We have designed a randomized control trial to assess the impact of a laparoscopic simulator curriculum on operating room performance amongst gynecology residents. At the beginning of the study, each resident that chooses to participate will take a multiple choice pre-test assessing his or her background knowledge of laparoscopic surgical principles. Everyone will then listen to a series of lectures / video demonstrations teaching the fundamentals of laparoscopic surgery. The final part of the orientation is to have each resident perform 5 tasks on the laparoscopic simulators (peg transfer, endoloop, pattern cutting, intracorporeal suturing, and extracorporeal suturing) while being proctored by a faculty member who is timing the task and recording any errors made. Each resident will have 2 proctored performances (pre- and post) on the simulator and it will be set up as a typical OSCE-type exam. One thing I have noticed in the program i am at, the attending who are heavily involved in research really love just pumping out research papers on material we already know with a slight twist. Its like they find any small reason to write a paper and try to get it published. Sometimes they will have students write BS on research work already done and then present it. Im regretting joining a program that is affiliated with a university that loves research. It has really taken us away from being great surgeons in training. In terms of what is written above about so many fellowships people taking away procedures from gen surg residents. I would say this is variable. How does a student know if surgery is right for her? Thats a tough one. Im afraid that working only 2 weeks of nights, while certainly hard, is a little misleading. Anyone can do 2 weeks of nights. Surgery training is 5 years with a lot of nights, weekends and long days. I think that one of the reasons there is a 25% attrition rate among general surgery residents is that their medical school experience is nothing like what residency really is. [See my blog about this.] Dont forget that it continues when you become an attending. The attending surgeons in private practice where I now work are putting in a lot of hours, sometimes more than 80/week. Id suggest you take an elective of at least 6 weeks in general surgery as your first 4th year course. Find a good elective in a busy trauma center. Mattar and coauthors have surveyed a select group of fellowship program directors (FPDs) regarding their opinions about the capabilities of those entering advanced training following completion of a general surgery residency.
