Introduction: Endoscopic submucosal dissection has been widely accepted as a standard treatment for early gastric cancers. However, endoscopic submucosal dissection for an early gastric remnant cancer post-proximal gastrectomy with jejunal interposition is not yet widespread. Case Series: A large, flat, elevated lesion was detected in an 83-year-old male (Case 1), whereas two separate flat, elevated lesions were detected in a 75-year-old male (Case 2). Successful curative en bloc resection of these lesions by endoscopic submucosal dissection using the insulation-tipped diathermic knife-2 revealed well-differentiated adenocarcinoma confined to the mucosa. No postoperative bleeding nor perforation was observed in the present cases. Conclusion: These cases underscore the importance of periodic upper gastrointestinal endoscopy follow-up and the value of endoscopic submucosal dissection using the insulation-tipped diathermic knife-2 for treating early gastric remnant cancers post-proximal gastrectomy in the
Parents/caregivers of 85 children were interviewed for long-term results.. The researchers observed perioperative complications in 12%.. The team noted that 24% died at a median of 15 months after percutaneous endoscopic gastrostomy.. The researchers interviewed 85 families with a median follow-up time of 6 years.. A substantial majority of parents/caregivers reported that the percutaneous endoscopic gastrostomy had a positive influence on their childs situation.. The researchers reported that 98% would have chosen percutaneous endoscopic gastrostomy insertion again.. Vomiting/retching improved in 61% of the children, and oral intake enhanced in 43%.. Stoma-related complications were frequent, occurring in 73%.. The researchers found that the gastrostomy tube was permanently removed at a median of 3 years after percutaneous endoscopic gastrostomy placement in 25%.. Delayed closure of the gastrocutaneous fistula after gastrostomy removal occurred in 48% of the patients.. The research team ...

In some cases, you may be referred for a gastroscopy to look inside your stomach directly and see whether you have a stomach ulcer.. The procedure is carried out in hospital and involves passing a thin, flexible tube (an endoscope) with a camera at one end into your mouth and down into your stomach and first section of the small intestine (duodenum).. You may be given a mild sedative injection before the procedure and have your throat sprayed with a local anaesthetic to make it more comfortable to pass the endoscope.. The images taken by the camera will usually confirm or rule out an ulcer. A small tissue sample may also be taken from your stomach or duodenum, so it can be tested for the H. pylori bacteria.. A gastroscopy is usually carried out as an outpatient procedure, which means you wont have to spend the night in hospital.. Read more about what happens during a gastroscopy.. ...

Results All stomach tags were identified in 87.2% (41/47) of examinations. Missed tags (marked in figure as red dots, representing an incomplete examination) included antrum (3/6), cardia (2/6) and posterior wall (1/6): none were missed in the latter 25 procedures. Mean examination times for the first 23, second 23 and all procedures were 10.28, 6.26 (p , 0.001) and 8.27 (3.25-16.32) minutes and all were completed by 4 mins after 39 procedures. The order in which tags were identified in the mid-body of the stomach (greater, anterior and posterior) was variable and interchangeable. If this area was considered as one site, the order of tag identification would be: cardia (1), fundus (2), mid body (3), lesser curve (4), antrum (5) and D1 (6) in 76.6% of examinations. No difficulties were observed with the current procedure protocol and therefore no modifications recommended.. ...

Given that aldolase B is Commonly current in kidney and intestinal mucosa and in liver, Cox et al. (1982) ended up ready to detect heterozygotes by intestinal biopsy. In a Jewish relatives, they shown that evident dominant inheritance was the result of a homozygote-heterozygote mating. Diagnosis In aldolase B-deficient tissues, cytoplasmic accumulation of fructose-1-phosphate causes sequestration of inorganic phosphate with ensuing activation of AMP deaminase that catalyzes the irreversible deamination of AMP to IMP (inosine monophosphate), a precursor of uric acid. During the cytoplasm, AMP, ADP, and ATP are managed inside a point out approaching equilibrium. Depletion of tissue ATP takes place by means of large degradation to uric acid and impairment of regeneration by oxidative phosphorylation while in the mitochondria on account of inorganic phosphate depletion ...

