Ongoing light chain rearrangement, a process termed receptor editing, is a major mechanism for tolerizing self-reactive B cells. We have observed that highly edited B cells become less frequent once fully assembled antibody molecules are expressed on the B cell surface. We hypothesized that the decrease in highly edited B cells is due either to negative selection (death of cells that have failed to edit successfully), positive selection of cells with fewer rearrangements or escape of edited B cells to a hitherto unidentified peripheral compartment. A multimodality approach is used in the diagnosis of lymphomas. Diagnosis may be particularly challenging in some cases solely based on morphology and immunophenotyping. The molecular testing exploits the rearrangement of the immunoglobulin heavy chain and kappa genes. These genes rearrange over kilobases of genetic sequence and are unique for each normal mature B-cell and plasma cell. A reactive process, thus will demonstrate a polyclonal expansion of B-cells with different sixe rearrangements whereas a malignant process will often demonstrate expansion of a B-cell population that originates from a single cell and hence all will have the same rearrangement. Nonhomologous end-joining (NHEJ) repairs DNA double-strand breaks (DSBs) during V(D)J recombination in developing lymphocytes and during immunoglobulin (Ig) heavy chain (IgH) class switch recombination (CSR) in peripheral B lymphocytes. We now show that CD21-cre-mediated deletion of the Xrcc4 NHEJ gene in p53-deficient peripheral B cells leads to recurrent surface Ig-negative B lymphomas (CXP lymphomas). Remarkably, CXP lymphomas arise from peripheral B cells that had attempted both receptor editing (secondary V[D]J recombination of Igκ and Igλ light chain genes) and IgH CSR subsequent to Xrcc4 deletion. Correspondingly, CXP tumors frequently harbored a CSR-based reciprocal chromosomal translocation that fused IgH to c-myc, as well as large chromosomal deletions or translocations involving Igκ or Igλ, with the latter fusing Igλ to oncogenes or to IgH. Our findings indicate that EμR replacement greatly increases Igk rearrangement in pro-B cells. In addition, the germline transcription of both Vκ and Jκ regions as well as DNA demethylation at the Jκ region are substantially increased in EμR pro-B cells, indicating that Eμ is sufficient to promote the accessibility of these loci to V(D)J recombinase and activate V(D)J recombination in pro-B cells.. To determine whether PU.1 and Spi-B are required for B cell development in the bone marrow, Spi1 (encoding PU.1) was conditionally deleted in B cells by Cre recombinase under control of the Mb1 gene in Spib (encoding Spi-B)-deficient mice. The monoclonal antibody, MDX-1097, is currently progressing through clinical trials as a possible therapy for multiple myeloma. MDX-1097 targets a cell membrane bound form of free immunoglobulin kappa light chain (FkappaLC), termed kappa myeloma antigen (KMA), which is found on the surface of malignant plasma cells. The clinical potential of MDX-1097 highlights the need to characterise the expression of its cognate antigen, KMA, in normal tissue. In this study, we have analysed the expression of KMA on B cell subsets found in tonsils, peripheral blood and bone marrow. We found KMA expression on a small population of tonsillar and in vitro derived plasmablasts. In contrast, no KMA expression was observed on peripheral blood or bone marrow resident B cell subsets. This study yields important insights into the possible subsets of B cells that might be depleted as a result of an immunotherapy targeting KMA.

The VH gene repertoire of human peripheral B cells was analyzed using PCR analysis of individual blood B cells. Because genomic DNA of single B cells was analyzed, data from both productive and nonproductive VDJ rearrangements were obtained. Nine out of 75 B cells contained both functional and nonfunctional rearrangement products, whereas 62/75 had a single productive VDJ rearrangement. The distribution of VH families was ordered in accordance with the germline complexity, although a bias toward VH3 and some of its members was found. This bias was noted in both the productively and nonproductively rearranged repertoires, indicating that it resulted from molecular and not selective processes. Evidence for negative selection of certain VH3 and VH4 family members was noted in that they were found less often as productive than nonproductive VDJ rearrangements. The former are usually of high affinity and exist only in autoimmune individuals, whereas the latter often have low reactivity towards a wide spectrum of self-antigens and are present in all individuals, ordinarily without causing pathology.
