Results: Seropositivity (43%), usually unknown (72% of seropositives), was predicted by injection drug use, alcohol use, older age, and risky sexual behavior. No regular source of care, risky sexual behavior, less case management, and greater age predicted not knowing ones positive status. Health insurance, younger age, alcohol use, perceived bad health and more medical conditions predicted emergency room (ER) use; ER use was less likely among seropositives. Hospitalizations were predicted by more medical conditions and greater percentage of life homeless and were less frequent among African Americans and males. Ambulatory visits were predicted by a regular source of care, case management, more education and perceived bad health; they were less likely among seropositives ...
This study found that older adults receiving a statin after discharge from hospital with IHD realized significant mortality benefits; however, reductions in re-hospitalizations, physician visits, or coronary revascularizations were not demonstrated over the follow-up period of approximately two years.. Thirty-eight percent (38%) of older adults discharged from hospital received statins in our study; this may seem inconsistent with current North American guidelines which suggest that most patients at high-risk for CVD disease should receive treatment with these medications regardless of age [46, 47]. The low utilization of statins in our population may be a reflection of the study period (1997-2001), with other studies reporting utilization of statins in older adults with established disease over this time period describing similar prescribing rates [48-51].. This study demonstrated that older adults with increased markers for disease severity were less likely to be dispensed statins after ...

There is increasing interest in deploying Screening, Brief Intervention, and Referral to Treatment (SBIRT) practices in emergency departments (ED). However, the current literature is inconclusive on whether or not SBIRT practices are cost effective and cost beneficial. In order to answer this question, new analytical methods need to be developed.. The objective of the following study was to pilot a new experimental methodology - modeling costs using a multilevel generalized linear model (GLM) - for studying the impact of SBIRT on healthcare utilization and costs. ...
This national survey was conducted to compare health services utilization and expenditures in 1970 with results of similar studies done in 1953, 1958, and 1964. In the survey, respondents from 3,763 families plus additional older individuals -- a total of 11,619 persons -- were interviewed in 1971. One or more persons in each family provided information regarding use of health services, the cost of such services, and how these costs were met for the calendar year 1970. Information was also collected on perceptions of illness and health, attitudes and opinions about medical care in the United States, and health beliefs. An attempt was made to verify all hospital admissions, physician visits, and insurance reports and claims. These verifications had two purposes: first, to determine if the reported care was, in fact, provided during the survey year, and second, to elicit more precise information than the families were likely to give on diagnoses, costs, kinds of treatment, and sources of payment ...

Asking people how they would seek healthcare in a hypothetical situation can be an efficient way to estimate healthcare utilization, but it is unclear how intended healthcare use corresponds to actual healthcare use. We performed a cross-sectional survey between August and September 2012 among households in Soweto and Klerksdorp, South Africa, to compare healthcare seeking behaviors intended for hypothetical common infectious syndromes (pneumonia, influenza-like illness [ILI], chronic respiratory illness, meningitis in persons of any age, and diarrhea in a child | 5 years old) with the self-reported healthcare use among patients with those syndromes. For most syndromes, the proportion of respondents who intended to seek healthcare at any facility or provider (99-100%) in a hypothetical scenario exceeded the proportion that did seek care (78-100%). More people intended to seek care for a child | 5 years old with diarrhea (186/188 [99%]) than actually did seek care (32/41 [78%], P | 0.01). Although most

