Glutethimide is a CYP2D6 enzyme inducer. When taken with codeine, (known on the streets as hits, cibas and codeine , Dors and 4s) it enables the body to convert higher amounts of the codeine to morphine. The general sedative effect of the glutethimide also adds to the effect of the combination.[4] It produces an intense, long lasting euphoria similar to IV heroin use. Quite a few deaths have occurred from abuse of this combination.[5] The effect was also used clinically, including some research in the 1970s in various countries of using it under carefully monitored circumstances as a form of oral opioid agonist substitution therapy, e.g. as a Substitutionmittel that may be a useful alternative to methadone.[6][7] The demand for this combination in Philadelphia, Pittsburgh, Newark, NYC, Boston, Baltimore, and surrounding areas of other states and perhaps elsewhere, has led to small-scale clandestine synthesis of glutethimide since 1984,[8]:203 a process that is, like methaqualone (Quaalude) ...

Background: Drug overdose continues to be the most common cause of acute poisoning worldwide. There has been a substantial increase in drug overdose incidence and prevalence over the past decade, probably as a result of the emergence of new synthetic designer drugs. The purpose of this study is to describe the clinical and epidemiological characteristics of patients with acute drug intoxication admitted to the Intensive Care Unit (ICU). Methods: A single center, prospective, observational study was conducted among all adult patients with clinical signs suggestive of acute drug intoxication admitted from the Emergency Department (ED) to ICU during a 6-month period (September to March ...

Artificial kidney is often a synonym for hemodialysis, but may also, more generally, refer to renal replacement therapies (with exclusion of kidney transplantation) that are in use and/or in development. This article deals with bioengineered kidneys/bioartificial kidneys that are grown from renal cell lines/renal tissue. The first successful artificial kidney was developed by Willem Kolff in the Netherlands during the early 1940s. Kolff was the first to construct a working dialyzer in 1943. Kidneys are paired vital organs located behind the abdominal cavity at the bottom of the ribcage corresponding to the levels T12-L3 of the spine vertebrae. They perform about a dozen physiologic functions and are fairly easily damaged. Some of these functions include filtration and excretion of metabolic waste products, regulation of necessary electrolytes and fluids and stimulation of red blood cell-production. These organs routinely filter about 100 to 140 liters of blood a day to produce 1 to 2 liters of ...
TY - JOUR. T1 - Multilayered microfilter using a nanoporous PES membrane and applicable as the dialyzer of a wearable artificial kidney. AU - Gu, Ye. AU - Miki, Norihisa. PY - 2009. Y1 - 2009. N2 - We present a multilayered microfilter for use as a dialyzer of a wearable artificial kidney separating metabolic wastes such as urea, uric acid and creatinine from blood. The microfilter device is assembled by alternately bonding chamber layers made of Ti by wet etching and semipermeable polymeric membranes made of polyethersulfone (PES) by the wet phase inversion method. The PES membranes sandwiched between each two chamber layers act as barriers to molecules larger than 1.7 nm. The multilayered microfilter was geometrically optimized with respect to our theoretical equations and experimental results in order to obtain kidney-competitive performance. Each diffusing unit of our device, which is only 24 × 24 × 0.4 mm3 in size, was proved experimentally to be capable of allowing a flow rate of up to 1 ...
The Wearable Artificial Kidney (WAK) will offer patients across the world suffering from kidney failure freedom from endless hospital appointments - and hospitals the chance to save on resources - states a new report by medical industry experts at GBI Research.. The new report Renal Dialysis Equipment Market to 2017- Lower Costs and Similar Efficacy Rates as Hemodialysis to Increase Adoption Rates of Peritoneal Dialysis by GBI Research shows the growing prevalence of diabetes and hypertension, alongside an increasingly large elderly population, has meant that doctors have encountered a rapid growth in the number of patients suffering from End Stage Renal Disease (ESRD), which has put pressure on burgeoning healthcare budgets. However, the lucrative market these patients represent for dialysis device manufacturers has led to the development of the WAK, the latest innovation to provide patient care on a global scale.. One in 10 adults are at risk of chronic kidney disease (CKD), totaling roughly ...

There just arent enough kidney transplants available for the millions of people with renal failure. Aside from a transplant, the only alternative for patients is to undergo regular dialysis sessions to clear harmful cellular waste from their bodies. Now, scientists report in ACS Nano a new urea sorbent that could accelerate progress toward the development of a lightweight, wearable artificial kidney with the potential to make dialysis more convenient, comfortable and effective.
For this type of treatment, the patients blood flows through an artificial kidney to be filtrated. To make this possible, the patient has to undergo an operation to form a fistula into which needles can be inserted.. The needles are then attached to bloodlines, which flow through the artificial kidney. This artificial kidney is a bundle of hollow fibres with a semi-permeable membrane.. The membrane is a thin film with thousands of microscopic holes. The holes allow water and other small dissolved parts through but prevent the transfer of larger elements such as proteins and blood cells.. During dialysis, the blood flows on one side of the membrane and an electrolyte solution on the other end of the membrane. Through osmosis and diffusion and exchange of waste products takes place between the two liquids. A dialysis machine and artificial kidney are used in the process.. The whole process is monitored and controlled by the dialysis machine and the technician. The apparatus controls the movements ...

