Abstract STUDY OBJECTIVE: Chronic endometritis is a subtle condition that is difficult to detect; however, it may cause abnormal uterine bleeding and infertility. Few data exist about the appearance of chronic endometritis at fluid hysteroscopy and about the value of diagnostic fluid hysteroscopy in the detection of this condition. In our experience, at fluid hysteroscopy chronic endometritis is characterized by consistent association of stromal edema and either focal or diffuse hyperemia; in some cases, this finding is associated with endometrial micropolyps (less than 1 mm in size). This study attempted to describe diagnostic criteria for chronic endometritis at fluid hysteroscopy and assess the diagnostic accuracy of fluid hysteroscopy in the detection of this condition. DESIGN: Retrospective Study (Canadian Task Force classification II-2). SETTING: University hospital. PATIENTS: Nine hundred-ten women in whom hysteroscopy was indicated. INTERVENTIONS: Fluid hysteroscopy followed by ...

Endometritis is caused by infections such as chlamydia, gonorrhea, tuberculosis, or mixtures of normal vaginal bacteria. Endometritis is more likely to occur after miscarriage or childbirth, especially after a long labor or c-section.. A medical procedure that involves entering the uterus through the cervix will increase the risk of developing endometritis. This includes a hysteroscopy, and placement of an intrauterine device (IUD).. Endometritis can occur at the same time as other pelvic infections such as acute salpingitis, acute cervicitis, and many sexually transmitted infections (STIs).. ...
OBJECTIVE: To evaluate the type and etiopathogenic role of infectious agents detected in endometrial cultures obtained from women with chronic endometritis (CE). DESIGN: Prospective controlled study. SETTING: University hospital. PATIENT(S): 2190 women undergoing hysteroscopy for different indications. INTERVENTION(S): Vaginal and endometrial samples were collected from 438 women with a CE diagnosis at hysteroscopy and 100 women with no signs of CE (controls). MAIN OUTCOME MEASURE(S): Histology and cultures for common bacteria, Neisseria gonorrhoeae and Mycoplasma, and molecular biology testing for Chlamydia were performed. RESULT(S): We compared results of vaginal and intrauterine cultures obtained from women with and without CE. Histologic results were positive in 388 of these cases (88.6%), and at least one microorganism was found in 320 endometrial samples (73.1%). In the control group, histologic results and endometrial culture were positive in only 6% and 5% of cases, respectivel
Knowing if the vaginal and endometrial microbiome can be a diagnostic method for chronic endometritis is one of the objectives of Instituto Bernabeus
The aim of this study was to determine signs of subclinical endometritis found by ultrasound that are associated with reduced fertility in dairy cows. The maximum diameter of the uterine lumen was determined by ultrasound in 324 cows without clinical signs of endometritis after evaluation of the genital tract 21 to 27 days postpartum. Cows were classified into healthy or with subclinical endometritis by three threshold values for the maximum uterine lumen diameter of 0.2 cm, 0.5 cm or 0.8 cm. Examinations by rectal palpation and ultrasound as well as classifications were repeated 14 days later. In addition, ovaries were scanned by ultrasound to determine the stage of the estrous cycle. In a subgroup of 103 cows the echotexture of the uterus and its contents was evaluated. In these cows the diagnosis of subclinical endometritis was performed by a scoring system. The diameter of the uterine lumen was significantly affected by stage of the estrous cycle at the time of examination. However, no ...
Endometritis is inflammation of the lining of the uterus that usually occurs as a complication of an infection. Signs of endometritis include fever, lower abdominal pain, and bleeding between menstrual periods.. Endometritis can be caused by an infection in a womans uterus related to childbirth or by an infection that spreads from another part of the body, such as the vagina.. Treatment with antibiotics usually clears up the infection and inflammation within a few days. Without treatment, the infection can spread to the bloodstream and can cause serious illness.. ...
Cesarean delivery rates are increasing in Turkey and a major component of this increase is cesarean on demand. Although data on the rate of cesarean delivery in Turkey is limited, a national study reported a rate of 23.8%. Infectious morbidity, consisting primarily of endomyometritis and wound infection, remains a leading cause of postoperative complications. Estimates of postcesarean infection rates range from 7% to 20%, depending on demographic and obstetric variables. Infection following cesarean delivery results in not only increased hospital stay but also increases the cost of care. Strategies to minimize postoperative infectious and other morbidities have included modifications of surgical technique, changing of gloves, methods of placental delivery, cervical dilatation during cesarean delivery, and altering the uterine position during repair of the uterine incision. Despite these interventions, endometritis is still major problem after cesarean delivery.. Endometritis appears to result ...

