Purpose: Over 90,000 osteoarthritis (OA) related TKR surgeries take place across the UK annually, with patients undergoing regular post-surgery physiotherapy that is reliant on home-based exercise rehabilitation and driven by personalised self-management. With poor patient adherence that is difficult to ascertain, clinicians who are challenged to optimise patient outcomes are unable to determine whether improvements (or lack of) can be attributed to an exercise intervention or (non) adherence. Background-Exercise training is an effective treatment for important atrial fibrillation (AF) comorbidities. However, a high level of endurance exercise is associated with an increased AF prevalence. We assessed the effects of aerobic interval training (AIT) on time in AF, AF symptoms, cardiovascular health, and quality of life (QoL) in AF patients. Methods and Results-Fifty-one patients with non-permanent AF were randomized to AIT (n=26) consisting of four 4-minute intervals at 85-95% of peak heart rate 3 times a week for 12 weeks or a control group (n=25) continuing their regular exercise habits. An implanted loop recorder measured time in AF continuously from 4 weeks before to 4 weeks after the intervention period. Cardiac function, peak oxygen uptake (VO2peak), lipid status, QoL, and AF symptoms were evaluated before and after the 12-week intervention period. OBJECTIVE To determine whether an activity specific exercise program could improve ability to perform basic mobility activities in long-term care residents with Alzheimer disease (AD). DESIGN Randomized, controlled, single-blinded clinical trial. SETTING Residents of 7 long-term care facilities. PARTICIPANTS Eighty-two long-term care residents with mild to severe AD. INTERVENTION An activity specific exercise program was compared to a walking program and to an attention control. MEASUREMENTS Ability to perform bed mobility and transfers was assessed using the subscales of the Acute Care Index of Function; functional mobility was measured using the 6-Minute Walk test. RESULTS Subjects receiving the activity specific exercise program improved in ability to perform transfers, whereas subjects in the other 2 groups declined.
While obesity is known to have many physiological consequences, the psychopathology of this condition has not featured prominently in the literature. Cross-sectional studies have indicated that obese children have increased odds of experiencing poor quality of life and mental health. However, very limited trial evidence has examined the efficacy of exercise therapy for enhancing mental health outcomes in obese children, and the Sheffield Obesity Trial (SHOT) will provide evidence of the efficacy of supervised exercise therapy in obese young people aged 11-16 years versus usual care and an attention-control intervention. In a recently published paper, a team of researchers, led by Federal University of Porto Alegres Felipe B. In this pilot study, the authors investigated the feasibility of a home-based, remotely guided exercise intervention for patients with gliomas. This was achieved through a pilot randomized controlled trial (RCT) with randomization (2:1) to exercise or control group.. The purpose of this study was to identify factors of adherence, evaluate current exercise interventions, and determine areas of risk factor modification within the context of developing exercise intervention programs for healthy adult females with overweight and obesity. In recent years, overweight and obesity rates have significantly increased in the adult female population while their physical activity levels have declined. Health and wellness benefits of exercise interventions are widely known, however; practical guidelines do not exist for nurses to utilize in management of overweight and obesity. An integrative review was conducted to identify and evaluate components of exercise intervention development that would contribute to higher rates of adherence. Exercise therapy has been shown to be beneficial in other cancer populations, but no evidence is available for brain cancer survivors. Therefore, we report results from 2 preliminary cases. Methods . Two female patients diagnosed with glioblastoma multiforme and oligodendroglioma participated in a structured and supervised 12-week exercise program. The program consisted of two 1-hour resistance and aerobic exercise sessions per week and additional self-managed aerobic sessions. Outcome measures of strength, cardiovascular fitness, and several psychological indicators (depression, anxiety, and quality of life) were recorded at baseline, after 6 weeks and at the conclusion of the intervention.

Implementing care of stroke patients can be challenging in many ways. Medication adherence, access to early intervention, dietary management, and many other tactics to improve health are hard to move from theory to practice. In this session, we will take on the special problem of implementing exercise interventions to improve fitness after stroke. The literature indicates, strongly and with consensus across many disciplines, that exercise improves health after stroke. However, most neurologists are not familiar with implementation science: this literature offers a roadmap toward optimal use of evidence-based care methods. We will start by discussing specific exercise interventions, and the evidence and discipline-specific consensus documents supporting their use. Then, the emphasis of this session will be on obstacles to implementing these interventions in neurological care.
