Anorexia Nervosa (AN) has the highest mortality rate compared to any other psychiatric disorder. The most promising treatment for adolescents with AN is family-based treatment (FBT). However, only 50% of patients receiving FBT fully remit at 12-month follow-up. Consequently, providing an alternative therapy early in the treatment course for those not responding to FBT may enhance overall outcome. This study aims to develop a new treatment - Intensive Family-Focused Treatment (IFT) - to improve outcomes in those adolescents, aged 12-18 years, who do not show an early response to FBT ...

The results of a randomized clinical trial (RCT) comparing Parent-Focused Treatment (PFT) and Family-Based Treatment (FBT) for adolescent Anorexia Nervosa (AN) were published this month in the Journal of the American Academy of Child & Adolescent Psychiatry. As a practitioner of FBT, and as a clinician who is always looking for ways to improve patient outcomes, I read this article with great interest.. Family-Based Treatment (FBT), when applied strictly according to the manual, entails a psychologist or other mental health professional meeting with the family as a whole - the adolescent patient, both parents, and siblings - in a single session. In contrast, Parent-Focused Treatment (PFT) involves the psychologist meeting privately with the parents, while the patients weight, vitals, and mental status are monitored by a nurse.. While FBT and PFT therapy sessions are conducted in different formats, the essence of the treatment - which is implemented by parents in the home - is the same. Both ...
The paper by Dare et al (2001), on a trial of psychological treatments for anorexia nervosa, has two major shortcomings. The investigators planned for a year of weekly sessions of 50 minutes of psychoanalytic therapy; a year of weekly to 3-weekly sessions (60 to 75 minutes) of family therapy; 23 sessions (50 minutes) of cognitive-analytic therapy (CAT), and an unstated frequency of 30 minute sessions for 1 year for the routine treatment group. The patients in the psychoanalytic arm ended up receiving a mean of 24.9 sessions as opposed to 12.9 for the CAT, 13.6 for the family therapy and 10.9 for the routine arm. The differences in the numbers of sessions planned and those actually taking place has not been taken into account in evaluating the results. A summarised by Bergin & Garfield (1994), a large number of different studies show that more sessions are associated with greater improvements. However, the relationship is not linear and begins to taper off after 26 sessions: a figure almost ...
Treatments for teenagers with anorexia that involve the entire family may be more effective than therapies that focus just on the teen, according to a new study.. One year after treatments for the eating disorder were completed, the percentage of patients considered fully recovered was twice as high for the family-based treatment as for the individual treatment, researchers found.. The study was the largest yet comparing the treatment types, researchers said. Their findings were published today (Oct. 4) in the journal Archives of General Psychiatry.. For a really long time, parents have been seen as being an obstacle to treatment, said study researcher James Lock, a professor of psychiatry and behavioral sciences at Stanford University. Family-based treatment and the effectiveness that it has shown in this study suggests that families really are a resource to these kids, and they really should be included in the care of their children.. However, the researchers did not discount treatments ...
Research suggests that family therapy is the most effective treatment for young people with eating disorders. This works on the assumption that parents and families are the solution and never the cause of a childs eating problems. Family therapy aims to discover how resources or strengths in the family can be developed to help young people recover from their eating difficulties. It also helps to consider the impact of the eating disorder on the whole family or the familys caring on the young person.. ...
Anxiety disorders that your child could be experiencing are:. Generalized anxiety disorder. With this common anxiety disorder, children worry excessively about many things, such as school, the health or safety of family members, or the future in general. They may always think of the worst that could happen. Children with generalized anxiety tend to be very hard on themselves and strive for perfection. Children with this disorder are self-conscious, self-doubting, and excessively concerned about meeting other peoples expectations. Along with the worry and dread, kids may have physical symptoms, such as headaches, stomachaches, muscle tension, or tiredness. With generalized anxiety, worries can feel like a burden, making life feel overwhelming or out of control.. Phobias. These are intense fears of specific things or situations that are not inherently dangerous, such as heights, dogs, or flying in an airplane. Phobias usually cause people to avoid the things they fear. These fears come and go ...

Alarmingly, little more than one third of the people suffering from any form of Anxiety disorder are getting treatment (adaa.org). Often the barriers to treatment include not knowing that treatment is available or where to find it, or not wanting to resort to medications. Some people may not realize they have a diagnosable disorder, while other people may know they have it but not want to have to do therapy. If you or someone you know is not getting treatment for whatever reason, some lifestyle choices could help, including a clean, balanced diet, appropriate exercise, adequate sleep, and something thats getting more attention in scientific research, meditation.. In January 2017, Psychiatry Research, a peer reviewed scientific journal, published the results of a randomized study that found significant reductions in stress related hormones in those participants who practiced Mindfulness-Based Stress Reduction (MBSR) as compared to the control group participants who did not (Hoge et al., 2017). ...

