We read with interest the report of Frolich and colleagues (1) of opioid toxicity in a patient with a 3-day-old transdermal fentanyl patch in situ, undergoing surgery under lumbar epidural anesthesia and IV propofol. The opioid toxicity was related temporally to the use of a body-warming blanket to correct intraoperative hypothermia. In our study of the perioperative pharmacokinetics of transdermal fentanyl, we found that two elderly patients experienced marked respiratory depression that persisted after fentanyl patch removal. In one of these patients, plasma fentanyl concentrations were high (2.22 ng · mL− 1), and in the other, plasma concentrations were low (0.645 ng · mL− 1), but the patient had required relatively large amounts of morphine via a patient-controlled analgesia device (2). Previous studies have reported that fentanyl is eliminated predominantly by hepatic biotransformation, and that some is eliminated unchanged in urine and stools. No reports have described the elimination of fentanyl via the lungs. In this study, exhaled gas samples from eight anaesthetized patients undergoing cardiac surgery were analysed using solid-phase microextraction (SPME) coupled with gas chromatography-mass spectrometry (GC-MS). Hi Lucia,. My PCP initially started me on the patch a few yrs back. I was on the 25s and they did offer me some relief at the time. I was hurting so bad at that point I really did not care if it was a pill or a patch. I just needed some relief because emotionally the pain had begun to take a toll on me. My PCP does not, I repeat does not dole out pain meds either. I was a long time patient and he knew when I told him something I was being honest. It was my gastro dr that ref me to a pain specialist. My gi was as shocked as me that the PCP would write a script for a patch because most PCPs do not want the responsibility that goes with writing a triplicate script.. I stayed on the 25s for awhile and was taking BT meds only in dire circumstances. I then had to have an increase to the 50 because 25 lost its effectiveness. I was on fentanyl patches for a couple of months & I did not have much trouble getting them to stick. I know that this is a common problem, but I never had any major difficulties. I used the generic brand from Watson. There was always a little tab on the end of the patch that did not stick, so that you could easily pull the patch off -- I dont know if this is what you are referring to? I used to wear them on my chest & on the side of my torso, although mainly on my side. I tried to place them such that they wouldnt be too effected by the movement of my body. If I held them down for ~30 sec, then the were usually good to go for the 3 days. If I ever had problems, it would be because I placed the patch near a piece of clothing that would rub. Then, sometimes, the corners/edges would start to peel off. If that happened, Id just tape them down with cloth bandaging tape (just be sure not to cover the patch with non-clear tape, as this can affect the rate of absorption of the meds). If you are ...
