Objectives: This study aims to evaluate the radiological and functional outcomes of hallux valgus patients treated with distal oblique metatarsal osteotomy technique.. Patients and methods: Twenty-six feet of 22 patients (4 males, 18 females; mean age 46.2±18 years; range, 16 to 70 years) who were diagnosed as hallux valgus between March 2013 and April 2016 and who underwent distal oblique metatarsal osteotomy were included in this retrospective study. American Orthopedic Foot and Ankle Society/Hallux Metatarsophalangeal-Interphalangeal Scale (AOFAS/HMIS) was used for clinical and functional evaluation. The hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), sesamoid position, first metatarsal length and forefoot bone and soft tissue width were measured for radiological evaluation.. Results: The mean follow-up time was 33.1±9.8 months. The AOFAS/HMIS score increased significantly postoperatively (p=0.001). In the footwear section of the AOFAS/HMIS, ...
The Cochrane review of hallux valgus surgery has disputed the scientific validity of hallux valgus research. Scoring systems and surrogate measures such as x-ray angles are commonly reported at just one year post operatively but these are of dubious relevance to the patient. In this study we extended the follow up to a minimum of 8 years and sought to address patient specific concerns with hallux valgus surgery. The long term follow up also allowed a comprehensive review of the complications associated with the combined rotation scarf and Akin osteotomies. Between 1996 and 1999, 101 patients underwent rotation scarf and Akin osteotomies for the treatment of hallux valgus. All patients were contacted and asked to participate in this study. 50 female participants were available allowing review of 73 procedures. The average follow up was over 9 years and the average age at the time of surgery was 57. The participants were physically examined and interviewed. Post-operatively, in 86% of the participants
Minimally invasive surgery (MIS) represents one of the most innovative surgical treatments of hallux valgus (HV). However, long-term outcomes still remain a matter of discussion within the orthopaedic community. The purpose of this longitudinal prospective study was to evaluate radiographic and functional outcomes in patients with mild-to-severe HV who underwent Reverdin-Isham and Akin percutaneous osteotomy, following exostosectomy and lateral release. Eighty patients with mild-to-severe symptomatic HV were treated by MIS. Clinical evaluation was assessed preoperatively, as well as at 3 and 12 months after surgery and at final follow-up of 48 months, using the American Orthopaedic Foot and Ankle Society (AOFAS) hallux grading system. Patient satisfaction and complications were recorded. Computer-assisted measurement of antero-posterior radiographs was taken preoperatively, as well as at 3 and 12 months after surgery and at 48-month follow-up, analysing the intermetatarsal angle (IMA), the hallux valgus
This is a randomized, double-blind, placebo-controlled prospective study to assess the influence and efficacy of intramuscular (IM) Botulinum Toxin Type A (BTA) injection in patients with painful hallux valgus with the use of total contact insole.. Fifty feet with painful hallux valgus will be collected. Informed consent is obtained according to the hospitals medical ethics and the human clinical trial committee. The feet will be randomized into 2 groups: Group A (n=25, treatment group) and Group B (n=25, control group). Clinical diagnosis of hallux valgus is made based on observation of great toe lateral deviation. Inclusion criteria were hallux valgus with angle of at least 20°, single or bilateral hallux valgus. To obtain the hallux valgus angle, foot roentgenography in AP and sesamoid views under the weight bearing condition will be obtained. The angle formed by lines drawn to bisect the first metatarsal bone and the proximal phalanx of the great toe will be measured. The exclusion ...
PROPOSE: This study was retrospectively to review the surgical results for moderate to severe hallux valgus corrected with a modified McBride procedure and proximal metatarsal crescentic osteotomy. MATERIALS AND METHODS: Between August 1997 and August 2001, 15 patients with 22 bunion underwent surgical correction and were followed for an average 29.3 months (range, 18 to 53 months). Clinical results were evaluated with AOFAS clinical rating system for hallux and radiological measurements were done preoperatively and at a minimum of 12 months postoperatively. RESULTS: The average AOFAS clinical rating score improved from 47.5 to 86.0. Union of the osteotomy site occurred at 9.5 weeks except one delayed union. The hallux valgus angle improved an from 36.5 degrees to 15.7 degrees and the intermetatarsal angle improved from 17.4 degrees to 8.6 degrees on average. Dorsiflexion of the first metatarsal at the osteotomy site was present in three (13.6%) with average 4 degrees. Complications were two ...
Given that high intermetatarsal angles can complicate hallux valgus correction, these authors discuss key preoperative considerations, highlight important aspects of corrective procedures and provide pearls on dealing with complications and postoperative protocols.

