My spouse actually tried to keep up the practice of thorough follow-up care on his patients in the large system he used to work for (7th largest in the country). When new administrators arrived on the scene, they saw the beauty of cutting care because of the bundled payments, and mandated that PAs do follow-up in the hospital. That way, they could keep the surgeons in the OR and still bill for the PAs work. My spouse objected. At that hospital he was known among the 4 neurosurgeons as the only one who takes care of his patients. BACKGROUND: Human infection studies (HIS) are valuable in vaccine development. Deliberate infection, however, creates challenging questions, particularly in low and middle-income countries (LMICs) where HIS are new and ethical challenges may be heightened. Consultation with stakeholders is needed to support contextually appropriate and acceptable study design. We examined stakeholder perceptions about the acceptability and ethics of HIS in Malawi, to inform decisions about planned pneumococcal challenge research and wider understanding of HIS ethics in LMICs. METHODS: We conducted 6 deliberative focus groups and 15 follow-up interviews with research staff, medical students, and community representatives from rural and urban Blantyre. Clinical ethics consultation (CEC) is a service provided by clinical ethicists (or sometimes, clinical ethics committees) to enhance patient care by identifying, analyzing, and resolving ethics dilemmas in clinical settings. CEC has long been offered as part of health care services in the US, but it is less common in other countries, perhaps because of a lack of trained personnel due to limitations in the number of clinical ethics fellowships. A result of this relative lack of clinical ethics training is that, in some parts of the world, CEC is either not available or it is performed by unskilled personnel.

The study was conducted over a five-month period in the Intensive Care Unit (ICU), Royal Childrens Hospital, Melbourne, and was approved by the hospital ethics committee. The ICU is an 18-bed multidisciplinary unit treating critically ill infants and children. During the study, 807 patients were admitted: 37% had undergone cardiac surgery, 27% had respiratory illnesses, 9% had general medical illnesses, 8% had newborn medical and surgical illnesses, 8% had trauma, and 8% had undergone neurosurgery and 3% general surgery. Mechanical ventilation was provided for 84% of patients. Most had invasive monitoring or therapeutic devices inserted into peripheral arteries, peripheral or central veins, the thoracic cavity, the urinary bladder or the extradural space. The mortality rate was 5.9%. The subjects of the study were any medical officers who made hand contact with any patient in the ICU. Background Increasing numbers of severely obese young people undergo bariatric surgery in the USA with reports of 58-73 % of excess weight lost after one year. In 2006, NICE suggested considering surgery for young people in exceptional circumstances. We present 5 patients operated upon between 2005-2011 at our centre focussing on the ethical and clinical dilemmas faced when establishing a service.. Case series/methods Five patients (4 male) aged 14-16 years (mean age 15.25) underwent bariatric surgery. Initial cases were discussed by the local clinical ethics committee prior to the procedures. Mean pre-operative BMI was 60.3 kg/m2 and BMI SDS + 4.3. Co-morbidities included hypertension, insulin resistance, obstructive sleep apnoea, limited mobility and psychosocial issues.
