One of the core practices of your certified organic operation is developing good quality recordkeeping and documentation (commonly referred to as an audit trail). The most important element of a good audit trail is establishing a common link between all steps and all documents in your control system. For instance, using lot code numbers can easily allow an inspector or you to work through each phase of your operation - ordering, receiving, processing and distribution to name a few - to make documentation traceable, timely and transparent.. During the inspection, you should expect to walk your inspector through at least two audit trail exercises to determine the effectiveness and accuracy of your recordkeeping systems.. There are three types of audit trail exercises:. Sales/financial audits. ...

This week 54-year-old Gary Dudek was arrested for allegedly heisting skin grafts. He worked at a regenerative medicine firm in Pennsylvania where he ordered epidermal material for Mercy Philadelphia hospital. Between November 2011 and July of 2013, he may have ordered up to 200 fleshly splices without authorization. The hospital never received the grafts so authorities went looking for the missing skin after Mercy Hospital noticed they were missing during a financial audit.. Officials say Dudek was caught on tape taking epidermis samples, meant for surgery, from the hospital and putting them in his car. No one is aware of his motive but conjecture has it that he may have been somehow trying to make money from them. Each graft is worth about $1,700.. Stealing and selling human organs and tissue, though unscrupulously bizarre, is not totally unique. According to Annie Cheney, author of Body Brokers: Inside Americas Underground Trade in Human Remains, human corpses are supposed to be buried, ...
During the Mid Term Review (MTR) in March 2008, the project team proposed an in-depth restructuring of the project in order to address the following problems: i) weak M&E framework, namely inadequate indicators; ii) low disbursement levels for components 1 and 2; iii) inadequate coordination between the components; iv) lack of integrated planning; and v) weak health information system. The proposed restructuring reformulated objectives (with clearer indicators) and streamlined the project components. However, due to the coup in August 2008, the restructuring process was put on hold and project implementation was suspended. After Bank operations were resumed in September 2009, the Bank proposed that the Government submit a formal request for restructuring; however, the submission of the request was delayed due to non-compliance with a financial audit. By the time the Bank received the request, the project period was nearly ended and the Bank decided not to proceed with restructuring. While the ...
Auditing typically refers to financial auditing of public companies. Financial audits are conducted by independant external auditors, who provide an opinion about a companys financial reports. These auditors must adhere to Generally Accepted Auditing Standards (GAAS) in the United States, and their opinions give investors reasonable assurance that financial reports are true, fair, and adhere to applicable accounting standards. The auditor must also include an opinion about the effectiveness of a companys internal controls. ...
In the wake of the international publicity the Nebraska Peace Foundation -- the 501 (c) 3 arm of Nebraskans for Peace -- generated with its shareholder resolution at the 2016 Berkshire Hathaway Shareholder Meeting on April 30th, the foundation has submitted a follow-up resolution for consideration at the forthcoming May 6, 2017 meeting in Omaha.. Earlier this year, in both the 2015 Annual Shareholder Letter and his remarks at the 2016 Berkshire Hathaway Annual Meeting of Shareholders itself, Warren Buffett did the world and the financial community a great service by acknowledging both the reality and the threat of climate change. In fact, he did so not just once, or in passing - he made this message an explicit focus.. In follow-up, a shareholder proposal has been formally submitted for a vote at the 2017 Annual Shareholders Meeting (proposal text attached below). Downloadable (with restrictions)! Shareholders have increased their ability to influence management decisions significantly over the past two decades. This paper proposes an extension of the Post Keynesian theory of the firm proposed by Lavoie (1992) to analyze the effects of this change in shareholder power on investment decisions. This microeconomic analysis is complemented by a Kaleckian macroeconomic model. Shareholder power is found to reduce investment and output, while increasing profits, which is consistent with the stylized facts of the neoliberal era.