This study aims to test an intervention to reduce HIV/STI risk behavior among girls in the juvenile justice system with the triple-threat risk of delinquency, drug abuse, and trauma. This is a competing continuation of the ProTeens study (Preventing Drug Abuse & HIV/AIDS in Delinquent Youths: An Integrated Intervention) which focused on testing a family-centered intervention for treating HIV risk, drug use, and delinquency in boys. The current study will adapt the ProTeens intervention for girls. Girls and their parent(s) will be randomly assigned to either a family-centered intervention condition (INT; n = 100) or to an active comparison condition (n = 100) consisting of group therapy and case management (GCM). The INT condition will consist of individually administered HIV preventive intervention for adolescent girls involved in the juvenile justice system that integrally addresses delinquency, drug abuse, and trauma exposure, along with group-based training and support for parents. The girl ...

OBJECTIVE: Although evidence-based treatments for pediatric obsessive-compulsive disorder (OCD) exist, many youth fail to respond, and interventions tailored to the needs of specific subsets of patients are lacking. This study examines the efficacy of a family intervention module designed for cases of OCD complicated by poor family functioning. METHOD: Participants were 62 youngsters aged 8 to 17 years (mean age = 12.71 years; 57% male; 65% white) with a primary diagnosis of OCD and at least 2 indicators of poor family functioning. They were randomized to receive 12 sessions of individual child cognitive-behavioral therapy (CBT) plus weekly parent psychoeducation and session review (standard treatment [ST]) or the same 12 child sessions plus 6 sessions of family therapy aimed at improving OCD-related emotion regulation and problem solving (positive family interaction therapy [PFIT]). Blinded raters evaluated outcomes and tracked responders to 3-month follow-up. RESULTS: Compared to ST, PFIT ...

Treatment for children with ADHD and an anxiety disorder relies on a combination of approaches geared to each childs specific situation-including educating the child and her family about the condition, encouraging ongoing input from school personnel, initiating behavior therapy including cognitive behavioral techniques, as well as traditional psychotherapy, family therapy, and medication management. Behavior therapies are among the most proven and effective non-medication treatments for anxiety disorders. (The effectiveness of traditional psychotherapy has been less well studied.) Behavior therapies target changing the childs behaviors caused by the anxiety rather than focusing on the childs internal conflicts. Cognitive-behavioral therapy techniques help children restructure their thoughts into a more positive framework so that they can become more assertive and increase their level of positive functioning. For example, a child can learn to identify anxious feelings and thoughts, recognize ...
Multiple Group Family Therapy may work for some, but I think the question needs to be asked what is the goal for the individual family groups. I ve been involved in family support groups for family members who have a child with a mental illness. I did not find the support very helpful or useful. Actually I found it quite depressing. For one, my goal was so much different than the goals of the other members. Many families were fine with keeping their mentally ill child dependent and living at home for as long as the caretakers were alive. This for me is not an acceptable solution. I have no desire for my child to live with me until I die of old age or mental exhaustion. My goal is to help my child create a support network so they can live independently.. Moeder. Posted by: Moeder at August 14, 2004 01:57 AM. August 14, 2004. Can anyone really help a family deal with psychotic outbursts? Eating disorders (ED) usually develop during adolescence, and intervention to stop further weight loss is believed to improve outcome and long-term prognosis. Adolescents with ED who do not receive effective treatment risk poor outcome and even untimely death as adults.. The first aim of this thesis was to investigate long-term mortality and causes of death in a series of female adults with chronic ED. The second aim was to study the one-year outcome of an unselected series of adolescent girls with anorexia nervosa (AN) and other restrictive eating disorders who had been treated within a specialist ED out-patient service focused on nutritional rehabilitation based on family therapy and without planned hospitalization. The third aim was to investigate the possible metabolic and hormonal side effects of olanzapine when used as an adjunct to facilitate nutritional rehabilitation. The fourth aim was to investigate the relationship between polyunsaturated fatty acid (PUFA) status and ...
Most dual-diagnosis programs provide an extensive counseling component. Patients participate in individual and group therapy, with an emphasis on family therapy. Many times, chronic dynamics within the family or core support system can contribute to the patients substance abuse. Skilled family therapists can help patients begin healing those issues. In addition, families learn about addiction and recovery, so they can be supportive of the patient.. Considering the high incidence of co-occurring mental health conditions in people with substance abuse problems, it is encouraging that many treatment programs are incorporating dual-diagnosis models. Patients may have more success staying clean and sober and making permanent changes when the root of the problem is acknowledged and treated alongside the substance abuse issue. ...
Eating disorders among women age 30 and older in the United States are increasing. The medical complications of eating disorders tend to worsen as women age, says Angela Redlak, a clinical psychologist at the Renfrew Center Foundation in Charlotte, where Kelly is treated. The chronic pain of these conditions causes many middle-aged women - unlike teenagers and women in their 20s - to acknowledge they have an eating disorder and seek help. When her longtime boyfriend died in an ATV accident, Kelly turned to drugs, then alcohol. Eating disorders are very deceptive by nature, says Redlak, Renfrews clinical supervisor. To help pull women out of their disorder, Renfrew uses traditional talk therapy, group therapy, couples and family therapy. Looking back, she thinks she would have pursued a career had she not suffered from an eating disorder. The trend seems to be true across the country, says Cynthia Bulik, director of the eating disorders program at University of North Carolina-Chapel Hill.

