KCNH2 encodes the Kv11.1 α-subunit that underlies the rapidly activating delayed-rectifier K+ current in the heart. Loss-of-function KCNH2 mutations cause long QT syndrome type 2 (LQT2), and most LQT2-linked missense mutations inhibit the trafficking of Kv11.1 channel protein to the cell surface membrane. Several trafficking-deficient LQT2 mutations (e.g., G601S) generate Kv11.1 proteins that are sequestered in a microtubule-dependent quality control (QC) compartment in the transitional endoplasmic reticulum (ER). We tested the hypothesis that the QC mechanisms that regulate LQT2-linked Kv11.1 protein trafficking are mutation-specific. Confocal imaging analyses of HEK293 cells stably expressing the trafficking-deficient LQT2 mutation F805C showed that, unlike G601S-Kv11.1 protein, F805C-Kv11.1 protein was concentrated in several transitional ER subcompartments. Background-Type 2 long QT syndrome (LQT2) involves mutations in the human ether a-go-go-related gene (hERG or KCNH2). T421M, a S1 domain mutation in the Kv11.1 channel protein, was identified in a resuscitated patient. We assessed its biophysical, protein trafficking and pharmacological mechanisms in adult rat ventricular myocytes (ARVMs). Methods and Results-Isolated ARVMs were infected with WT- and T421M-Kv11.1 expressing adenovirus and analyzed using patch clamp, Western blot and confocal imaging techniques. Results from this study show that hydrogen peroxide modulates the activation and deactivation kinetics of HERG, a channel that produces a current with characteristics similar to the major cardiac repolarizing current IKr. External application of H2O2 induced an acceleration of activation and deactivation of HERG and a negative shift of about 10 mV in V½ of the activation I-V curve (which reflects the acceleration of the activating current) at all H2O2 concentrations tested (30 μM, 100 μM, and 1 mM). Action potentials recorded from Purkinje cells in cerebellar slices exhibited a decreased threshold potential and increased frequency of spontaneous and repetitive activity following application of the specific erg channel blocker E-4031. Accommodation was absent before and after drug application. The erg current of these Purkinje cells activated at membrane potentials near -60 mV and exhibited fast gating kinetics. The two main changes in hERG K+ current caused by STX (Fig. 1 and 2) observed at steady-state were the reduction of current amplitude and the shifting of activation curves toward more positive membrane potentials. STX, acting as a hERG K+ channel-gating modulator, alters membrane potential dependence of the open probability.. We can rule out a simple pore block mechanism based on several experimental observations. The decrease in current was greatest at membrane potentials where the activation curve was shifted the most, not where Popen was maximal (as expected for a open state pore blocker). There was no tail current crossover as is seen with some open state blockers that slowly dissociate from the open state during channel closure. HERG has unusual biophysical properties distinct from those of other K(+) channels. While the conventional pulse protocols in patch-clamp studies have helped us elucidate these properties, their limitations in assessing HERG function have also been progressively noticed. We employed AP-clamp techniques using physiological action potential waveforms recorded from various regions of canine heart to study HERG function in HEK293 cells and identified several novel aspects of HERG function.