While the results from the national program are trending in a positive direction for CMS, the agencys data does not provide a complete picture. Other factors must be assessed to determine whether the program, as a whole, is positive for all entities within the Medicare FFS system. Notably, the RAC program must also be analyzed from the providers perception. From their point of view, the national Recovery Auditor program has become an administrative nightmare.. The American Hospital Association (AHA) created a program called RACTrac, a free, web-based survey developed in response to what AHA felt was a lack of data provided by CMS on the impact of the RAC program on hospitals. Recently, AHA enhanced the RACTrac survey to capture more detailed information on medical necessity review denials and administrative difficulties due to problems with the RAC post-payment review process. Data from the latest RACTrac survey for the first quarter of 2012 yield some interesting results.. The 2012 first ...

In early February, U.S. Senator Mary L. Landrieu (D-La.) introduced legislation aimed at amending the Patient Protection and Affordable Care Act (PPACA) to exclude insurance agent and broker commissions from the calculation of administrative costs. Senators Johnny Isakson, (R-Ga.), Ben Nelson (D-Nebr.) and Lisa Murkowski (R-Ark.) signed on as co-sponsors of the bill.. The cost calculation is vital to the computation of new pricing rules under the PPACA, which require carriers to establish premium rates based on a Medical Loss Ratio (MLR). The MLR measures the expenses of insurance carriers and forbids non-medical payment expenses to exceed 20% of premiums charged.. As Secretary of Health and Human Services (HHS) Kathleen Sebelius explains on her blog: The laws 80-20 Rule requires insurers to spend at least 80 cents of each premium dollar on actual health care services and activities that improve health care quality, rather than administrative costs and CEO bonuses. If insurers dont abide by ...

The Faulty Math Behind Medicares Low Administrative Costs. In the private sector, administrative costs are calculated using the medical loss ratio (MLR). Financial analysts use medical loss ratios to calculate an insurers gross margin. A private insurers medical loss ratio consists of the ratio of how much the insurer has paid to health-care providers in claims divided by how much the insurer has collected in premiums.. If an insurer has paid less in claims than it has collected in revenues, the remaining funds can be used to pay for its administrative costs, such as employee wages, business expenses, and a minimal profit margin. From 1960 to 2010, the average administrative cost ratio for private insurers was 12 percent. The administrative costs of Medicares government-run hospital insurance program, by contrast, is said to be only 3 percent.. There are substantial problems with this comparison. Medicare covers nearly all Americans older than 65; private insurers cover Americans younger ...

I do want to see that happen. The answer to this is much more transparency about the levels of pay, much more accountability, and strengthening the hand of shareholders. There is something else we need to do, which is to make sure that non-executive directors on boards are not the usual sort of rotating list of men patting each others backs and increasing the level of remuneration. I want to see more women in Britains boardrooms, which I think would have a thoroughly good influence ...

First of all, the process of deregistration is relatively simple, quick and inexpensive and is instigated by the lodgment of ASIC Form 6010 after certain pre-requisites have been met by the directors. One of those pre-requisites is that the company must have less than $1,000 in assets, which means that most of the companys assets must be distributed to shareholders before deregistration can occur. The normal method of distribution is by dividend which is generally taxable in the hands of the shareholder pursuant to section 44 of the Income Tax Assessment Act 1936. However, where a company has certain exempt profits, such as capital gains on pre CGT assets, the dividend remains taxable in the hands of the shareholder when received. Clearly this is not a desirable outcome for the shareholders. ...