The literature on social cognition and intergroup relations suggests that gender and age are social concepts which, because they are at the same level of abstraction, may produce interactive effects on person perception judgments. The purpose of this study was to explore gender stereotypes that therapists hold about people who differ in age; examine the differences that therapist characteristics such as sex and age make in therapist perceptions of others; and investigate the clinical sex bias hypothesis that psychotherapists subscribe to one standard of mental health for healthy men and adults but hold another, more stereotypically feminine standard for healthy women. The sample used in these analyses comprises 322 clinical members of the American Association for Marriage and Family Therapy and 311 psychologists listed in the National Register of Health Service Providers in Psychology. Subjects completed Brovermans Sex-Role Stereotype Questionnaire and therapists were asked to rate
An Introduction to Pediatric Psychopharmacology by Sandra DeJong, MD, Associate Training Director, Child Psychiatry, Cambridge Hospital, Harvard Medical School October 2007 The information below is intended as general guidelines to the pediatrician. In no way is this information intended to guide specific practice with specific patients. DO NO HARM - As with all medications, psychotropic medications work best when the diagnosis is correct. In certain cases, they can make thoughts, feelings and behaviors worse, or cause physical problems. When in doubt regarding a diagnosis or treatment, an evaluation by a qualified mental health professional is advised. MULTIMODAL TREATMENT - Medications alone are rarely the optimal treatment for a psychiatric problem. Other treatment modalities such as individual psychotherapy, family therapy, individual tutoring, school accommodations, group therapy, community supports such as social clubs, etc., are often warranted. FDA APPROVAL - Until recently, the FDA did ...
The Essex Edge of Care Social Impact Bond (SIB) was contracted by Essex County Council (ECC) to provide therapeutic support and improve outcomes for adolescents at risk of going into care.. Eight investors provided a total commitment of £3.1m to fund for five and a half years an intervention which would support 11-17 year olds who are at risk of entering care or custody. The financial return for these investors is linked to the success of the programme in helping children remain out of care and safely at home with their families.. The national childrens charity Action for Children was selected to deliver the funded intervention, Multi-Systemic Therapy (MST), an intensive evidence-based family therapy. The service aims to work with 380 young people over five and a half years. We hope to divert approximately 100 young people from entering care, with investor returns determined by the reduction in days spent in care amongst the service users compared to a historical baseline. Essex County Council ...

Hypnosis in dentistry is not widely used today but in the hands of a trained and experienced Dentist or Hypnotherapist its advantages can be quite profound.. Children usually make excellent hypnotic subjects.. They have such wonderful imaginations that you can easily induce a hypnotic state by persuading them to imagine taking off in a spaceship or taking part in their favourite television show by allowing their eyes to be the television sets. A large percentage of childhood and adolescent problems stem from unresolved emotional issues. These children are often anxiety ridden with a lack of confidence and they become totally dependent on one or both of the parents. They sometimes exhibit various problems such as nail biting, stuttering, obsessive compulsive disorders, bed wetting, behavioural problems, nightmares and even bulimia. Hypnosis can resolve some of these symptoms but in others individual and family therapy is necessary.. From a dental aspect the thumb sucking, grinding, examination ...

Every night before bed, I tiptoe into each of my boys rooms. Mikey is usually scrunched up in the fetal position with everything thrown off his toddler bed except his beloved blanky. His hair is matted with sweat and mouth is sucking adorably on the binky I rationally know he is too old to have. He still looks like my sweet baby boy (I know my attempts to keep him as such border on pathological and will be a source of family therapy topics later on in his life). Then I make my way to Wills room. There he is, lying on his back with his hands behind his head (just like his Daddy), sleeping in his full size bed, Lightning McQueen sheets, and favorite stuffed animal of the week. Books are strewn about him, as he is proudly learning to sound out letters and words (proud mommy face), and undoubtedly there are a few action figures in his bed. His pooh blanky, which until recently he clung to ferociously, is nowhere to be seen most nights. Its most likely shoved under blankets or under his bed. ...
