This study presents a novel intelligent Fuzzy Genetic Differential Evolutionary model for the optimization of a fuzzy expert system applied to heart disease prediction in order to reduce the risk of heart disease. To this end, a fuzzy expert system has been proposed for the prediction of heart disease. The proposed model can be used as a tool to assist physicians. In order to: (1) tune the parameters of the membership function of the fuzzy expert system, (2) improve its performance and (3) increase its accuracy, the Genetic Algorithm combined with the Differential Evolution Algorithm has been applied to the fuzzy expert system. The proposed hybrid models, Fuzzy-GA, Fuzzy-DE, and Fuzzy-GA-DE were evaluated using ROC curve analysis and 10- fold cross-validation methods. Hospital-acquired infections are responsible for an increase in patient mortality and costs. Their detection is essential to permit better infection control. We developed an expert system specifically to detect infections in pediatric patients. The expert system is implemented at LDS Hospital that has a level three newborn intensive care unit and well baby units. We describe how the knowledge base of the expert system was developed, implemented, and validated in a retrospective study. The results of the system were compared to manual reviewer results. The expert system had a sensitivity of 84.5% and specificity of 92.8% in detecting hospital-acquired infections when compared to a physician reviewer. Here, the term design denotes the shaping or modeling of the prostheses and not their functional design. The challenge of the task at hand lies in designing prostheses that fit perfectly to the anatomy of the patient, and in many cases have to support additional functionality. Hence, each prosthesis has to be unique. The present invention tests computer systems for defects that are able to cause performance and functional problems. An agent application is first installed on a users system. The agent application then retrieves problem data from the expert system library pertinent to the users operating environment. The agent application tests the users system for each problem description within a downloaded data structure from the expert system library. If a problem is discovered, a script attempts to remediate the problem and/or notifies the user, describing the issue. If an unknown problem arises, a feedback mechanism expands the expert system library by sending a notification that a problem was discovered. The feedback is then utilized to increase the total knowledge in the expert system library. Then when another user with a similar or the same setup is tested, the agent application will recognize the issue and will take the appropriate remedial actions.

The results of this study indicated that the VITEK 2 system and AES could be used on the majority of clinical isolates encountered in a typical university-based laboratory for identification and antimicrobial susceptibility testing with little or no input from laboratory staff concerning the interpretation of results. In attempts to circumvent the laws controlling drugs of abuse, new chemical structures are very frequently introduced on the black market. They are obtained by slightly modifying the controlled molecular structures by adding or changing substituents at various positions on the banned molecules. As a result, no substance similar to those forming a prohibited class may be used nowadays, even if it has not been specifically listed. Our study contributes new findings to the understanding of how a CDSS is used in the initial management of a common symptom. Despite use of the OMA CDSS in 86 % of consultations for which it was available, there was little evidence of impact on medication prescribing or on investigation choice. Through qualitative data analysis we gained an in-depth understanding of the reasons for this discrepancy. We identified problems in entering patient data into the CDSS. These included difficulties in classification of symptoms and risk factors, and in the incorporation of all available clinical information emerging during the consultation. In the majority of observed cases, the CDSS was used after the patient had left the room. Structural and practical barriers to the use of the CDSS included the availability of investigations and the prescribing competencies of the clinicians. The diagnosis of many diseases can be often formulated as a decision problem; uncertainty affects these problems so that many computerized Diagnostic Decision Support Systems (in the following, DDSSs) have been developed to aid the physician in interpreting clinical data and thus to improve the quality of the whole process. Fuzzy logic, a well established attempt at the formalization and mechanization of human capabilities in reasoning and deciding with noisy information, can be profitably used. Recently, we informally proposed a general methodology to automatically build DDSSs on the top of fuzzy knowledge extracted from data. We carefully refine and formalize our methodology that includes six stages, where the first three stages work with crisp rules, whereas the last three ones are employed on fuzzy models. In this paper, we propose a novel medical knowledge acquisition approach that leverages routinely generated electronic medical records (EMRs) as an alternate source for CBR-compliant cases. We present a methodology to autonomously transform XML-based EMR to specialized CBR-compliant cases for CBR-driven medical diagnostic systems.