Often, breathing disorders can seem similar to serious disease when in reality the patient may not be getting an adequate breath. In fact, hyperventilation syndrome (HVS) and breathing pattern disorders (BPD) have the following incidence: 10% of general medicine practice patients have HVS/BPD as their primary diagnosis. Female:male is about 2:1 to 7:1; most commonly in the 15-55 year age group. Acute HVS only makes up about 1% of cases. Normal Breathing The normal resting breathing rates equate to around 10-14 breaths per minute, which moves around 3-5 liters of air per minute through the airways. Not so Normal Breathing HVS/BPD can be defined as a pattern of overbreathing where the depth ...
The Centers for Medicare and Medicaid Services (CMS) continues to mull over the knotty problem of what it means to identify an overpayment from the government. Healthcare providers do not have the same luxury. Five years ago, the Affordable Care Act required a provider that received an overpayment from the government to report and repay it within 60 days of identifying the … Continue Reading ...
This is a prospective, multi-centered study to assess whether urine metabolomics can play a role in the screening of colorectal cancer (CRC). Urine samples will be collected from 1000 patients going through an established CRC screening program, and from a further 500 patients who already have a diagnosis of CRC. Using nuclear magnetic resonance (NMR) spectroscopy, the 1H NMR spectrum of urine samples will be analyzed for specific metabolites, and establish the metabolomic signature of colorectal cancer. The results from metabolomic urinalysis of this screening cohort will be compared with results from colonoscopy, histological descriptions, fecal occult blood testing (FOBT), and fecal immune testing (FIT) to assess the accuracy of urine metabolomics in identifying patients with polyps and malignancies. The urine metabolomic results from the colorectal cancer group will be correlated with operative, histological and clinical staging to define the role of urine metabolomics in assessing colorectal ...
On February 5, 2001, the Department of Banking and Insurance (Department) proposed new rules (N.J.A.C. 11:4-53) to permit the sale of specified disease and critical illness policies in New Jersey and to establish standards applicable to the sale of such policies (see 33 N.J.R. 361(a)). N.J.A.C. 11:4-53.6 addressed the loss ratio standards for these policies. As proposed, N.J.A.C. 11:4-53.6 would require at least a 75 percent loss ratio for group policies, and at least a 65 percent loss ratio for individual policies. The Department received several comments from the industry objecting to the proposed minimum individual loss ratio requirements, and requesting that they be lowered. These comments appear in the Departments adoption of N.J.A.C. 11:4-53, which is published elsewhere in this issue of the New Jersey Register. The Department reviewed the individual loss ratio requirements in light of the comments received. The Department determined that the 65 percent individual loss ratio is ...
Thanks. After first getting delisted and then filing for bankruptcy, things have not been going well for them and MadPunsters short pick has played out superbly. The most recent update was that effective May 1st, all shares have been cancelled, so they no longer trade at all. However, given their assets where purchased and there seems to be some expected recovery net to the common shareholders, I closed out the short position at the last trade price of $0.0064 (less than a penny) - congratulations for the 97% gain to Crypto-o-Currencies fund investors. Having read their bankruptcy filings, it appears that this last trade price is probably a little higher than shareholders are likely to get, but given these things tend to take years to finalize all the costs and make the last distributions to shareholders, closing it out at the last market price seems good enough for contest purposes ...
Shawn, when you say At least I know Medicare doesnt have shareholders to please every quarter -- it has tax-paying citizens to please, are you remembering the big difference between these two situations? Shareholders own the stock voluntarily. If you dont please the shareholders, they will sell the stock and take their money elsewhere. Taxpayers, on the other hand, have no such choice and thus there isnt the same need for accountability. Medicare can do a horrible job and the taxpayers still pay, especially as long as those who get the entitlements vote for politicians who will legislate to keep growing the bloated bureaucracy. Without the accountability of CHOICE, its like the difference between the government postal service vs Fedex or UPS. Giving more control to politicians will make the situation worse. ...
The review panel of the Australian Takeovers Panel canceled the order prohibiting Rio Tinto from compulsorily acquiring shares in Energy Resources of Australia Ltd., noting that the orders would be unfairly prejudicial to Rio Tinto. The panel added that Energy Resources needs to disclose to its shareholders Rio Tintos intentions regarding compulsory acquisition if the latter becomes a 90% shareholder, following the formers A$476 million entitlement offer and underwriting agreement.. * Buoyed by strong iron ore prices in 2019, analysts are forecasting that shareholders of Rio Tinto and Fortescue Metals Group Ltd. will receive huge payouts, with an analyst from Macquarie estimating that Rio Tinto may pay out US$5.62 per share, topping its record dividend of US$5.50 per share in 2018, The Australian Financial Review reported.. * Finnish stainless steelmaker Outokumpu Oyj expects adjusted EBITDA of about €70 million for the fourth quarter of 2019, rising from earlier guidance of about €45 ...
