Extravasations (EV) of anthracyclines (AC) are a rare but potentially seriously debilitating complication of chemotherapies (CT) that can lead to severe skin lesions with the need for surgical debridement, a functional disability in the short and longer term, and a delay in the CT programme following on from EV management.. Savene is the only antidote authorised for the treatment of AC EVs. The treatment involves a peripheral intravenous infusion lasting 1 to 2 hours, to be started as soon as possible and within 6 hours following the EV, to be repeated 24 hours and 48 hours after the 1st infusion. The treatments effectiveness was first demonstrated during clinical testing on peripheral EVs, confirmed by biopsy, with a rate of 98% of being able to dispense with the need for surgery due to ulceration or tissue necrosis, and the initial CT programme being maintained in 71% of patients. These efficacy data were recently confirmed in Belgium, in clinical practice, with 41 patients showing an AC EV, ...
Acute type A aortic dissections starting at the arch pose a challenge for cardiac surgeons. Open surgical repair requires deep hypothermic circulatory arrest for arch reconstruction and is associated with significant morbidity and mortality. Hybrid aortic repair techniques, with open arch debranching and thoracic endovascular aortic repair, have been employed in high-risk cases and challenging aortic pathology. Herein, we present a case of a 33-year-old African American male with a history of open thoracoabdominal aortic reconstruction and femoral-femoral artery bypass for a type B dissection who subsequently presented with new-onset chest pain and was found to have a retrograde type A dissection of a bovine arch with multiple dissection flaps and possible contrast extravasation on chest computed tomography ...
Vesicants Vesicant drugs may cause severe tissue injury, usually resulting in necrosis; consequently, vesicant extravasation is a medical emergency requiring prompt situationally appropriate action. Vesicant extravasation injuries may be subtle with a delayed onset of morbidity, or immediately apparent with a rapid onset of symptoms. The severity of lesions, the time over which they develop, and the likelihood of spontaneous healing without intervention, also vary among vesicant drugs. DNA-binding agents (eg, anthracyclines, anthracenediones, mitomycin, dactinomycin) tend to produce more severe and persistent lesions than drugs that do not bind to DNA (eg, Vinca alkaloids, taxanes). Over a period of days to weeks, erythema, swelling, induration, and pain may increase and progress to brawny discoloration, blistering, or dry desquamation. Lesions may ulcerate and form eschars. Ulcers are characteristically painful with erythematous borders and necrotic bases. Spontaneous reepithelialization ...

RESULTS There was no angiographic evidence of vasculitis. Histologic evidence of vasculitis was absent or mild in 92% of cases and histologic evidence of perivascular inflammation was absent or mild in 73% of cases. The frequency of histologic changes associated with vasculitis, perivascular inflammation, and vessel necrosis varied with the time interval between embolization and AVM resection. Intracranial hemorrhage, as a direct complication of silk use, occurred in one patient. Another patient had subarachnoid hemorrhage 24 hours after embolization, caused by rupture of a posteroinferior cerebellar artery aneurysm. Intraventricular high-density material appeared on routine postembolization CT scans in two other patients who had intraventricular AVM extension. This high-density material was thought to be contrast extravasation from intrinsically leaky AVM nidus vessels and not frank hemorrhage. ...
Prostate cancer is the most common form of cancer in men. The manifestation of distant metastasis is the major cause of cancer-related deaths. The metastatic dissemination is an inefficient, complex and dynamic process, involving tumour cells detaching from the primary tumour site, entering the bloodstream, extravasation at distant locations and establishing secondary tumours. The reciprocal regulation of miRNA and their targets has been established as crucial regulators of the balance between epithelial and mesenchymal differentiation states, as well as cancer stem cell regulation. We focus on the miRNA regulation of the transient processes leading to metastases as the reversibility of this type of regulation present a possible interference option. Different miRNAs have evolved to coordinate the regulation of hundreds of genes involved in coordinated cellular functions. When these functions are disrupted in e.g. cancer progression, they provide a strategy to target systems rather than ...