Dialysis to remove toxins from your blood. If toxins build up in your blood, you may need temporary hemodialysis - often referred to simply as dialysis - to help remove toxins and excess fluids from your body while your kidneys heal. Dialysis may also help remove excess potassium from your body. During dialysis, a machine pumps blood out of your body through an artificial kidney (dialyzer) that filters out waste. The blood is then returned to your body. ...

We dont teach our technical community of students, developers, computer scientists and engineers how to solve problems.. But I dont mean original technical problems like how to build a faster computer or how to diagnose cancer through advanced imaging analysis. Those problems are the types of technical and social challenges that got us into engineering and science in the first place.. I mean the unintended problems that plague us on the quest to create a portable artificial kidney or deliver high fidelity movies over a cell phone. Problems that werent supposed to be there in the first place.. I mean Bugs.. Were putting more bugs into our products without having the skills and expertise to take them back out again.. more…. ...

Much that passes for modern nutrition is obsessed with a mania for quantitative counting. The body is treated like a check account. Deposit calories (like dollars) and withdraw energy. Deposit proteins, carbohydrates, fats, vitamins and minerals-balanced quantitatively-and the result, theoretically, is a healthy body. People qualify as healthy today if they can crawl out of bed, get to the office and sign in. If they cant make it, call the doctor to qualify for sick pay, hospitalization, rest cure-anything from a days pay without working to an artificial kidney, courtesy of the taxpayers. But what does it profit someone if the theoretically required calories and nutrients are consumed daily, yet this random eat-on-the-run, snack-time collection of foods ferments and putrefies in the digestive tract? What good is it if the body is fed protein, only to have it putrefy in the gastrointestinal canal? Carbohydrates that ferment in the digestive tract are converted into alcohol and acetic acid, not ...

In this work, velocity and concentration fields on both sides of blood and dialysate in artificial kidneys are simulated, not only coupling the convection and diffusion mass transfers across the hollow fiber membrane, but also considering both effects of the inlet and outlet structure of blood and dialysate. Our results show that compared to the blood velocity field, the dialysate velocity filed is more uneven; furthermore, the inlet and outlet structure of dialysate affects not only the concentration field on the dialysate side but the one on the blood side as well. Therefore, in practice, for the structure optimization of artificial kidneys, the inlet and outlet of dialysate should be focused on.. Copyright © 2013 by ASME ...

An artificial kidney comprises an exchanger (1) having two compartments (2, 3) separated by a semipermeable membrane (4), a first compartment (2) being connected to a circuit (5, 7) for extracorporeal blood circulation, the second compartment (3) having an outlet for draining waste liquid. Means (22, 25) are provided for controlling the flow rate (QA) of a sterile solution containing a substance (A) to be perfused into the circuit (5, 7) for conveying a flow of blood outside the body, which flow rate is controlled as a function of the flow rate (QOUT) of waste liquid. This kidney makes it possible to dose a substance such as a medicine, glucose or bicarbonate, accurately into the blood of a patient undergoing treatment by hemofiltration, hemodialysis, or hemodiafiltration.

Leonard Skeggs, PhD, and Jack Leonards, MD, developed the first parallel-flow artificial kidney at Case Western Reserve Univ. (Cleveland, OH). Two sheets of membrane were sandwiched between two rubber pads in order to reduce the blood volume and to ensure uniform distribution of blood across the membrane. The device had a very low resistance to blood flow and it could be used without a blood pump. Insomnia is a disorder defined as persistent difficulty falling asleep, staying asleep or non-restorative sleep which is associated with diminished daytime function without any identifiable underlying cause. This condition is extremely common among HIV infected individuals and can lead to significant distress and reduction in the quality of life. The mechanisms for disrupted sleep in this population are diverse, including potential direct effects of the tat protein upon the sleep centers in the central nervous system. Insomnia has been documented to be one of the most common psychiatric disorders in HIV disease, but no trial has systematically examined the efficacy of available hypnotic agents, which are commonly used in this population.. Comparison(s): Two commonly prescribed hypnotic agents used for insomnia will be compared to placebo over a 6 month treatment study. ...

Patients with prescriptions for hypnotics had approximately 4.6 times the hazard of dying over an average observation period of 2.5 years as compared to non-users. These findings were robust with adjustment for multiple potential confounders and consistent using multiple strategies to address confounding by health status. A dose-response effect was seen. Among users in the highest tertiles of annualised dosages, the HRs for death were 5.3, 5.7 and 6.6, respectively, for all hypnotics, zolpidem alone and temazepam alone. This top third of users were prescribed 92.8% of all the prescription doses of hypnotics (supplemental figure 2). Those in the top third were also 35% more likely to develop a new major cancer.. Perhaps the most striking finding was that an increased hazard for death was present even in the lowest tertile of hypnotic use, such that hypnotic drugs were associated with a 3.6-fold increased risk of dying for patients using ,18 hypnotic pills per year. Several strategies to discover ...
