Standard treatment for unresectable esophageal squamous cell carcinoma (ESCC) without distant metastasis is definitive chemoradiotherapy (dCRT), in which the incidence of esophageal fistula (EF) is reported to be 10-12%. An ad hoc analysis of JCOG0303, a phase II/III trial of dCRT for patients with unresectable ESCC (including non-T4b), suggested that esophageal stenosis is a risk factor for EF. However, risk factors for EF in patients limited to T4b ESCC treated with dCRT have yet to be clarified. The aim of this study was to investigate risk factors for EF in T4b thoracic ESCC treated with dCRT. We retrospectively analyzed the data of consecutive T4b thoracic ESCC patients who were treated with dCRT (cisplatin and fluorouracil) at Shizuoka Cancer Center between April 2004 and September 2015. Primary aortoesophgeal fistulas (AEF) are a rare but life-threatening condition because of substantial hemorrhage, requiring fast treatment to ensure patient survival. We report a case of a 69-year-old male with diagnosis of squamous cell carcinoma of the esophagus who suffered an episode of hematemesis and hemorrhagic shock. Gastrointestinal (GI) endoscopy revealed an ulcerated lesion with pulsatile hemorrhage. CT-scan confirmed the diagnosis of AEF. A stent-graft was placed in the descending aorta to control bleeding, and 2 days later an esophageal stent was deployed to reduce risk of aortic graft infection. The patient was discharged 13 days after admission and had no other episode of GI bleeding in a 6-month follow-up period. TEVAR may be used as a palliative or bridge treatment of AEF.. ...

The exact incidence of this condition is unknown, as are the major precipitating factors and best treatment options. We carried out a systematic review of the available case report literature reporting AAF. We systematically reviewed literature on AAF formation and closure. Separate Medline (PubMed), EMBASE, and Cochrane database queries were performed. Splenic arteriogastric fistula is a rare cause of upper gastrointestinal (GI) hemorrhage, only reported a handful of times in the literature. Herein, we present a case of a 49-year-old woman with metastatic adrenocortical carcincoma who developed a fistula between the splenic artery and gastric lumen as a result of local invasion of her primary tumor. This fistula led to several episodes of massive upper GI bleeding. Selective splenic artery embolization was successful in ceasing the GI bleed; however, the intervention was not timely enough for the patient to survive the hemorrhage. We outline several clinical and imaging findings to assist physicians with earlier detection of splenic arteriogastric fistulas, and advocate prophylactic selective splenic artery embolization when this rare but highly fatal entity is discovered.

An exploratory celiotomy was performed revealing no obstruction of the duodenum or pylorus. Approximately six liters of impacted feed were removed from the abomasum.