All patients with suspect splenic injury should be assessed, resuscitated, and treated according to ATLS principles. Patients who are haemodynamically unstable* or with a grade 5 injury (a shattered spleen or major hilar vascular injury) need urgent laparotomy.. *If there is evidence of active extravasation of the contrast during the arterial phase of the CT scan, the patient should undergo embolisation (if locally available) or laparotomy with splenectomy. Haemodynamically stable patients with grade 1 - 3 injuries without active extravasation can be treated conservatively. They should be resuscitated as appropriate, admitted to a high dependency area for observation, and have serial abdominal examinations for any evidence of deterioration.. With any evidence of increasing tenderness or peritonitis, there should be a low threshold for re-imaging and / or laparotomy (as associated injuries such as small bowel injuries are easily missed on initial CT imaging). All patients who are treated ...
By EDWARD F. JACKSON, PhD. One key advantage of MR is the wide range of intrinsic tissue properties that can be assessed by the appropriate choice of parameters defining the associated measurement technique.. In neuroimaging applications, for example, routine MR imaging provides a qualitative means of assessing the breakdown of the blood brain barrier as reflected by the extent of gadolinium contrast agent extravasation (T1-weighted sequences), the extent of vasogenic edema (T2-weighted and T2-weighted fluid attenuated inversion recovery sequences), the presence of blood products such as hemosiderin, methemoglobin (T1-weighted and T2* susceptibility-weighted images), and the ability to assess brain atrophy and regional white/gray matter abnormalities (using proton density-weighted and/or magnetization prepared gradient-echo sequences).. Neurovascular anatomy can be evaluated with or without the injection of exogenous contrast agents (using time-of-flight and/or phase-contrast MR angiography ...
Bleeding from an ECF is not a commonly described complication. When present, it tends to respond to noninvasive treatment such as applying local pressure and adequate wound care. The present case prompted the performance of a CT angiography to search for potential culprit vessel(s). Although no active bleeding was seen at the arterial phase, venous phase CT images showed active extravasation in the external blood clot overlying the skin, yet without evident source vessel. The right gastroepiploic artery was the vessel seen most closely located to the bleed, but appeared normal and was separated from the contrast extravasation by a clean, non-infiltrated subcutaneous fat plane and by the overlying skin ...

OBJECTIVE: Extravasation is a potential complication associated with intravenous therapy administration. Inadvertent leakage of medications with vesicant properties can cause severe tissue necrosis, which can lead to devastating long-term consequences. Recognizing potential agents is an essential step in mitigating the risk of extravasation.. DATA SOURCE: A literature search was carried out using PubMed with the following key words: extravasation, soft tissue injury, phlebitis, and infiltration, from January 1961 through January 2014.. STUDY SELECTION AND DATA EXTRACTION: The publications were screened manually and reviewed to identify reports for medications that included synonyms of the International Nonproprietary Name, while excluding antineoplastic agents, radiographic contrast material, investigational or ...
Results Of 323 patients with ICH, 172 (53%) survived after 1 year, 127 (39%) after 5 years and 57 (18%) after 13 years. Mortality of the 172, 1 year survivors (mean age 67.7 years at ICH) persistently exceeded expected mortality; 13 years post ictus survival was only 34% compared with 61% in the general population. Of 115 deaths among the 172, 1 year survivors, 36% were from cerebrovascular disease and 19% from ischaemic heart disease. Independent risk factors for death among 1 year survivors were age (HR 1.08 per year; 95% CI 1.06 to 1.10; p,0.001), diabetes mellitus at baseline (HR 2.10; 95% CI 1.18 to 3.74; p=0.012) and anticoagulant therapy (HR 1.99; 95% CI 1.12 to 3.53; p=0.018) at ICH onset.. ...

A system for injection of a fluid into a body includes: a source of a first fluid, a source of a second fluid; the first fluid being less toxic than the second fluid; at least one pressurizing system in operative connection with the source of the first fluid and with the source of the second fluid; at least one controller in operative connection with the pressurizing system; and at least a first sensor in communicative connection with the controller. The first sensor is adapted to transmit a signal of a detected change in fluid level in tissue indicative of extravasation to the controller. The controller is adapted to cause injection of the first fluid into the body to determine if extravasation of the first fluid occurs using the sensor in an administration phase before injection of the second fluid.

