Internet-Draft Running Code January 2013 1. Introduction Most IETF participants are familiar with the saying, "rough consensus and running code" [tao], and can identify with its pragmatic approach. However, there are many examples of Internet-Drafts containing protocol specification that have gone through to publication as Proposed Standard RFCs without implementation. Some of them may never get implemented. Over time, a variety of policies have been implemented within the IETF to consider running code. In the Routing Area it used to be a requirement that one or more implementations must exist before an Internet-Draft could be published as a Proposed Standard RFC [RFC1264]. That RFC was later obsoleted and the requirement for implementation was lifted, but each working group was given the authority to impose its own implementation requirements [RFC4794] and at least one working group (IDR) continues to require two independent implementations. The hypothesis behind this document is that there are ...
Implementing Agile poorly will yield poor results. There are three categories of poor implementation - partial implementations, failing to define interfaces with non-Agile groups and not training everyone involved. Poor implementations of Agile increase the risk of failure because those involved with Agile projects will not have enough guidance. That will generate conflict and put pressure…

Behaviour change is key to improving healthcare and health outcomes. Behaviours may be those of healthcare workers, such as implementation of evidence-based practice, of patients, such as medication adherence, or of the general population, such as smoking cessation and increasing physical activity. Despite high-level recommendations to improve implementation of evidence-based practice [1, 2] and a rapidly developing field of implementation science, implementation remains variable, with numerous organisational and individual factors influencing healthcare workers behaviour. These factors include the availability of evidence, its relevance to practice, the dissemination of evidence and guidelines, individual motivation, the ability to keep up with current changes, clarity of roles and practice, and the culture of specific healthcare practices [3, 4].. Improving implementation of evidence-based practice by healthcare workers depends on changing multiple behaviours of multiple types of people ...

Implementation science was not in Sight and Lifes wheelhouse until Klaus Kraemer, Managing Director of Sight and Life, reviewed and was mystified by the data in the 2013 DEVTA trial, which was published in The Lancet. Previously, randomized controlled trials conclusively demonstrated that high-dose vitamin A supplementation of children under five years of age reduces mortality by 24%. However, the DEVTA trial in India showed a non-significant 4% reduction in child mortality. Jean-Pierre Habicht observed that "This randomized controlled trial, as is typical of such trials, was carefully designed to interpret positive effects as due to the supplementation. However, it was not designed to interpret a lack of effect". In particular, it did not have plausible evidence of wide spread effective implementation.. The massive study had bare bone supervision of the intervention with only 18 monitors overseeing the work of over 8,300 Anganwadi workers and the participation of a million children. It also ...
Work status reports. These are just some of the workflow specific features of the EHR system as it relates to orthopedics. Many operations can be designed specifically for the clinic or orthopedic department. Templates are offered to make the system extremely efficient. These templates may include ACL planning, hip procedures, cast removal operations or total knee recognition and x-rays.. Things to Remember During the Integration Process. The implementation process can be eventful in any orthopedic office or department and as a result, keeping the lines of communication open between staff and doctors can be helpful. It is important to remember that in any organization there will always be those who resist change, especially when integrating a full system overhaul. The key thing to remember during this process is why the integration is taking place. Carefully remind staff resistant to change that there are reasons why the integration is taking place. Communicating with the front line team is as ...
Methods The features of effective cardiovascular training and an overview of what an implementation process involves will be discussed.. Generally, one of the two following approaches are applied: the more structured "rational model", where new findings are considered worth to be introduced into routine and subsequently are disseminated and implemented with steering from external and above, or the "participation model", which works bottom up, innovation is introduced incrementally and steered from practice.. Different theories from other disciplines and scientific areas can be used to support the process of change, i.e. theories that address the individual health professionals behaviour change, theories that focus on the social interaction and context and finally, theories on factors related to the organisational or economic context.. The implementation process itself is a systematic approach with clear strategies for dissemination, implementation and/or maintenance (1). In a project plan ...

An outreach facilitator helps the practice identify areas for improvement, set goals and targets, and agree on the processes needed to reach them in order to improve the care delivery within the practices. Each facilitator will be assigned up to 12 practice sites. The first year of program implementation will involve frequent (once every 3-4 weeks) visits to the practices. Afterwards, the intervention will move into a sustainability mode, during which the frequency of visits will decrease to one every 6-8 weeks during the second year, and one every 12-15 weeks during the third year and thereafter. After the first year of program implementation, each facilitator will be able to take on 12 new practices during the second year, while still being able to sustain contact with the previous 12 practices. Similarly, in the third year, another set of 12 practices will begin the intense phase of program implementation ...

