A look at hospital inpatient formularies and drug use for pneumonia reveals a dramatic shift in therapy over the last five years. This information is based on a panel of representative hospitals reporting their formulary and drug usage data by diagnosis on an ongoing basis to MediMedia USA. This ongoing study (the Hospital Diagnosis and Therapy Audit) monitors all inpatients in the panel hospitals and the drug therapy they received.. Based on an analysis of formulary and drug usage, cephalosporins remain the most widely prescribed therapy for inpatients with pneumonia; however, the introduction of quinolones to hospital formularies has eroded the cephalosporin market share considerably in the past few years. Seventy-six percent of pneumonia patients received some type of cephalosporin in 1998 compared to only 59 percent in 2002. Simultaneously, systemic quinolone therapy rose from 28 percent in 1998 to over 56 percent in 2002. In the quinolone market, Levaquin has remained on top, replacing ...

In 1995, changes in our hospital formulary were made to limit an outbreak of vancomycin-resistant enterococci and resulted in decreased usage of cephalosporins, imipenem, clindamycin, and vancomycin and increased usage of b ta-lactam/b ta-lactamase-inhibitor antibiotics. In this report, the effect of this formulary change on other resistant...
Not long ago serious infections with E.coli and K. pneumoniae were treatable with a wide variety of antibiotics. The evolution of ESBL and AmpC producing strains has changed things. Now it is common in Canada to see infections with strains that are resistant to all of the "workhorse" hospital antibiotics (ceftriaxone, piperacillin-tazobactam and ciprofloxacin) making carbapenems the primary agents. Carbapenem use is rising and carbapenemase producing organisms are appearing on our shores - a very large concern.. Cefepime first became available in 1994 and was marketed as the first "4th generation" cephalosporin with a spectrum of activity similar to ceftazidime but with standard Q12H dosing. It is approved for the treatment of pneumonia, urinary tract infections, skin and soft-tissue infections, intra-abdominal infections and febrile neutropenia. Despite its broad label indications it has never been used widely in Canada. On the current BC provincial hospital formulary it is restricted to ...
BACKGROUND: Therapeutic interchange of a same class medication for an outpatient medication is a widespread practice during hospitalization in response to limited hospital formularies. However, therapeutic interchange may increase risk of medication errors. The objective was to characterize the prevalence and safety of therapeutic interchange. METHODS AND FINDINGS: Secondary analysis of a transitions of care study. We included patients over age 64 admitted to a tertiary care hospital between 2009-2010 with heart failure, pneumonia, or acute coronary syndrome who were taking a medication in any of six commonly-interchanged classes on admission: proton pump inhibitors (PPIs), histamine H2-receptor antagonists (H2 blockers), hydroxymethylglutaryl CoA reductase inhibitors (statins), angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor blockers (ARBs), and inhaled corticosteroids (ICS ...
The National Institute for Occupational Safety and Health (NIOSH) Alert: Preventing Occupational Exposures to Antineoplastic and Other Hazardous Drugs in Health Care Settings was published in September 2004 (http://www.cdc.gov/niosh/docs/2004-165/). In Appendix A of the Alert, NIOSH identified a sample list of major hazardous drugs. The list was compiled from information provided by four institutions that have generated lists of hazardous drugs for their respective facilities and by the Pharmaceutical Research and Manufacturers of America (PhRMA) from the American Hospital Formulary Service Drug Information (AHFS DI) monographs [ASHP/AHFS DI 2003]. The 2004 list was updated in 2010; this update adds 26 drugs to the 2010 list. These additions are new drugs or existing drugs that had new warnings from 2007 to 2009 ...
Objective.-It is controversial whether physicians interactions with drug companies affect their behavior. To test the null hypothesis, that such interactions are not associated with physician behavior, we studied one behavior: requesting that a drug be added to a hospital formulary. Antimicrobial therapy is a cornerstone of therapy in critically ill patients; however, the wide use of antibiotics has resulted in increased antimicrobial resistance and outbreaks of resistant disease. To counter this, many hospitals have instituted antimicrobial stewardship programmes as a way to reduce the inappropriate use of antibiotics. However, uptake of antimicrobial stewardship programmes has been variable, as many clinicians fear that they may put individual patients at risk of treatment failure. In this paper, I argue that antimicrobial stewardship programmes are indeed a form of bedside rationing, and explore the risks and benefits of such programmes for individual patients in the intensive care unit, and the critically ill population in general. Using Norman Daniels Accountability for Reasonableness as a framework for evaluating resource allocation policies, I conclude that antimicrobial stewardship programmes are an ethically sound form of bedside rationing. ...

