THE CLINIC OF PEDIATRIC GASTROENTEROLOGY AND HEPATOLOGY is a subdivision of The Department of Pediatrics, Higher Medical School in Sofia. Diagnosis and management of the diseases of gastrointestinal tract, liver and pancreas as well as teaching students and research are the basic activities of our clinic.
Normal bowel patterns are very variable in children of all age groups. When there is delay in defaecation with difficulty or distress, the child is said to suffer from constipation. Constipation has also been defined as passing less than 3 stools per week. Constipation is a common problem. It is said to account for 3% of all visits to a paediatric practice and 25% to a paediatric gastroenterology clinic. Many children presenting with constipation are said to show stool-withholding manoeuvres e.g. crossing their legs or sitting up on their heels. Serious organic causes are rare and account for , 10% of constipation in children.. Generally, constipation starts after a specific identifiable trigger. A viral infection may result in decreased intake of liquids. Stools become solid causing the child to strain and passage of hard faeces may result in an anal fissure. Defaecation becomes painful which starts a negative cycle, leading to retention of stools. Other common triggers are unavailability of ...
To determine within one tertiary care center: 1) the variation between providers in testing for celiac disease in children with chronic abdominal pain; 2) the characteristics of those children who were more likely to be tested, and 3) the prevalence of celiac disease in those evaluated. Retrospective review of children with a primary complaint of chronic abdominal pain referred to a tertiary care childrens hospital for pediatric gastroenterology evaluation over a 2-year period was conducted. Children with at least two visits and without an identified organic etiology for the pain were included. 160 children were evaluated by 16 pediatric gastroenterologists and one nurse practitioner. Celiac serologic testing was completed in 63 (39.4%) children. There was no significant variance in the frequency of celiac serologic testing between providers. Child age, gender, body mass index, and baseline gastrointestinal symptoms did not predict whether celiac serologic testing occurred, though Caucasians (P | 0.01)

BACKGROUND: Although best practice guidelines for dyspepsia management have been disseminated, it remains unclear whether providers adhere to these guidelines. AIM: To compare adherence to best practice guidelines among dyspepsia experts, community gastroenterologists and primary-care providers (PCPs). METHODS: We administered a vignette survey to elicit knowledge and beliefs about dyspepsia including a set of 16 best practices, to three groups: (i) dyspepsia experts; (ii) community gastroenterologists and (iii) PCPs. RESULTS: The expert, community gastroenterologist and PCP groups endorsed 75%, 73% and 57% of best practices respectively. Gastroenterologists were more likely to adhere with guidelines than PCPs (P < 0.0001). PCPs were more likely to define dyspepsia incorrectly, overuse radiographic testing, delay endoscopy, treat empirically for Helciobacter pylori without confirmatory testing and avoid first-line proton pump inhibitors (PPIs).