A TCP project was launched in 2005 to reinforce capacities, at the national and regional levels, for the integration of a gender dimension in the programmes and projects implemented by the Ministry of Agriculture. The establishment of regional commissions to promote equal opportunities for men and women was instrumental in improving the collaboration between the central and regional levels. The project was successful in strengthening the institutional capacity of the national and regional services of the Ministry of Agriculture through the provision of training on socio-economic and gender analysis to 20 women and 5 men. A total of 125 professional staff (75 women and 50 men) in four macro zones were sensitised to gender issues through regional workshops. Due to its model character, the project approach was subsequently broadly applied within the National Womens Service (SHERMAN) and expanded to other ministries. The database developed by the project provided the basis for the collection of ...
The Committee on Legal Affairs adopted the own-initiative report drawn up by Klaus-Heiner Lehne (EPP, DE) on implementation of Article 2 of the Staff Regulations of the European Communities: the recruitment policy of the Institutions and organs of the EU. The principal objective of the report is to look at the implementation of Article 2 of the Staff Regulations in the context of the recruitment of officials and other servants of the EU institutions and organs. That Article provides in particular that one or more institutions may entrust to any one of them or to an inter-institutional body the exercise of some or all of the powers conferred on the Appointing Authority, other than decisions relating to appointments, promotions or transfers of officials. By Decision 2002/620/EC, the selection of officials and other servants was entrusted to European Personnel Selection Office (EPSO) pursuant to Article 2 of the Staff Regulation. It is therefore that aspect of recruitment policy, which is managed ...

Implementations of SVG are expected to behave as though they implement a rendering (or imaging) model corresponding to the one described in this chapter. A real implementation is not required to implement the model in this way, but the result on any device supported by the implementation shall match that described by this model.. The appendix on conformance requirements describes the extent to which an actual implementation may deviate from this description. In practice an actual implementation will deviate slightly because of limitations of the output device (e.g. only a limited gamut of colors might be supported) and because of practical limitations in implementing a precise mathematical model (e.g. for realistic performance curves may be approximated by straight lines, the approximation need only be sufficiently precise to match the conformance requirements).. ...

Implementation science is the study of methods to promote the adoption and integration of evidence-based practices, interventions and policies into routine health care and public health settings. Implementation research plays an important role in identifying barriers to, and enablers of, effective global health programming and policymaking, and leveraging that knowledge to develop evidence-based innovations in effective delivery approaches.. As outlined in the Centers Strategic Plan, Fogarty aims to support research and research training in implementation science using a distributed approach across its programs. Also, projects coordinated by the Center for Global Health Studies (CGHS) at Fogarty support implementation science research methods and frameworks, and use innovative approaches to supporting implementation science. These projects address a wide variety of critical global health research areas, such as HIV prevention, transmission and treatment; household air pollution; mobile health; ...
Press Release Date: November 23, 1999 The Agency for Health Care Policy and Research (AHCPR) today announced funding for five new research projects which are aimed at assessing quality improvement strategies. These strategies include: education; the use of information systems; continuous quality improvement; behavioral interventions; academic detailing and use of regulations. These grants are funded under AHCPRs Request for Applications (RFA) entitled "Assessment of Quality Improvement Strategies in Health Care," which was released on January 22, 1999. The goal of the RFA, part of a series of three new calls for research on quality of health care, was to rigorously evaluate strategies for improving health care quality which are currently in widespread use by organized quality improvement systems.. The funding for these new projects is anticipated to total $8.42 million over a 3-year period. The newly funded projects are: ...
Context Hospitals in European countries apply a wide range of quality improvement strategies; however, knowledge on the effectiveness of these strategies is limited. Objectives We propose to study the effectiveness of quality improvement systems in European hospitals. Specific research objectives are: a) to develop a maturity classification model for the assessment of organizational quality improvement systems in EU hospitals; b) to investigate associations between the maturity of quality improvement systems and measures of organizational culture, professional involvement and patient empowerment (at hospital level); c) to investigate associations between the maturity of quality improvement systems and measures of clinical effectiveness, patient safety and patient involvement (at patient and departmental level); d) to identify factors influencing the uptake of quality improvement activities by hospitals including external pressure as enforced by accreditation, certification or external assessment ...
We identified several limitations in the literature included in our analysis. First, similar to other studies of complex interventions,77 studies included in our meta-analysis reported few details about the intensity and "dose" of quality improvement strategies, as well as further details regarding delivery. The Standards for Quality Improvement Reporting Excellence (SQUIRE) guidelines have been developed to improve the reporting of quality improvement strategies,78 which will be of benefit to future meta-analyses such as ours. Second, in some studies, the duration of intervention may have been too short (e.g., 1 mo) to show any significant impact. Third, the duration of follow-up (as little as 3 mo) was also short in some studies. Fourth, the definition of a frequent user was inconsistent across the studies. Finally, most of the included studies had unclear or inadequate concealment of the allocation sequence and a high risk of bias owing to incomplete outcome data.. Our systematic review ...