EUS is an advanced endoscopic technique. Very few specialists and endoscopists undergo additional training after specialization to do this properly. Typically, it takes about an additional 1-2 years of dedicated EUS training after completing specialist and general endoscopy training in order to perform EUS competently. There are various levels of difficulty in EUS. Most EUS specialists are able to perform routine cases such as simple diagnostic evaluation and FNA. However, only top EUS experts are able to perform difficult cases successfully. It is important to ensure that the doctor performing the EUS has the correct level of expertise to match the difficulty of the case.. EUS is safe when performed by properly trained doctors. The risk of diagnostic EUS is equivalent to a simple gastroscopy. Complications and failures increase significantly when EUS is performed by Inadequately trained doctors or those with insufficient cases to maintain their skill. Hence, it is important to ensure that the ...

Esophageal dilatation is a therapeutic endoscopic procedure that enlarges the lumen of the esophagus. It can be used to treat a number of medical conditions that result in narrowing of the esophageal lumen, or decrease motility in the distal esophagus. These include the following: Peptic stricture Schatzki rings Achalasia Scleroderma esophagus Rarely esophageal cancer There are three major classes of dilators: Mercury-weighted bougies are blindly inserted bougies placed into the esophagus by the treating physician. They are passed in sequentially increasing sizes to dilate the obstructed area. They must be used with precaution in patients with narrow strictures, as they may curl proximal to the obstruction. Bougie over guidewire dilators are used at the time of gastroscopy or fluoroscopy. An endoscopy is usually performed first to evaluate the anatomy, and a guidewire is passed into the stomach past the obstruction. This may also be done fluoroscopically. Bougies are again introduced-this time ...
Hi. Have been having chest pain under my left breast for the past 3 days with the feeling of fainting as well. Went to walk-in today as I was scared. They did an ECG but it came back normal. My blood pressure is O.K. sitting or standing. But when she checked my heart rate from sitting to standing it jumped significantly. So she has suggested a Holter monitor. Of course like everything I have to wait. this is not going to happen until a month from now. how is that going to help me today? the only other option she can see is the CT scan but they take forever to get into as well. I did however today get the results that my Gastroscopy all came back fine and that there is nothing wrong in that area. Also my gall bladder has 1 small polyp on it and she doesnt think that would cause me any pain. Plus the pain is under my left breast instead of my right. Also my thyroid has been checked several times she said and it always comes back normal. She said my case is a puzzler. Well I am aware of that. I ...

It wasnt denial or wishful thinking, but in the final day before starting chemotherapy I found myself really concerned about the possibility that the whole thing was a big false positive. After all, they had told me that the CT scan showed very little, the PET scan showed nothing, and the MRI scan showed nothing. The only positive evidence we had to go on was a histologists report from the gastroscopy saying malignant adenocarcinoma and the photos and expert judgement of the surgeon that what he saw was definitely secondaries all over the peritoneum. But what if I have an unusual-looking peritoneum? And could the histology results have been mixed up somehow? A bad histologist? Specimens mixed up? It would be stomach-churningly terrible to undergo chemotherapy if in fact I didnt have cancer! I hadnt seen any data with my own eyes that really convinced me. I was only agreeing to treatment because I trusted the NHS staff. But I really wanted to feel more convinced myself. (My head of ...

An endoscopy is a procedure where a gastroenterologist (a doctor who specialises in the digestive system and liver) uses a special camera to inspect the gastrointestinal tract. An endoscopy may be a gastroscopy (which looks inside the beginning of the gastrointestinal tract), a colonoscopy (which looks inside the large bowel), or both. See our fact sheet Endoscopy - colonoscopy.. A gastroscopy looks at the oesophagus, the stomach and first part of the small bowel, also known as the duodenum. This is done by using a long flexible tube that has a light and a camera on the end, known as a gastroscope.. A capsule endoscopy is another way of looking at the gastrointestinal tract, in particular the small bowel (small intestine). This involves swallowing a capsule, which is the size of a large jellybean. The capsule contains a camera, which takes photos of the inside of the gastrointestinal tract over a period of 8-12 hours.. ...

bjective: Esophageal tumors arising in the muscularis propria are difficult to be resected endoscopically using standard electro surgical techniques, even the endoscopic submucosal dissection (ESD) technique appeared recently. Our purpose is to investigate the efficacy of endoscopic ultrasound (EUS)-assisted tunnel-type ESD for resection of these tumors. Methods: A total of 17 patients were included in this study. A standard endoscope was used. The submucosal tunnel was created with the triangle knife according to the standard ESD technique, about 5 cm proximal to the lesion. EUS was performed within the tunnel to detect the tumor, and then the tumor was separated both from the submucosal and the muscle layers. After the tumor was removed, several clips were used to close the mucosal defect. EUS was performed to evaluate the healing quality 1 week after the procedure. Result: In all the cases, the tumors were completely resected. Mean tumor size was 24.2 mm (12-50 mm) in diameter. The histo ...

