. We find that the rural-urban divide is a consistent feature at global, regional and national levels, the reports author Xelia Scheil-Adlung, told a news conference.. The place of residence can be considered as the entry door to access to health care or as the key barrier to be excluded from health care, she said.. An overwhelming 83% of Africans in rural areas were left uncovered, the report said.. The situation is worsened ...
Abstract (Word count: 122/ Character count: 848)While individuals of Mexican origin are the largest immigrant group living in the U.S., this population is also the highest uninsured. Health disparities related to access to healthcare, among other social determinants, continue to be a challenge for this population. The government of Mexico, in an effort to address these disparities and improve the quality of life of citizens living abroad, has partnered with governmental and non-governmental healthcare organizations in the U.S. by developing and implementing an initiative known as Ventanillas de Salud-Health Windows-(VDS). The VDS are located throughout the Mexican Consular network and aim to increase access to healthcare and health literacy, provide health screenings, and promote healthy lifestyle choices among low-income and immigrant Mexican populations in the U.S.

This policy synthesis identifies four major models for organizing primary healthcare that are relevant to the Canadian context. Two models fall into what can be broadly called a communityoriented approach, while the other two fall into what can be called a professional approach. The vision of community primary healthcare is to improve the health of specific geographically defined populations and to contribute to community development by providing a set of required medical, health, social, and community services. Within this vision, there are two types of models: the integrated community model and the nonintegrated community model. These models differ by their degree of integration into other parts of the healthcare system. Several characteristics of their resources, organizational structure, and practices reflect their varying degree of integration. The vision of professional primary healthcare is to deliver medical services to patients who seek these services, or to people who choose to ...

A Dallas-based organization that helps to fund and mentor healthcare startups has chosen KnKtd Behavioral Health, developer of an app to help care providers track patients progress between appointments, as one of 10 members of its 2016 accelerator class.. As a result of his companys selection by Health Wildcatters, KnKtd co-founder and CEO Matthew Holland will spend the next three months in Dallas to receive guidance for the company from high-level business mentors. Other members of the KnKtd team will visit periodically as well.. Consumers who use the KnKtd software platform can access coping strategies for meeting everyday challenges and can send messages to their addiction or mental health care provider. The systems progressive questions to the individual are part of a triggered system that, based on the responses received, can result in a notification to the individuals treatment team and support network. Care providers pay the systems service access fees.. ...
Paul Breckell, Action on Hearing Loss Chief Executive, says: We are pleased that Ofcom would welcome an amendment to the Digital Economy Bill that would ensure access services - subtitles, audio-description and BSL - are included for TV watched on-demand.Unlike traditional TV, there is currently no requirement for TV providers to include access services in their on-demand content. As a result, many services remain inaccessible to people with sensory loss, with over three-quarters of TV on-demand providers still not offering subtitles, despite a year on year growth in audience figures... Legislation in this area is what we have been calling for through our Subtitle it! campaign for some time and Ofcom as the regulator is best placed to ensure equal access for all.. ...
Musculoskeletal disorders have been identified as a major cause of chronic disability in the UK and one of the commonest reasons for referral into secondary care. This is a problem for both patients and organisations, because musculoskeletal disorders can cause pain, disability and socio-economic losses.. Musculoskeletal disorders are the biggest cause of days lost from work. In particular, back pain accounts for almost 120 million days of certified absence from work each year and about 50% of patients who are off work for more than six months may never return to employment again. The lack of early access to appropriate services means that factors that predict chronicity in common musculoskeletal disorders, such as back pain, cannot be addressed sufficiently early, which compounds the problem.. The physiotherapy service at the occupational health and wellbeing centre is an early access service for staff at the Royal Free London. The occupational health physiotherapist treats a wide variety of ...
While Canada has universal health care, there are still many barriers preventing individuals and families experiencing homelessness from accessing health services. The lack of identification - particularly their health ID card - is often cited as the biggest barrier to obtaining health care for people experiencing homelessness. Another significant barrier is following up on prescriptions (due to lack of insurance benefits or inability to pay the co-payment) or ongoing medical home treatment (such as sitz baths, bedrest or wound care).. Other barriers to adequate health care faced by people experiencing homelessness include the daily struggle for the essentials of life. A reliance on drop-ins and shelters for meals does not provide a nutritionally balanced or adequate diet. Living situations (i.e. sleeping outside, squats or crowded shelter) often result in increased disease transmittal. Many shelters require people to leave during the day preventing someone from obtaining needed rest. People ...

Another area affecting employees is health plan benefit design. Employers can ensure that benefit design includes discounts for preferred networks based on high-quality care, generic drug use, mail-order drugs, and other proven money savers. Another Memphis-area employer designed a self-insured health plan for the specific needs of its rural employees. Since preventive care saves money and preserves health later, the plan offers a $0 co-pay for preventive services. The employer also developed educational materials on how to access health care and use appropriate services at the appropriate time ...
