A team of University of Nairobi (UoN) Plastic Surgeons successfully re-implanted a completely severed hand at the UoN teaching hospital on Saturday 27th January 2018. The operation was performed on a 17 year old boy whose right hand was completely detached when a chaff cutter accidentally came on while he was in the process of cleaning it.. The young man was first seen at Kiambu County Hospital from where he was referred to KNH where the 7 hour operation was performed. The UoN surgeons who participated in the surgery were; Prof SO Khainga and Dr FW Nangole, assisted by budding Plastic Surgeons Dr Alex Okello and Dr Kenneth Aluora. In the photos accompanying this narration, the university surgeons are seen being congratulated by the KNH CEO Mrs Lily Koros and the Director of Clinical Services Dr BM Githae.. ...
The World Health Organization held a two day training to equip trainees with technical skills for the application of the Odon device. This was done in preparation for the roll-out of a multicountry, multisite study to evaluate the feasibility and safety of the Odon device for assisted vaginal delivery. In Kenya the study will be done at KNH.. Among the trainers was Professor Zahida Qureshi, one of the local PIs. Dr. George Gwako, another local PI and Mark Sigei, the Data Manager in the study were trained.. ...
After further testing, Odón was introduced to the chief of obstetrics at a hospital in Buenos Aires who saw the benefit of the method and arranged to have the device tested more thoroughly at an Iowa laboratory that has simulators designed to model delivery methods more realistically. Safety testing had been performed on 20 women in Argentina, all of whom had previously given birth and were experiencing uncomplicated pregnancies, including a woman who was able to deliver a baby weighing 9 pounds (4.1 kg) with only two pushes. Further testing will be conducted on more than 250 women in China, India and South Africa, with a mix of pregnant women experiencing normal and complicated labor.[1]. Becton Dickinson has agreed to manufacture and distribute the unit, and estimates that the Odón Device could be constructed for $50 per unit, and it is expected that it could be used by midwives as well as obstetricians who would need minimal training to use the device effectively.[1] The World Health ...
There are some side effects to be aware of.. An epidural may make your legs feel heavy, depending on the local anaesthetic used.. Your blood pressure can drop (hypotension), but this is rare because the fluid given through the drip in your arm helps to maintain good blood pressure.. Epidurals can prolong the second stage of labour. If you can no longer feel your contractions, the midwife will have to tell you when to push. This means that forceps or a ventouse may be needed to help deliver the baby (instrumental delivery).. When you have an epidural, your midwife or doctor will wait longer for the babys head to come down (before you start pushing), as long as the baby is showing no signs of distress. This reduces the chance youll need an instrumental delivery. Sometimes less anaesthetic is given towards the end, so the effect wears off and you can feel to push the baby out naturally.. You may find it difficult to pee as a result of the epidural. If so, a small tube called a catheter may be put ...

Our babies are primed to take advantage of this alert time with the inbuilt reflexes mentioned above to lead them to the breast and to latch, without any help from mum. A great example of a newborn crawling to the breast and self attaching can be seen here. So why doesnt this always happen? Lots of reasons. In NI, birth often doesnt go as we planned. In the vast majority of births, some kind of intervention has happened, maybe mum has had pain medication which passes over the placenta, maybe there has been an instrumental delivery or a c-section. Interventions adjust the normal pattern after birth. Babies are very immature and the pain medications can make them a little sleepy so that the normal reflexes dont show in the same way. It may make them too sleepy to latch, as in the example of Jack and Emily, or alternatively it may leave their movements and their sucking uncoordinated so that although they latch, they dont remove milk well. If it was an instrumental delivery they may be bruised ...