Figure 3 Ankle OA intraoperatively during fusion procedure.. Intra-articular corticosteroids remain widely used for symptomatic treatment of peripheral joint osteoarthritis, although the duration of the effect can be highly variable depending on many factors, such as type of joint involved and the use of image guidance or not [7-11]. Kevin et al suggest experienced surgeons may be able to place intra-articular injections without fluoroscopy in a normal posterior subtalar joint with a 97% accuracy rate [1]. Fluoroscopy may not be necessary for injections used solely for therapeutic purposes. However, if the injection is intended for diagnostic purposes and surgical decision making for potential arthrodesis or if the joint is abnormal, they recommend fluoroscopy to ensure accurate placement without extension or extravasations into nearby structures that also might be potential sources of pain. Concerns for surrounding soft tissues may warrant use of fluoroscopy in cases of arthrosis and indwelling ...

article{d69173db-9180-4395-8614-5dd71b3a01f2, abstract = {The aim of the present study was to characterize the airway effects of U46619, a stable thromboxane A2 mimetic, instilled into the trachea of guinea pigs in vivo, and to investigate the role of different mediators in these effects. The airflow obstruction was evaluated by measurement of airway insufflation pressure (P1) and plasma extravasation by quantification of Evans Blue dye (EBD) in airways. U46619, given as a single dose to each animal (1 pmol-10 nmol), caused a dose-dependent increase in P1 and extravasation of EBD. The threshold dose required to induce an increase in P1 was 30 times lower than the threshold dose necessary to evoke EBD extravasation. The role of inflammatory mediators was studied when 10 pmol (inducing only the increase in P1) or 10 nmol (inducing the increase in both P1 and EBD extravasation) of U46619 was administered. The effects of both doses of U46619 were abolished by ICI192,605, an antagonist of prostanoid ...
In this study, we demonstrated that (1) strict radiological criteria for the identification and scoring of the MDCTA spot sign can be applied reliably in patients with primary ICH, and (2) the spot sign has good accuracy for the prediction of in-hospital mortality (71%) and poor outcome among survivors at 3-month follow-up (67%) in this patient population. For the prediction of in-hospital mortality, our accuracy results are similar to those reported by Becker et al (74%)3 and Kim et al (73%),5 and higher than reported by Wada et al (64%)4 as well as the previously reported subset of our patient population that used a less strict spot sign definition (56%).6. Spot sign criteria similar to ours have been proposed by Thompson et al10 with the salient differences being that (1) we use an absolute attenuation cut-off for the definition of a spot sign (≥120 HU), compared to a relative attenuation cut-off of at least twice the mean background hematoma attenuation; and (2) we do not impose a minimum ...

N2 - Interleukin 1 β (IL-1 β) is a potent mediator of neutrophil accumulation. Antidromic stimulation of the rat saphenous nerve leads to neurogenic oedema formation mediated by endogenous tachykinins. Here, we have investigated links between IL-1 β and the tachykinin 1 (NK1) receptors in microvascular events in rat skin. Saphenous nerve-induced plasma extravasation was not modulated by skin pretreatment with IL-1 β (3 pmol/site intradermally). In addition, the long-lasting antidromic electrical stimulation did not induce significant neutrophil accumulation in naive rat skin. By comparison, the effect of IL-1 β-induced neutrophil accumulation was ...