Individuals living in rural mountain areas tend to use health services less to manage discomfort or illness. This study aims to identify the variables that best explain the health service utilization of a sample of the rural population in the Dabie Mountains in China. To obtain information about health service utilization, a cross-sectional household survey was conducted using face-to-face interviews among the residents of a poor town in the Dabie Mountains. A total of 1,003 residents aged 15 or more, who had felt unwell in the last two weeks before the survey, were included in the analysis. The χ2 test and binary logistic regression were used to analyze the factors influencing health service utilization. A total of 51.2% of those surveyed had not used health services when they felt unwell, higher than the data reported in the 4th National Health Services Survey of China. Enabling variables played an important role in predicting the utilization of health services. Factors associated with increased
Background: Knowledge of utilization of health services and asso- ciated factors is important in planning and delivery of interventions to improve health services coverage. This knowledge is however limited in many developing countries. We determined the preva- lence and factors associated with health services utilization in a rural area of Kenya. Our findings inform the local health management in development of appropriately targeted interventions. Methods: Design: Cluster sample survey. Population: Residents of Kaloleni sub-County in Kenya. Participants/respondents: Household key informants. Outcomes: (i) History of illness for household members and (ii) health services utilization in the preceding month, (iii) factors associated with health services utilization. Analyses: Estimation of prevalence (outcomes i and ii) and random effects logistic regression (outcome iii). Findings: 1230/6,440 (19.1%, 95% CI: 18.3%-20.2%) household members reported an illness in the month preceding the survey. Of these,
Downloadable (with restrictions)! We document the impact of the AIDS crisis on non-AIDS related health services in fourteen sub-Saharan African countries. Using multiple waves of Demographic and Health Surveys (DHS) for each country, we examine antenatal care, birth deliveries, and rates of immunization for children born between 1988 and 2005. We find deterioration in nearly all of these dimensions of health care over this period. The most recent DHS survey for each country collected data on HIV prevalence, which allows us to examine the association between HIV burden and health care. We find that erosion of health services is highly correlated with increases in AIDS prevalence. Regions of countries that have light AIDS burdens have witnessed small or no declines in health care, using the measures noted above, while those regions currently shouldering the heaviest burdens have seen the largest erosion in treatment for pregnant women and children. Using semi-parametric techniques, we can date the
There has been an expansion of circumcision services in Africa as part of a long-term HIV prevention strategy. However, the effect of infant male circumcision on morbidity and mortality still remains unclear. Acute morbidities associated with circumcision include pain, bleeding, swelling, infection, tetanus or inadequate skin removal. Scale-up of circumcision services could lead to a rise in these associated morbidities that could have significant impact on health service delivery and the safety of infants. Multidisciplinary training programmes have been developed to improve skills of health service providers, but very little is known about the effectiveness of health service provider education and/or training for infant male circumcision on short- and long-term morbidity outcomes. This review aims to evaluate the effectiveness of health service provider education and/or training for infant male circumcision on short- and long-term morbidity outcomes. The review will include studies comparing health
Background There is a large treatment gap for common mental disorders (CMD), with wide variation by world region. This review identifies factors associated with formal health service utilisation for CMD in the general adult population, and compares evidence from high-income countries (HIC) with that from low-and-middle-income countries (LMIC). Methods We searched MEDLINE, PsycINFO, EMBASE and Scopus in May 2016. Eligibility criteria were: published in English, in peer-reviewed journals; using population-based samples; employing standardised CMD measures; measuring use of formal health services for mental health reasons by people with CMD; testing the association between this outcome and any other factor(s). Risk of bias was assessed using the adapted Mixed Methods Appraisal Tool. We synthesised the results using
Background: The phenomenon of inequitable healthcare utilization in rural China interests policymakers and researchers; however, the inequity has not been actually measured to present the magnitude and trend using nationally representative data. Methods: Based on the National Health Service Survey (NHSS) in 1993, 1998, 2003, and 2008, the Probit model with the probability of outpatient visit and the probability of inpatient visit as the dependent variables is applied to estimate need-predicted healthcare utilization. Furthermore, need-standardized healthcare utilization is assessed through indirect standardization method. Concentration index is measured to reflect income-related inequity of healthcare utilization.
A method and system for allowing a user to select a healthcare services package is disclosed where a central server generally receives personal information data from the user over the Internet regarding the users preferences with respect to healthcare service providers and the users sponsor, i.e., the party responsible for paying for all or part of the users healthcare costs (such as an employer, an insurance company or the user himself). The server thereafter provides a list comprising a plurality of healthcare service providers and information regarding the healthcare service providers (e.g., specialty, rate, location, hospital affiliation, etc.) to the user for selection by the user of a healthcare services panel. Once the server has received the selected healthcare services panel from the user comprising at least one of the listed healthcare service providers, the server determines a healthcare services package and provides the determined healthcare services package to the user for selection by
Context: Few studies in developing settings seek to determine caregivers level of satisfaction with medical services provided for their children. High-quality medical care is associated with improved health outcomes and higher health-care utilization. Aims: We sought to understand caregiver satisfaction in the pediatric ward of a large hospital in Botswana. Settings and Design: A cross-sectional, descriptive survey was conducted in the pediatrics medical ward at a referral hospital in Gaborone, Botswana, between September and October 2017. Subjects and Methods: A cross-sectional survey was conducted on caregivers admitted with their children in the pediatric ward of a large multispecialty hospital in Botswana. A total of 250 caregivers took a modified Service Quality Instrument from September to October 2017. The quality service instrument provides expectation and perception data in two subscales and assesses the difference between expectation and perception score, or gap score, for five ...

December 31, 2009. For the past 19 years, Frank Santos, CFO of the privately held, seven-property Rosen Hotels and Resorts in Orlando, Florida, has prided himself on delivering high-quality health care to his employees in a unique and low-cost way.In 1991 the company set up a primary-care clinic in one of its hotels, allowing employees to get basic health-care services during their working hours. By cutting out other primary-care options and contracting directly with hospitals and specialists for additional services, Santos says, the company has been able to offer a full health-care package to its 5,000 or so employees and their families and save at least $10 million a year compared with national averages.Beyond the basics, the company goes to great lengths to keep its employees healthy, including offering them many wellness services, such as exercise classes and serving only healthy foods (no French fries) in its cafeterias.. Archie Cochrane is best known for his influential book, Effectiveness and Efficiency: Random Reflections on Health Services published in 1972. The principles he set out in it so clearly were straightforward: he suggested that, because resources would always be limited, they should be used to provide equitably those forms of health care which had been shown in properly designed evaluations to be effective. In particular, he stressed the importance of using evidence from randomised controlled trials (RCTs) because these were likely to provide much more reliable information than other sources of evidence. Cochranes simple propositions were soon widely recognised as seminally important - by lay people as well as by health professionals. In 1979 he wrote, It is surely a great criticism of our profession that we have not organised a critical summary, by specialty or subspecialty, adapted periodically, of all relevant randomised controlled trials. His challenge led to the establishment during the ...

Being able to see a health professional can help a person to stay well and ensure that they receive appropriate care as early as possible if they become unwell. In rural and, especially, remote areas such timely access is more difficult because of shortages of health professionals. Plenty of evidence is available about the sort of health services that work well for people who live in rural and even isolated areas. But frequently these services cant be provided because of the maldistribution of health professionals.

Under the ADA, service animals must be harnessed, leashed, or tethered, unless such restraints interfere with the service animals work or the individuals disability prevents using these devices. In that case, the individual must maintain control of the service animal through voice or other controls. The owner/handler of a service animal is responsible for controlling the service animal and paying to repair any damage caused by the service animal. If the animal does become unreasonably disruptive or threatening, the owner is responsible for controlling it. If it cannot be controlled, the animal may be excluded, but the guest should be welcome to stay without the animal. Courts have made it clear that unless a service animal is disruptive (in a manner unrelated to its service function) or dangerous, it may not be removed or excluded. In any event, most service animals are particularly trained to be around people and are not dangerous ...