Babies cannot always be delivered through the birth canal without assistance from medical staff and sometimes medical instruments or tools will be needed to help the baby to come out. Forceps or ventouse can be used to achieve delivery in the second stage of labour, when the baby is ready to be born and the cervix (vaginal opening) is fully dilated.. An instrumental delivery may be needed after a very long labour or when the mother is too exhausted to be able to push her baby out during contractions in the second stage. A spinal epidural can also result in the mother not being able to "push" when required.. Sometimes the baby will have been identified as becoming distressed (with an abnormal heart rate), or the head is in an unusual position. When any of these events arise delivery may need to be achieved quickly to ensure the safety of mother and child. A caesarean section may not always be possible or the safest option at this stage and an instrumental delivery may be needed.. Which instrument ...
Forceps delivery is one of the many assisted delivery options that you may have heard of. The procedure is used in vaginal deliveries when the baby is in the head-down position,

What was the birth like? Did you have any complications? I had 2 instrumental deliveries with my first 2 and so the hospital policy is not to allow a homebirth but I was determined and with all the benefits of a homebirth and being comfortable in my own environment pushed my daughter out myself in our birthing pool. I am so proud of myself and will always have such wonderful memories of her birth. She was back to back so came out face up like my 2nd son and I am convinced that if I had been in hospital she would have been another forceps delivery ...

Vacuum extraction deliveries can lead to serious birth injuries for infants. Here, learn more about these deliveries and the risks they present.
A vacuum extraction birth injury lawyer at Rusk, Wadlin, Heppner & Martuscello can help malpractice victims who experience injuries caused by extractions.
A woman was admitted to the hospital for elective induction of labor. She gained a significant amount of weight while pregnant. During delivery, her family practitioner (FP) determined that vacuum extraction was needed but he was not qualified to use the device. An in-house ObGyn was called in to use the vacuum extractor. The FP delivered the babys shoulders. The infant was born with a floppy right arm and later diagnosed with rupture injuries to the C-5 and C-6 vertebrae and permanent brachial plexus damage. She has limited range of motion in her right arm and shoulder.. PARENTS CLAIM:. The FP was relatively inexperienced in labor and delivery. He should not have ordered vacuum extraction because of risk factors including the mothers small stature, her significant weight gain during pregnancy, the use of epidural anesthesia, and induction of labor. Using vacuum extraction increases the risk of shoulder dystocia.. The FP improperly applied excessive downward traction on the fetus causing the ...

The rates of induction, cesarean section, operative delivery, and episiotomy in this low-risk population were 23.9 percent, 10.1 percent, 15.2 percent, and 19.6 percent, respectively, and 52.0 percent of deliveries included at least one of them. Unit size was unrelated to any intervention except operative delivery (adjusted odds ratio [aOR] = 1.47 (95% CI, 1.10-1.96) for units with ,3,000 deliveries per year vs units with ,1,000). The rate of every intervention was higher in private units, and the aOR for any intervention was 1.82 (95% CI, 1.59-2.08). After adjustment for maternal characteristics and facility size and status, significant variations in the use of interventions remained between units, especially for episiotomies. Results for level of care were similar to those for unit size. ...

Aneurysms arising from peripheral segments of cerebral arteries may be congenital, but they are often reported as traumatic, mycotic, or associated with an arteriovenous malformation (2, 9). Traumatic false aneurysms usually involve the distal cerebral circulation, and fewer than 10% are located in the posterior circulation (3). Six cases of traumatic aneurysms involving the SCA have been reported (3, 10-13), including one case of distal aneurysm of the SCA in a 5-day-old infant who presented with SAH after a traumatic birth that necessitated forceps delivery (12). The SCA may be particularly vulnerable to injury caused by stretching of the edge of the tentorium (7). In case 2, a history of precipitous birth with forceps delivery was the only traumatic event reported by the patient. The presence of cystic lesions on the right side of the midbrain with no clinical manifestation led us to speculate that an injury of the right SCA may have occurred at the time of birth with the ensuing development ...

