The lung allocation score (LAS) is a numerical value used by the United Network for Organ Sharing (UNOS) to assign relative priority for distributing donated lungs for transplantation within the United States. The lung allocation score takes into account various measures of a patients health in order to direct donated organs towards the patients who would best benefit from a lung transplant. The LAS system replaces the older method within the United States of allocating donated lungs strictly on a first-come, first-served basis, according to blood type compatibility and distance from the donor hospital. The older method is still used for patients under the age of 12. Various possible allocation systems are reviewed for how decision makers might evaluate proposed programs for legislative funding. Using a case-based exercise, readers are asked to weigh the potential costs and benefits of six health and social programs. These programs are also systematically examined for factors that are likely to affect resource allocation decisions. BACKGROUND Recently, a global commitment has been made to expand access to antiretrovirals (ARVs) in the developing world. However, in many resource-constrained countries the number of individuals infected with HIV in need of treatment will far exceed the supply of ARVs, and only a limited number of health-care facilities (HCFs) will be available for ARV distribution. Deciding how to allocate the limited supply of ARVs among HCFs will be extremely difficult. Resource allocation decisions can be made on the basis of many epidemiological, ethical, or preferential treatment priority criteria. METHODS AND FINDINGS Here we use operations research techniques, and we show how to determine the optimal strategy for allocating ARVs among HCFs in order to satisfy the equitable criterion that each individual infected with HIV has an equal chance of receiving ARVs. Downloadable! We study the portfolio allocation decisions of Australian households using the relatively new Household Income and Labour Dynamics in Australia (HILDA) survey. We focus on household allocations to risky financial assets. Our empirical analysis considers a range of hypothesised determinants of these allocations. We find background risk factors posed by labour income uncertainty and health risk are important. Credit constraints and observed risk preferences play the expected role. A positive age gradient is identified for risky asset holdings and home-ownership is associated with greater risky asset holdings. A unifying theme for many of our empirical findings is the important role played by financial awareness and knowledge in determining risky asset holdings. Many non-stockholding households appear to lack the experience and financial literacy that might enable them to benefit from direct investment in stocks.

N2 - Once a potential candidate is chosen, and all of the above workup is performed, the patient can then be placed on the waiting list. A MELDNa will be calculated and used to stratify the patient according to how acutely the patient needs the donor organ. From here, organ allocation will proceed as described above, with the ultimate goal of getting the donor organs to the patients who need it the most and will benefit the most from it. Nuances and challenges to the allocation system exist, including MELD exceptions for those with HCC, as well as geographic disparities in transplant rate and waiting list mortality. EuroTransplant has developed an efficient system for minimizing kidney wastage that includes the Recipient Oriented Extended Allocation system and the EuroTransplant Senior Program. Ah, you might say that our resources are scarce, that no treatment should be offered without "medical necessity" whatever that may mean -- its a term invented by insurers to justify denying payment for a problem. So let me put it this way: our sickest members are often our least productive members, even with treatment. They take the most expensive medications, often get government disability payments, and may require expensive hospitalizations repeatedly, not to mention case managers and placement in day programs. They are clearly ill and one would would deny them care. So what about a rocket scientist or a neurosurgeon or a school teacher, who simply finds that talking to someone once a week or once a month, or whenever a problem arises, enables them to be more emotionally comfortable, to understand and manage their interpersonal motivations in a way that gives them more control and makes life smoother and more productive -- might it be okay for therapy to continue without a definite endpoint? ...

I think the arguments most likely to persuade the obstinate are the historical and fairness ones. I dont think car people respond to arguments about health, the environment, or conservation of scarce resources because those benefits are either abstract or enjoyed by someone other than them. What they care about is the immediate (perceived) convenience and comfort of being able to take their cars everywhere and not being slowed down by bikes (although they dont seem to mind much being slowed down by other cars).. So if I were making a pro-bike speech, this would be my approach: (1) the streets, for thousands of years, have been public space for all to use and (2) people who drive cars, especially in dense urban environments, impose costs on others and dont bear the full costs of their decision to drive. Health systems have experienced unprecedented stress in recent years, and as yet no consensus has emerged as to how to deal with the multiple burden of disease in the context of HIV and AIDS and other competing health priorities.