Between 1% and 2% of the UK population undergo gastroscopy per annum and there has been a 30% increase in demand since the publication of the National Institute of Clinical Excellence guidance for dyspepsia in 2004 (CG141).1 Despite this, in dyspeptic patients, over 70% of patients have normal gastroscopies.2 Upper gastrointestinal (GI) capsules can be controlled by patient position changes (Pillcam UGI, Medtronic, Minneapolis, USA) and good views can be achieved 80% of the time, but require a separate reading session lasting a mean of 48 min.3 On the other hand, using handheld magnets to control upper GI capsules (Mirocam Navi, Intromedic, Seoul, Korea) allows a live examination. In a single blinded comparative study of recurrent or refractory iron deficiency anaemia, patients reported better tolerance to capsule endoscopy and clinicians found more lesions likely to cause iron deficiency on capsule endoscopy than gastroscopy. Mean magnet capsule examination time by the operator was 23 min. ...

Occurrence of metastatic cancer to the stomach is rare, particularly in patients with prostate cancer. Gastric metastasis generally presents as a solitary and submucosal lesion with a central depression. We describe a case of gastric metastasis arising from prostate cancer, which is almost indistinguishable from the undifferentiated-type gastric cancer. A definitive diagnosis was not made until endoscopic resection. On performing both conventional and magnifying endoscopies, the lesion appeared to be slightly depressed and discolored area and it could not be distinguished from undifferentiated early gastric cancer. Biopsy from the lesion was negative for immunohistochemical staining of prostate-specific antigen, a sensitive and specific marker for prostate cancer. Thus, false initial diagnosis of an early primary gastric cancer was made and endoscopic submucosal dissection was performed. What happens during a gastroscopy?. Gastroscopy is usually done as an outpatient day case. It is a routine test which is commonly done. The operator may numb the back of your throat by spraying on some local anesthetic. You may be given a sedative to help you to relax. You lie on your side on a couch. You are asked to put a plastic mouth guard between your teeth. This protects your teeth and stops you biting the endoscope. The operator will then ask you to swallow the first section of the endoscope. Modern endoscopes are quite thin and easy to swallow. The operator then gently pushes it further down your oesophagus, and into your stomach and duodenum. The operator watches the screen for abnormalities of the oesophagus, stomach and duodenum. Air is passed down a channel in the endoscope into the stomach to make the stomach lining easier to see. The operator may take one or more biopsies (small samples) of parts of the inside lining of the gut - depending on why the test is done and what they ...

Editor-The finding that gastric cancer detected early has a long doubling time (several years) whereas that diagnosed in an advanced stage has a short doubling time (several months)1 is consistent with the well known length bias. A non-aggressive course is associated with a long subclinical period with a high chance of early detection, whereas aggressive growth with a rapid onset of severe clinical symptoms leaves little chance of recognition of disease in an early stage. This confounds the observation that early cancer has a better prognosis than late stage cancer. The low case fatality from gastric cancer in Japan is a logical consequence of such biases, given that over six million Japanese people (presumably a selection of healthy people) have stomach screening annually. The suggestion that patients with new onset dyspepsia and with changes in long standing dyspepsia must be referred promptly [for gastroscopy] 1 is therefore not evidence based.. In practice, early detection of cancer ...

My beautiful Mom was admitted to a private hospital in South Africa on the 4th of May. She has been treated by her doctor for an ulcer, which came and gone after my Dads passing. She was also treated for Kidney pipe infection. But that Wednesday morning, she was in bed, complaining of alot of pain on the sides of her chest and struggling to breath due to the pain. We had to let the ambulance take her to the hospital, because she couldnt be moved. After x-rays, blood tests, and CT scans, the doctor informed me that she will be admitted to the wards to remove a kidney stone. So while she was in hospital, medications for pain was given, but nothing helped to ease her pain. She was so much in pain, she couldnt lift herself in bed or even walk. The surgery, to remove the stone was scheduled, as well as a gastroscopy, for Friday the 6th of May. After the surgery, we were told that the stone was not removed, because it is not the reason for her pain. Instead they only did a gastroscopyo, and took ...