Antimicrobial stewardship program (ASP) is a distinguished method to improve the prescription and efficacy of antibiotics. The efficacy of ASP and conventional methods was compared to measure the effectiveness of meropenem (MPM) and vancomycin (VMN) antibiotics in pediatric patients. In an interventional quasi-experimental study, 135 children admitted in Childrens Hospital affiliated to University of Medical Sciences in time periods of 2014-2015 and 2015-2016 were assessed. The conventional and ASP methods in 2014-2015 and 2015-2016 were respectively utilized to provide the best antimicrobial therapy of MPM and VMN antibiotics in patient children. The data of mortality rate (MR), antibiotic prescription (AP), antibiotic dose (ADe), antibiotic duration (ADn), length of hospital stay (LOHS), and blood cultures (BCs) were compared across the years using the Chi square, independent t test, and Fishers exact test. The levels of MR, AP, ADe, ADn, LOHS, and positive BCs using the ASP method in 2015-2016 were

Enterobacteriaceae bloodstream infections (EB-BSI) are a common manifestation of Gram-negative sepsis, and are initially managed with empiric intravenous (IV) antibiotics. Upon stabilization and source control, patients are often transitioned to an oral agent. The fluoroquinolone (FQ) class plays a prominent role in stepdown therapy for severe infections due to favorable pharmacokinetic parameters; however, serious adverse effects have been documented with their use. Two hundred and twenty-four adults with EB-BSI initiated on empiric IV antibiotics with stepdown to oral beta-lactam (BLM) (n= 84) or FQ (n= 140) were included to compare clinical success and identify risk factors for treatment failure ...
Abgentis goal is to provide a new broad-spectrum IV/oral antibacterial to treat acute and chronic, life-threatening and drug-resistant infections. Based on a previously marketed but uncompetitive compound, Abgentis "fast-follower" programme will deploy unique insight into the structure-activity relationships, microbiology, pharmacology and efficacy of antibacterial DNA supercoiling inhibitors to rapidly re-engineer and optimise, to create an internationally competitive product. The keys to market penetration are the IV and oral bioavailability (enabling step-down therapy), the spectrum of activity including both Gram+ve and Gram-ve pathogens and the ultra-low spontaneous resistance frequency.. ...

This secondary data analysis found that most patients admitted to the hospital have their medications changed to another medication of the same class to be consistent with the hospital formulary. Researchers found that patients who undergo this therapeutic interchange are more likely to have errors in medication reconciliation at the time of hospital discharge compared to patients whose medications are not changed.
NEW ORLEANS — Switching inhaler types due to insurance formulary changes may lead to reduced lung function in children with asthma, according to data presented at the CHEST Annual Meeting. “Insurance formulary changes have been a major reason why many of our children with asthma who were otherwise stable have been switched from one long-term controller to another. Infection with MDRO is frequently observed during clinical treatment [15-17], especially in critically ill patients [18]. Preventing infection with MDRO is very important to our clinic workers. To the best of our knowledge, this is the first study concerning the effect of the antimicrobial stewardship program of Chinese MOH on colonized or infected with MDRO in critically ill patients. In the present study, we found that DDD in the hospital and in most departments decreased significantly after the implementation of the antimicrobial stewardship.. Consequently, the MDRO isolation in critically ill patients correspondingly decreased when they admitted into ICU. However, we found a low relationship between DDD in the ICU and increased MDRO rate from ICU admission to discharge. Moreover, we did not find a positive relationship between DDD in the ICU and MRDO at ICU or between the DDD of carbapenem and carbapenem-resistant A. baumannii.. Antimicrobial stewardship can reduce the DDD in the entire ...
Certain formulary designs could cut prescriptions of costly compounded drugs.. Compounds, which usually are customized for patients, include topical creams and gels, injections, oral liquids, anesthetics, anticonvulsants, analgesic painkillers and muscle relaxants.. Texas closed drug formulary requires preauthorization to prove medical necessity only when compounded medications contain "N drugs," such as OxyContin. However, compounds made up entirely of "Y drugs" are treated as approved medications and dont require preauthorization. "We saw an immediate decrease in N drug compounds in Texas" when its closed formulary went into effect in 2011, said Brian Allen, Westerville, Ohio-based vice president of government affairs at pharmacy benefit manager Helios. "It was like a cliff - they just fell off. But what we have since seen is a commensurate spike in compounds that are all Y drugs. ". In Texas, Y drug compounds are subject to retrospective review, which means "theyre getting through ...

New Hampshire health officials have discovered one additional case of hepatitis C related to the outbreak at Exeter Hospital, bringing the total number of infected patients to 31. The New Hampshire Department of Health and Human Services, Division of Public Health Services, announced Tuesday that its public health lab has confirmed another former hospital patient is carrying the virus linked with the outbreak. All 31 patients were treated in Exeter Hospitals cardiac catheterization laboratory between Oct. 1, 2010 and May of this year. A former hospital technician is accused of spreading the disease to patients in the course of abusing stolen hospital drugs.
The fundamental building blocks addressing the prevention of skin breakdown are generally overshadowed by the deluge of intervention strategies touted for patients with chronic wounds. It is paramount that providers take these proactive steps in clinical practice to develop sound skin care prevention and intervention pathways.. Use the following skin care product categories to help you develop a skin care formulary for your facility. When creating the skin care formulary, remember to include products under these categories. And be sure to understand the reimbursement guidelines for all products ordered. You may find that many skin care products are considered routine supply items and are included in the general cost of an inpatient stay, whether the stay is for acute or long-term care. It is the clinicians responsibility to verify whether a product has been assigned a Healthcare Procedure Coding System Code (HCPCS). The ultimate responsibility for correct HCPCS coding lies with the provider or ...

However even providing one NRT product plus ongoing support from the Service will still double clients chances of quitting compared to no support.. Evidence shows that adding in a second NRT product can further increase the chances of quitting. Therefore the Service will advise that clients can purchase small packs of a second NRT product (such as chewing gum, lozenges which typically cost £5-£6 for a pack of 20) or choose to purchase an unlicensed nicotine containing product such as an e-cigarette..