A Chevron osteotomy is indicated for correction of a mild to moderate hallux valgus deformity. This allows for a small reduction of the angle between the first and second metatarsal. It is ideal for bunions that are not particularly pronounced.
Introduction: There are many techniques to correct the hallux deformity. Most of them include metatarsal and/or phalanx osteotomies. Akin osteotomy of the proximal phalanx is used to correct the distal articular set angle or the interphalangeal angle. However, indications for distal phalanx osteotomy remain unpublished. The aim of this study is to prove the benefit of performing an osteotomy on the distal phalanx in some cases to correct hallux deformities. Methods. We report 9 cases in which osteotomy of the distal phalanx was performed to correct the hallux deformity. Radiographic measurements were performed on standing dorsoplantar radiographs to analyze the distal articular set angle, interphalangeal obliquity, interphalangeal angle, and metatarsophalangeal angles. The cases are described. The surgical technique was performed by minimal incision surgery using fluoroscopy. The clinical results were evaluated by the VAS and the AOFAS forefoot scale. The mean follow-up was 92 months. Result: ...
Hallux varus is a deformity of the big toe joint that results in the big toe pointing outward, or away from the other toes, whereas a bunion would cause the big toe to lean inward. Hallux varus can be congenital, or it can occur from overcorrective bunion surgery, a tight or short abductor hallucis tendon, or injury.. Aside from the direction of the toe pointing outward, symptoms of hallux varus are pain, redness, and blisters on the toe from it rubbing against footwear. An ingrown nail on the big toe may also develop from pressure. The condition can be treated by stretching and splinting the abductor hallucis tendon. For more severe cases, surgery can straighten the toe.. ...
Hallux valgus (or bunion) refers to the condition when the big toe (called hallux) has a pointed appearance towards the second toe (called angulation), causing a pathological bump on the external side of the toe. This is basically an enlargement of bone or tissue around the joint at the top of the big toe, while the tissues that surround the joint would be tender and swollen. It is commonly found in women, and many a times run in families. If an individual is born with some abnormality in bones of his or feet, he or she is more likely to form hallux valgus. Major cause of a bunion is wearing high heels and narrow-toed shoes.. ...
OBJECTIVES: To assess responsiveness and minimally important change (MIC) for the Manchester-Oxford foot questionnaire (MOXFQ) using anchor and distribution-based approaches. Responsiveness and estimates of minimal clinically important difference (MCID) and minimal detectable change are compared with those from the Short-Form 36 (SF-36) and American Orthopaedic Foot & Ankle Society (AOFAS) measures. METHODS: A prospective observational study of 91 consecutive patients (125 foot operations) undergoing hallux valgus surgery at an orthopaedic hospital. Pre- and 12 month post-surgery, patients completed the MOXFQ and SF-36, and foot surgeons assessed all four AOFAS scores corresponding to four regions of the foot. Transition items were asked about perceived changes compared with before surgery. RESULTS: Mean changes in all domains of each instrument were statistically significant, but foot-specific MOXFQ and AOFAS domains produced much larger effect sizes (|1) than any SF-36 domains, indicating superior

Overview Bunions are common but they can be misdiagnosed. We sometimes assume that any lump at the bottom of the big toe is a bunion. But as the Latin name (hallux valgus) suggests, the hallmark of a bunion is what happens to the toe itself (the hallux) rather than to the joint at its base.…
I had a chevron bunionectomy procedure done in January of 05′. I am deployed overseas and had an accident jumping off an aircraft that was off the ground about 2-3 feet. I now have pain and noticied my big toe in now tucked underneath the second toe. I am in great pain, x rays do not show anything abnormal and Army doctors cant figure out what is wrong. My big toe was properly aligned after surgery, now it is back to its deformity before I had surgery. Can anyone help me with this issue?. ...
Patients undergoing bunionectomy (a surgical procedure to remove a bunion, an enlargement of the joint at the base of the big toe comprised of bone and soft tissue) often experience moderate to severe acute pain post-surgery. Normally such pain is controlled when patients receive repeated doses of opioid analgesics. Tapentadol (CG5503) is a newly synthesized opioid drug acting as a centrally acting analgesic like opioid analgesics but has a different mode of action. This study, a randomized (patients are assigned different treatments based on chance), double-blind (neither investigator nor patient knows which treatment the patient receives), placebo-controlled (placebo is an inactive substance that is compared with a drug to test whether the drug has a real effect in a clinical trial), parallel-group (each group of patients will be treated at the same time), multicenter study (the study is performed at more than one clinic) is designed to evaluate the effectiveness (level of pain control) and ...

