Postoperative gastrointestinal dysfunction (PGD) is a common complication following laparotomy under general anesthesia (GA). Abdominal distension occurs in 8-28% of surgeries within 24 h postoperatively. The present study aimed to analyze the efficacy of electroacupuncture (EA) for the prevention of PGD by applying preoperative EA stimulation of PC6 (Neiguan), ST36 (Zusanli), and ST37 (Shangjuxv) bilaterally twice within 24 h prior to surgery, compared with no acupuncture treatment. The study participants were assessed and selected from participants undergoing vascular laparotomy under GA at the Liver and Vascular Surgery Unit in West China Hospital of Sichuan University. Animal models suggest that a neonatal stress such as invasive procedures and maternal separation could be responsible for visceral hypersensitivity and FGID. We tested the hypothesis that congenital esophageal atresia (EA), a condition corrected during the neonatal period and associated with multiple stresses, is a clinically significant risk factor for the development of FGID later in life. We postulated that, to be clinically significant, the effect of neonatal stress on the incidence of FGID should be as strong as that of enteric infections in the development of irritable bowel syndrome in children. However, studies are rarely conducted to determine the prevalence and clinical profile of AP-FGIDs in 5-12-year age group. METHODS A cross sectional survey was conducted in four randomly selected schools in Gamapaha district of Sri Lanka. All children aged 5-12 years were recruited after obtaining parental consent. Data was collected using a translated and validated parental questionnaire. AP-FGIDs were diagnosed using Rome III criteria. A central difficulty in recognizing and characterizing gastrointestinal dysfunction with ASDs is the communication difficulties experienced by many affected individuals. A multidisciplinary panel reviewed the medical literature with the aim of generating evidence-based recommendations for diagnostic evaluation and management of gastrointestinal problems in this patient population. Even patients with common functional GI disorders such as irritable bowel syndrome (IBS), functional dyspepsia and functional heartburn were reluctant to accept such a concept, wrongly assuming their symptoms would be explained in psychological terms. The emerging research into the gut microbiome has dramatically changed the equation in the last five years, with the study of brain-gut microbiome (BGM) interactions taking center stage in many fields and specialties, including gastroenterology, cardiology, chronic pain and psychiatry, to name only a few.
Each patient developed severe neuropathic pain within 8 weeks of initiating intensive glycemic control. Nine patients (age range, 19-29) had type 1 diabetes, and 7 patients (age range, 31-58) had type 2 diabetes. Other common causes of neuropathy were ruled out. Average glycosylated hemoglobin levels were about 14% before intensive glycemic control and about 7% afterward. Pain was in a stocking-glove distribution in 13 patients and was diffuse in 3 patients. AIM: To study the different endocrine cell types in the oxyntic mucosa of patients with irritable bowel syndrome (IBS).. METHODS: Seventy-six patients with IBS were included in the study (62 females and 14 males; mean age 32 years, range 18-55 years), of which 40 also fulfilled the Rome III criteria for functional dyspepsia (FDP). Of the entire IBS cohort, 26 had diarrhea as the predominant symptom (IBS-D), 21 had a mixture of diarrhea and constipation (IBS-M), and 29 had constipation as the predominant symptom (IBS-C). Forty-three age and sex-matched healthy volunteers without any gastrointestinal complaints served as controls. The patients were asked to complete the Birmingham IBS symptom questionnaire. Both the patients and controls underwent a standard gastroscopy, during which three biopsy samples were taken from the corpus. Methodology was developed to measure the glycoprotein content of the whole stomach as an index of changes in the gastric mucus. Indomethacin significantly reduced the glycoprotein content of the stomach, whilst calcitonin reversed this reduction to near normal levels and carbenoxolone sodium to significantly higher than normal levels. Despite this reversal, carbenoxolone sodium failed to reduce the ulceration produced by indomethacin. In contrast, calcitonin significantly inhibited the indomethacin-induced ulceration. Results Faecal calprotectin was requested in 2 groups of patients: 68 with lower gastrointestinal symptoms and 44 with Inflammatory Bowel Disease (IBD). Of the 68 patients with lower gastrointestinal symptoms, faecal calprotectin was normal in 48 patients: 13 (9 males, 4 females, median age 30 years, commonest symptom abdominal pain) at initial presentation had no further investigation, 23 (11 males, 12 females, median age 41, commonest symptom abdominal pain) went onto have further investigations (12 colonoscopies, 3 flexible sigmoidoscopies and 1 small bowel meal and follow through; all normal); in 12(3 males, 9 females, median age 45, commonest symptom chronic diarrhoea) the negative faecal calprotectin was as a second line following initial investigation including 8 normal colonoscopies and 1 normal flexible sigmoidoscopy. The investigators found that patients who started treatment with non-aspirin NSAIDs were at greater risk of experiencing atrial fibrillation or flutter when compared with patients who did not use NSAIDs. The increased risk differed depending on the drugs used: there was about a 40 percent increased risk for those who used non-selective NSAIDs and 70 percent among those who used COX-2 inhibitors. The research team, led by Professor Henrik Toft Sorensen at Aarhus University Hospital in Denmark, also identified certain groups at greatest risk for atrial fibrillation in the study. They included older people and patients with chronic kidney disease or rheumatoid arthritis who started treatment with COX-2 inhibitors. Growth retardation in childhood may be one of the earliest signs of an underlying disease, such as coeliac disease. In the Netherlands, the growth of nearly every child is monitored. When growth is retarded, the child is referred to secondary health care. After referral it has been advised to perform a diagnostic work-up containing routine laboratory tests to search for diagnostic clues for, among others, coeliac disease. The tests presently used for coeliac disease are IgA EmA and IgA antitissue transglutaminase antibodies. The total immunoglobulin A count is determined as well, because coeliac disease is associated with IgA deficiency.