The obstetrical anesthesia database will be queried for all forceps deliveries between the dates of January 2004-October 2005. To minimize the influence of different anesthetic and obstetric care providers, the control group consisted of parturients who has spontaneous vaginal deliveries (SVD) in the same 24 hour period who were case-matched for gravidity and parity. Parturients with twin deliveries and fetal demise were not selected for either group.. The database will be queried for the following: maternal age, parity, gestational age, type of analgesia, changes in epidural infusion rate and/or concentration, and supplemental bolus doses of local anesthesia. The data will be stripped of identification by the database manager and entered into a secured computer which is password protected and maintained in the Department of Anesthesiology. The primary endpoint is to evaluate if decreasing the epidural infusion rate was associated with a forceps delivery. ...
100 newborns will be assessed for somatic dysfunction of the head, cervical, lumbar, and sacral areas. These findings will be compared to the findings of a standard newborn physical exam and the newborn and maternal history. These records will be reviewed for maternal age, parity, gravity, anesthesia during labor, and estimated gestational age. Birth history and initial newborn assessments will be reviewed for labor augmentation, duration of labor, length of second stage of labor, presence of instrumental delivery(vacuum extraction, forceps delivery, etc.), multiple birth, and presentation at delivery (breech, transverse, or compound presentation). The purpose of the study is to quantify the presence of somatic dysfunction in the newborn. We will assess any relationships between the identified somatic dysfunctions and factors present in the mothers and newborns medical history ...
Birth trauma or injuries to a baby related to the birthing process can be some of the most catastrophic injuries possible. Such injures are often lifelong with consequences for the child and parents lasting decades. Birth injuries and birth trauma can affect the child both physically and cognitively. Matters involving birth trauma and birth injuries are some of the most perplexing and complex cases in the legal profession and require experienced and well-funded counsel to investigate and litigate them properly.. Birth trauma cases range from injuries to the brachial plexus nerve complex, often occasioned by shoulder dystocia events, to skull injuries and malformations. Birth injuries usually center on injuries to the brain of the child due to lack of oxygen to the fetus during the labor and delivery process. In many instances, birth trauma and birth injuries are completely avoidable with careful monitoring of the mother and baby during the labor process. Contact Bounds Law Group today at (407) ...
All American forceps are derived from French forceps (long forceps) or English forceps (short forceps). Short forceps are applied on the fetal head already descended significantly in the maternal pelvis (i.e., proximal to the vagina). Long forceps are able to reach a fetal head still in the middle or even in the upper part of the maternal pelvis. At present practice, it is uncommon to use forceps to access a fetal head in the upper pelvis. So, short forceps are preferred in the UK and USA. Long forceps are still in use elsewhere. Simpson forceps (1848) are the most commonly used among the types of forceps and has an elongated cephalic curve. These are used when there is substantial molding, that is, temporary elongation of the fetal head as it moves through the birth canal.[4] Elliot forceps (1860) are similar to Simpson forceps but with an adjustable pin in the end of the handles which can be drawn out as a means of regulating the lateral pressure on the handles when the instrument is ...

Forceps are tools used during some deliveries that, if used incorrectly, could cause serious complications resulting in serious birth injuries and or even death.
Background: Trauma to the perineum is extremely common during childbirth and the intensity of perineal pain has been reported to vary from being mild to very severe in nature. Aim: To evaluate the effectiveness of two localised cooling treatments (ice pack and cooling gel pad) compared with a no localised treatment group at relieving perineal pain. Method: A randomised controlled trial conducted in a large maternity unit in the north of England and followed up in womens own homes in which 450 women who had either undergone a normal or instrumental delivery that required suturing of an episiotomy or second-degree tear were randomly assigned to three treatment groups. Results: The response rate was 316 out of 450(71%). Perineal pain was most severe when sitting compared to lying down or walking and there was a significant difference between the three groups in estimates of overall pain when sitting on day four (Kruskal-Wallis test,df=2 p=0.01). Estimates of overall pain were lower in the gel pad ...