Plasma extravasation is one of the characteristic symptoms of an inflammatory response and of neurogenic inflammation, a phenomenon which occurs following nerve damage, and can lead to significant changes in the control of the cutaneous circulation (1). Evans Blue binds with high affinity to serum albumin, and the complex has been often used to quantitatively assess the extent of extravasation that accompanies peripheral and neurogenic inflammation (2,3). Here we present a protocol for evaluating Evans Blue extravasation in rodents with the aid of local application of capsaicin to initiate a neurogenic response exciting the local nociceptors, causing pain, axon reflex vasodilatation and plasma extravasation due to local release of inflammatory mediators. The method is inexpensive, precise and efficient, and can be adapted to assess extravasation in a range of experimental pathological conditions such as parotitis (4) or craniofacial muscle inflammation (5), and their dependence on different ...
Computed tomography (CT) is the most common imaging method used to investigate stroke. It demonstrates haemorrhage accurately immediately after the stroke1 but as the haematoma is broken down, the characteristic hyperdensity on CT disappears, becoming indistinguishable from infarction. This has been reported to occur by 14 days for moderately sized lesions,2 and possibly sooner for smaller haemorrhages.3. Primary intracerebral haemorrhage is thought to cause more severe stroke symptoms than an infarct of the same size,4,5 and so primary intracerebral haemorrhage is considered to be a less frequent cause of mild than severe stroke. However, it is possible that this apparently low incidence is an artefact caused by the misclassification of mild stroke resulting from primary intracerebral haemorrhage as ischaemic stroke because of delay in CT scanning in epidemiological studies.6 The actual proportion of primary intracerebral haemorrhages among mild strokes is unknown. Thus there is no information ...
The findings of the present study indicate that the leakage sign is useful for predicting growth of a primary ICH, with good reproducibility, a clear definition, the convenience of waiting only 5 minutes, a restrictive ROI of a 1-cm diameter, and detection of a 10% increase in HU. Furthermore, the sensitivity (93.3%; 95% confidence interval, 0.757-0.988) and specificity (88.8%; 95% confidence interval, 0.815-0.912) for hematoma expansion were high, and the leakage sign can predict poor outcomes for all patients with a primary ICH and specifically those with an ICH in the putamen.. The spot sign is currently considered a useful radiological sign on CTA showing extravasation of contrast media or a bleeding artery in a hematoma and also as a prognostic predictor.5,11 However, the spot sign reportedly indicates both active bleeding13 and a striate artery.14. A modified spot sign has been described recently, in which delayed-phase CT was performed 2 to 5 minutes after the first CT.6,15,16 The ...
A non-enhanced computed tomography (CT) based finding, termed the satellite sign, was reported to be a novel predictor for poor outcome in spontaneous ICH. However, it is still unclear whether the presence of the satellite sign is related to hematoma expansion.. Initial computed tomography angiography (CTA) was conducted within 6h after ictus. Satellite sign on non-enhanced CT and spot sign on CTA were detected by two independent reviewers. The sensitivity and specificity of both satellite sign and spot sign were calculated. Receiver-operator analysis was conducted to evaluate their predictive accuracy for hematoma expansion.. This study included 153 patients. Satellite sign was detected in 58 (37.91%) patients and spot sign was detected in 38 (24.84%) patients. Among 37 patients with hematoma expansion, 22 (59.46%) had satellite sign and 23 (62.16%) had spot sign. The sensitivity and specificity of satellite sign for prediction of hematoma expansion were 59.46% and 68.97%, respectively. The ...

Interleukin (IL)-15 is a proinflammatory cytokine whose described functions largely overlap with those of IL-2. The latter is attributable in large part to its binding of the heterotrimeric receptor that contains the β and γ chains of the IL-2R in combination with an unique IL- 151α chain. However, unlike IL-2, IL-15 and its receptor have a wide tissue and cell type distribution, including endothelial cells. Here, we examine the effect of IL-15 on hyaluronan expression by endothelial cells, and investigate its role in vivo in promoting the ...

Free fluid is associated with numerous types of injury, which may be detected on ultrasound with varying success. These include rupture of the liver, spleen, kidney, pancreas or bowel (Fig. ULTRASOUND IMAGES BELOW). A notable limitation of sonography in the trauma situation is in detecting free fluid in the pelvis, as the bladder is frequently empty or underfilled, and the use of the Trendelenburg position, if possible, helps to reduce the number of false-negative results in this respect by allowing any free fluid to collect Ultrasound is more successful in detecting free fluid than in detecting organ injury directly.6,7 One study reported a 98% sensitivity for detection of fluid, but only 41% of organ injuries could be demonstrated.8 However, most of the published studies have concentrated only on the presence or ...

OBJECTIVE: Early hematoma expansion is a known cause of morbidity and mortality in patients with intracerebral hemorrhage (ICH). The goal of this study was to identify clinical predictors of ICH growth in the acute stage.. MATERIALS AND METHODS: We studied 201 patients with acute (,6h) deep ganglionic ICH. Patients underwent CT scan at baseline and hematoma expansion (,33% or ,12.5ml increase) was determined on the second scan performed within 24h. Fourteen clinical and neuroimaging variables (age, gender, GCS at admission, hypertension, diabetes mellitus, kidney disease, stroke, hemorrhagic, antiplatelet use, anticoagulant use, hematoma density heterogeneity, hematoma shape irregularity, hematoma volume and presence of IVH) were registered. Additionally, blood pressure was registered at initial systolic BP (i-SBP) and systolic BP 1.5h after admission (1.5h-SBP). The discriminant value of the hematoma volume and 1.5h-SBP for hematoma expansion were determined by the receiver operating ...
