In the first double-blind, placebo-controlled surgical trial testing the safety and effectiveness of fetal dopamine cell implants for the treatment of Parkinsons disease, many patients who received the implants showed growth of the new brain cells and improvement in their symptoms. Compared to patients who received the placebo operation, transplanted patients were better able to move and perform other activities before taking their medications. The study also showed that there was a great deal of variability in the transplant outcome, so that the benefit for any individual patient was unpredictable. In this study, we measured circulating nucleic acids in patient-derived orthotopically implanted xenograft (PDOX) models of GB (n=64) and show that tumor size and cell proliferation, but not the integrity of the blood-brain barrier or cell death, affect the release of ctDNA in treatment naïve GB PDOX. Analysis of fragment length profiles by shallow genome-wide sequencing (,0.2x coverage) of host (rat) and tumor (human) circulating DNA identified a peak at 145 bp in the human DNA fragments, indicating a difference in the origin or processing of the ctDNA. This mouse model was used to test compounds for antiviral efficacy. Two weeks after infection with HIV, 100 percent (40/40) of SCID-hu mice were positive for HIV by the polymerase chain reaction. When first treated with 3-azido-3-deoxythymidine (AZT), none (0/17) were HIV-positive by this assay. However, AZT-treated SCID-hu mice did have a few infected cells; after AZT treatment was stopped, viral spread was detected by polymerase chain reaction in such mice. Human fetal spinal cord tissue was obtained from the Einstein Human Fetal Tissue Repository (New York, NY) as approved by the Institutional Review Board. Tissues from abortuses of normal women were collected after elective pregnancy termination. Informed consent was obtained from all tissue donors. The ages of the abortuses were determined by multiple parameters, including the date of the last menstrual period by history, uterine size by bimanual and abdominal examination, ultrasonography by using predominantly the maximum biparietal diameter, and, postabortally, by measurement of fetal foot length (Streeter, 1920; Hern, 1984). After retrieval of tissue the spinal cords were stored in sterile medium with antibiotics on ice and were obtained from the neuropathologist within the hour. We evaluated cardiac muscle development in the absence of hemodynamic work load but in the presence of host factors including blood vessels, nerves, and circulating neurohumoral agents by transplanting 12-day fetal rat ventricle into the anterior eye chamber of adult host rats. Implants were studied by electron microscopy at intervals from 1 to 14 weeks in oculo. For comparison with myocardium developing in oculo, 12-day fetal tissue and 3-, 8-, and 28-day-old normally growing rats were also studied. At 1 week in oculo, myofibrils were laterally located and more frequent than in the 12-day fetus. Fibrils had clear Z bands and H bands, but no M bands. At 10 days in oculo (comparable to birth in normally growing animals), myocyte mitoses were present and tritiated thymidine autoradiography revealed many labeled myocyte nuclei. Neural cell transplantation therapies have significant translational potential to promote regeneration post-injury with clinical trials commencing for various pathologies. However, there are challenges associated with current clinical approaches used for systemic or direct delivery of transplant cells to neural tissue in regenerative applications. These include risks associated with surgical microinjection into neural tissue (e.g. haemorrhage, cell clumping) and high cell loss due to systemic clearance or with cell passage through fine gauge needles into densely packed neural tissue. These data highlight two essential principles that may help realize the functional potential of striatal transplants. First, it was demonstrated that distinct response deficits were alleviated only if grafted animals received extensive postoperative training. Second, comparisons of different postoperative training regimes revealed that only specific training on the impaired contralateral side conferred functional benefit.
