Maudsley family therapy also known as family-based treatment or Maudsley approach, is a family therapy for the treatment of anorexia nervosa devised by Christopher Dare and colleagues at the Maudsley Hospital in London. The study showed family therapy to be the more effective approach in patients under 18 and within 3 years of the onset of their illness. OBJECTIVE: Previous studies have found that family-focused treatment is an effective adjunct to pharmacotherapy in stabilizing symptoms in adult bipolar disorder. The authors examined whether pharmacotherapy and family-focused treatment for adolescents with bipolar disorder was more effective than pharmacotherapy and brief psychoeducation (enhanced care) in decreasing time to recovery from a mood episode, increasing time to recurrence, and reducing symptom severity over 2 years. Expressed emotion (EE) has been associated with poor outcomes in anorexia nervosa (AN); however, whether changes in EE predict superior treatment outcomes is unknown. The current study examined whether decreases in EE during an open trial of a novel family-based treatment for AN predicted symptoms at end of treatment. Forty-seven adolescents (12-18 years of age) with AN or sub-threshold AN and their parents (mothers: n = 47, fathers: n = 39) participated in 6 months of family treatment. Measures of AN symptomatology (Eating Disorder Examination completed by adolescent and end of treatment recovery status) and parental EE (Family Questionnaire completed by parents which measures two facets of EE: critical communication [CC] and emotional over-involvement [EOI]) were collected at baseline and end of treatment. The investigators planned for a year of weekly sessions of 50 minutes of psychoanalytic therapy; a year of weekly to 3-weekly sessions (60 to 75 minutes) of family therapy; 23 sessions (50 minutes) of cognitive-analytic therapy (CAT), and an unstated frequency of 30 minute sessions for 1 year for the routine treatment group. This is a competing continuation of the ProTeens study (Preventing Drug Abuse & HIV/AIDS in Delinquent Youths: An Integrated Intervention) which focused on testing a family-centered intervention for treating HIV risk, drug use, and delinquency in boys. I ve been involved in family support groups for family members who have a child with a mental illness. I did not find the support very helpful or useful. Actually I found it quite depressing. For one, my goal was so much different than the goals of the other members. Many families were fine with keeping their mentally ill child dependent and living at home for as long as the caretakers were alive. This for me is not an acceptable solution.