BACKGROUND: Exostoses at the base of the distal phalanx of the great toe are usually asymptomatic. The literature has not generally considered them as the origin of a possible problem resulting from a pressure conflict between hallux and shoe (medial aspect) or second toe (lateral aspect) nor a potential complication of surgical correction of hallux valgus deformity. No studies, to our knowledge, have evaluated its possible correlation with other foot disorders. When one of these neglected exostoses became painful after surgical correction of hallux valgus, we decided to start a study to determine their possible origin, prevalence in daily practice and histo-pathological morphology ...

Yes, chiropractic helps with feet and ankle issues. The average person takes between 3,000 and 5,000 steps per day. In addition many of us are engaged in activities such as running and aerobic exercise that add many more steps and stress to our poor over worked feet. It is no wonder that many people have aches and pain in this part of the body. Factors such as flat feet or over-pronated feet, our walking and running style, the type of shoe wear can determine if we will get foot and ankle pain. However these factors can also affect the health of your back, knees and legs. Below are a few of the common conditions that affect the feet and ankle.. Hallux Valgus - Bunions. Hallux valgus is a condition when the big toe or hallux deviates towards the outside of the foot often almost overlapping or bumping in to the adjacent toe. It is a slow developing condition that usually develops a bump on the inside of the big toe often called a bunion.. The pain associated with Hallux Valgus is usually felt ...
An Akin osteotomy is a medial closing wedge osteotomy of the proximal phalanx of the great toe. It is used to correct hallux valgus interphalangeus. The Akin osteotomy is usually a component of a primary procedure in which a hallux valgus deformity is corrected. The osteotomy site is fixed with a DynaNite™ Nitinol staple, low-profile ...
Pain on the inside of the heel and ankle can be a sign of abductor hallucis muscle dysfunction. The pain will often cause a person to limp.
The abductor hallucis muscle runs along the medial, or interior, border of each foot, covering the origins for most of the plantar (sole of foot) nerves and vessels.

Surgical treatment of hallux valgus (HV) is one of the major flagships of orthopedic surgeons. Due to relatively unsatisfactory radiological and clinical outcomes, the search for the best surgical technique and causes for unsatisfactory outcomes continues. The objective was to investigate associations of the number of screws and additional surgical techniques for HV with radiological and clinical outcome after reversed L-shaped osteotomy (ReveL). A retrospective cohort study of adults from a single University Hospital between 2004 and 2013 was performed. The primary exposure was the number of screws (one vs two) used for osseous fixation after ReveL. The secondary exposure was an additional surgical technique for HV (e.g., Akin osteotomy). The primary outcome was a radiological recurrence of HV (HV angle (HVA) | 15°) at last follow-up. The secondary outcomes were limited patient satisfaction, complication, revision surgery, and elective hardware removal. Odds ratio (OR) and 95% confidence interval (CI)

Dear Dr. Mitnick, The orthotics arrived four days ago and I slipped them into my shoes immediately. I was skeptical as to the usefulness of the item, they really didnt look very exotic. I have to say though, after using them for just four days, I have experienced grand relief from my foot pain. Even the very first day, I was able to do a lot of work while on my feet with at least a 75% reduction of pain. It has only gotten better every day, and I go nowhere without my shoes with the orthotics. I had been experiencing extreme heel and sole pain for about six months and had to take extended breaks off my feet many times a day as well as regular doses of Ibuprofen. Since getting the orthotics, my life has returned to normal and I feel good again. Just wanted to say thanks for the recommendation for a very effective item, I had no idea what a change this item could affect ...