Patient-centered e-health has failed to achieve its promise despite considerable consumer interest in technology and research supporting its potential. E-health adoption rates in healthcare are poor, with specialty substance use disorder (SUD) treatment having the lowest technology adoption rate of any sector.1 Implementation science can address this emerging gap in the e-health field by augmenting existing models, that explain organizational and individual e-health behaviors retrospectively, with prospective models that can guide implementation. The organizational planning discipline, with its decades of research, could provide a cross-disciplinary jump start to developing an e-health implementation model for health organizations.2, 3 Henry Mintzberg, a respected pioneer in this field, describes 2 beneficial approaches to planning: the deliberate approach, which is grounded in pre-implementation planning, and the emergent approach that is grounded in adapting to the environment as the plan is ...
Rationale, aims and objectives Clinical pathways (CPs) can improve quality of care on intensive care units (ICUs), but are infrequently utilized and of varying quality. Knowledge regarding factors that facilitate versus hinder successful implementation of CPs is insufficient and a better understanding of the activities and individuals involved is needed. The aim of this study was to explore the implementation process of CPs within the context of ICUs. Methods An exploratory design with a sequential mixed method was used. A CP survey, including all Swedish ICUs, was used to collect quantitative data from ICUs using CPs (n = 15) and interviews with key informants (n = 10) were used to collect qualitative data from the same ICUs. Descriptive statistics and qualitative content analysis were used, and the quantitative and qualitative findings were integrated. Results The CP implementation was conceptualized according to two interplaying themes: a process to realize the usefulness of CPs and create ...

Downloadable! Based on the innovation and technological diffusion literatures, promising research questions concerning the implementation of a production and inventory control information system (material requirements planning: MRP) are identified and empirically examined. These questions focus on the interaction of managerial tasks with the information technology and the resulting effect on the adoption and infusion of that technology. Using a random sample of manufacturing firms across the United States, we find that this interaction does indeed affect the adoption of MRP, though it does not seem to affect MRP infusion. These results support the notion that though rational decision models may be useful in explaining information technology adoption, political and learning models may be more useful when examining infusion.

Building on the experience of our University of Alabama at Birmingham (UAB) interdisciplinary team we have designed an innovative, scientifically rigorous, and highly feasible implementation research project in partnership with two Kaiser Permanente (KP) research centers. Kaiser Permanente, one of the Nations largest Healthcare Organizations, uses an integrated electronic medical record (EMR), with full capture of pharmacy, clinical (including BMD results), and claims data, and cares for a racially/ethnically, socio-economically, and geographically diverse population. To address innovative questions in implementation research, we will perform a multi-modal group randomized trial involving over 18,000 patients seen by over 330 primary care providers (PCPs) at 25 KP facilities in the Northwest and Southeast. If proven ...

... is concerned with socially integrated mitigation of multiple structural risks in the urban environment. It aims to produce new knowledge on the performance of existing reinforced concrete (RC) structures subjected to earthquake and fire hazards and to develop an integrated framework for performance-based assessment and structural mitigation. The proposed technical engineering solutions will be developed accounting for the social context within which they are to be enacted. Part of the project focuses on community engagement through use of citizen science techniques and provision of tailored monitoring and data analysis tools to define acceptable performances, inform the technical programme of work and the means for effective implementation of mitigation strategies for multiple risks. Thus challenging RISK, aims amongst others, to suggest a community-centric learning about risk and resilience, and an understanding of how models, outcomes of research, guidelines and regulations ...

The DNSSEC protocol makes use of various cryptographic algorithms in order to provide authentication of DNS data and proof of non-existence. To ensure interoperability between DNS resolvers and DNS authoritative servers, it is necessary to specify a set of algorithm implementation requirements and usage guidance to ensure that there is at least one algorithm that all implementations support. This document defines the current algorithm implementation requirements and usage guidance for DNSSEC. This document obsoletes
