Guanfacine is commonly used for the treatment of tics in children with Tourette Disorder, but neither the immediate release compound nor the new extended release formulation have been evaluated for tics as a primary outcome. This pilot study is not designed to demonstrate efficacy of extended release guanfacine in the treatment of tics in children with TD. Rather, the goal of this trial is to determine whether extended release guanfacine warrants further study in a large scale trial. Immediate-release guanfacine is frequently used in children with TD, but dosing, time to effect and adverse effects with the new extended release guanfacine are unknown. The use of placebo in this trial reduces bias in the measurement of outcomes because it ensures blindness in the parent and clinician ratings.. This is a three-site, investigator-initiated, randomized, double-blind, placebo-controlled, parallel-group study. Subjects who show a positive response to extended release guanfacine in the 8-week ...
Efficacy of Guanfacine Extended Release in the Treatment of Combined and Inattentive Only Subtypes of Attention-Deficit/Hyperactivity Disorder
This study seeks to determine, using special sleep tests (polysomnography and actigraphy) if guanfacine extended release is able to improve nighttime sleep in children with ADHD - associated insomnia while improving daytime ADHD symptoms. Male and female children with diagnosed or suspected ADHD with sleep problems (difficulty falling asleep, difficulty staying asleep, or less than expected hours of sleep) will be recruited. After obtaining informed consent and assent (when appropriate) and after discontinuation of excluded medications, children will have evaluations of his or her sleep and evaluations confirming the ADHD diagnosis. Children who successfully pass screening will be enrolled into the double-blind, placebo-controlled, randomized investigation with 50% of participants receiving guanfacine extended release and 50% of participants receiving matching placebo. Using a flexible-dose optimization design based on ADHD symptom improvement and medication tolerability, the dose will be ...

Relative to an intravenous dose of 3 mg, the absolute oral bioavailability of guanfacine is about 80%. Peak plasma concentrations occur from 1 to 4 hours with an average of 2.6 hours after single oral doses or at steady state.. The area under the concentration-time curve (AUC) increases linearly with the dose.. In individuals with normal renal function, the average elimination half-life is approximately 17 hr (range 10-30 hr). Younger patients tend to have shorter elimination half-lives (13-14 hr) while older patients tend to have half-lives at the upper end of the range. Steady state blood levels were attained within 4 days in most subjects.. In individuals with normal renal function, guanfacine and its metabolites are excreted primarily in the urine. Approximately 50% (40-75%) of the dose is eliminated in the urine as unchanged drug; the remainder is eliminated mostly as conjugates of metabolites produced by oxidative metabolism of the aromatic ring.. The guanfacine-to-creatinine clearance ...

Similar to the recommendations from the previous guideline, stimulant medications are highly effective for most children in reducing core symptoms of ADHD.44 One selective norepinephrine-reuptake inhibitor (atomoxetine45,46) and 2 selective α2-adrenergic agonists (extended-release guanfacine47,48 and extended-release clonidine49) have also demonstrated efficacy in reducing core symptoms. Because norepinephrine-reuptake inhibitors and α2-adrenergic agonists are newer, the evidence base that supports them-although adequate for FDA approval-is considerably smaller than that for stimulants. None of them have been approved for use in preschool-aged children. Compared with stimulant medications that have an effect size [effect size = (treatment mean − control mean)/control SD] of approximately 1.0,50 the effects of the nonstimulants are slightly weaker; atomoxetine has an effect size of approximately 0.7, and extended-release guanfacine and extended-release clonidine also have effect sizes of ...

Abnormal heart rhythms: Guanfacine can cause abnormal heart rhythms. Certain medications (e.g., sotalol, quinidine, chlorpromazine, pimozide, gatifloxacin, moxifloxacin, mefloquine, pentamidine, dolasetron mesylate, probucol, tacrolimus) can increase the risk of a type of abnormal heart rhythm called QT prolongation, and should not be used in combination with this medication. If you have been told that your child is at an increased risk of developing an abnormal heart rhythm, discuss with your doctor how this medication may affect their medical condition, how their medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.. Drowsiness and reduced alertness: Guanfacine may affect the mental or physical abilities needed to perform tasks that require special attention. Have your child avoid riding a bike, operating machinery, or performing other hazardous tasks until you have determined how this medication affects your child.. Kidney ...

RESULTS: The level of neuronal uptake of norepinephrine in the pig pineal was 3.5+/-0.9 pmol/h/mg of wet tissue and represented about 77% of the total tissue radioactivity. Potassium ions at concentration of 60 mM significantly evoked tritium release. This effect was abolished in the absence of extracellular Ca(2+) and was diminished in the presence of Cd(2+). Antagonists of alpha(2)-adrenoceptors increased depolarization evoked tritium release. QUESTION: I have been treating a 7 year old boy with ADHD using Concerta 54mg and afternoon methylphenidate. He developed trouble sleeping which initially responded to clonidine but that seemed to stop working. I discontinued the methylphenidate in the afternoon, and switched to guanfacine at bedtime but that was of no help. Last week I had him stop everything. His mother reports that he goes to bed at a regular time, but plays in his bedroom until after midnight. During yesterdays interview, he giggles about this throughout the visit, as though as funny. She also reports that when he is not on stimulant medication he can be violent with himself and his brother. I have a sneaking suspicion that there may be something more going on and would welcome insight here.. ANSWER (Dr Rettew): This question brings up a number of important points to address.. 1. Sleep problems are very common in ADHD, and can exacerbate daytime attention problems. Sleep medications such as alpha agents and over the counter ...