Very few reports have been made about endometritis in the bitch. Today, transcervical catheterization by vaginal endoscopy allows us to evaluate the endometrium. Diagnosis criteria were determined according to previous studies on uterine cytology and bacteriology. The aim of our study was to test the efficiency of this technique to diagnose endometritis and furthermore, to evaluate its incidence within infertile bitches.. Material and Methods. Twenty-six bitches presented for infertility in Alfort Veterinary College were included. Classical infertility investigations were not indicative. A vaginal endoscopy was performed and presence of vaginitis and cervical discharge were evaluated. Then, a transcervical catheterization was performed using a human ureteral catheter (Ureteral CRU® ch.6 223602). Flushing of the uterine lumen was realized with sterile saline fluid (NaCl 0.9%, 2mL/10 kg instilled then reabsorbed) and collected samples were used for uterine cytology and aerobic bacteriology. If ...
Women who give birth via cesarean section (C-section) have an increased risk for developing specific infections, for example, postpartum endometritis and post-cesarean wound infection. Postpartum endometritis also afflicts women who deliver their babies vaginally, although it is more rare among this group. Further, all new mothers and women who breast-feed their babies should be aware of mastitis, which is an infection and inflammation in and around the milk producing glands of the breast. ...

If you have open brood mares at this time of year, then something occurred that prevented them from becoming pregnant. The most common cause of infertility mares is endometritis or inflammation of the uterus. When most people think of endometritis, they think of infection, and certainly infection can cause endometritis. However, a condition called breeding-induced endometritis can also lead to infertility in mares. The uterus has a normal inflammatory response to breeding that is necessary to remove contaminating bacteria and excess semen and seminal fluid from the uterus. In a healthy uterus, this inflammation subsides in 48 hours. However, in some mares the inflammation does not subside but continues through days 5-6 when the embryo drops from the fallopian tubes into the uterus. This persistent mating-induced endometritis is fairly common in older mares. If inflammation is in the uterus when the embryo arrives on days 5-6, the embryo will not be able to implant and pregnancy will not ...
During the early months of the COVID-19 pandemic, azithromycin was one of the viruss unproven, potential treatments, and the resulting shortage forced US doctors to rely on the substitute drug clarithromycin as an adjunct surgical prophylaxis for preventing endometritis in non-elective cesarean deliveries. Now, new research findings reveal that the option is a safe alternative.. In a recent study published by PLOS One, 4.5% of healthy women who were given clarithromycin and needed cesarean deliveries developed endometritis compared with 11.2% of those who didnt receive any adjunct macrolide prophylaxis (crude analysis, P = 0.025).. Postpartum endometritis is a womb infection with a 1% to 3% incidence rate after vaginal delivery, but anywhere from a 13% to 90% incidence rate after cesarean delivery, according to Medscape.. ...

Uterine diseases in cattle occur at all stages of the reproduction cycle but the majority of cases is found in the postpartum period. The inflammation of the uterus is generally defined as metritis or endometritis, with several graduations, e.g. puerperal metritis, clinical metritis, clinical or subclinical endometritis. Whether uterine diseases have a negligible, moderate or detrimental effect on fertility is still under discussion and depends on definitions and classification. In the past, it was assumed that the pregnant uterus is free of pathogens, but recent studies found several species including pathogens in the uterus and endometrium of pregnant cows. After parturition, a broad diversity of bacteria with >200 different species has been found in the early postpartum period. Not all of these bacteria, however, are considered as pathogens. Furthermore, bacteriological findings provide only evidence for infection but not for inflammation. For some bacteria, particularly Escherichia coli and ...
Low-dose insemination has been proposed to reduce persistent breeding-induced endometritis (PBIE) in mares with delayed uterine clearance (DUC). Others proposed that hysteroscopic insemination induces an exaggerated inflammatory response and should be avoided in DUC mares. The objectives here were to evaluate presence and severity of PBIE in normal and DUC mares after hysteroscopic insemination with fresh semen, and to determine if hysteroscopy could be used in DUC mares without inducing excessive inflammation. Reproductively normal (n=4) and DUC (n=5) mares received four treatments in random order: uterine body insemination (UB, 1×10 9 spermatozoa, 20ml), hysteroscopic insemination (HYST, 5×10 6 spermatozoa, 0.5ml), sham hysteroscopic insemination (SHAM, semen extender, 0.5ml) and hysteroscopic infusion of seminal plasma (SP, 0.5ml). Significantly more DUC (50%) mares than normal (14%) mares accumulated intrauterine fluid 24h post-treatment. The difference in fluid accumulation between DUC ...
article{8500859, abstract = {The aims of our field study in dairy cows were (1) to consolidate cytotape (CT) as a valid technique to diagnose cytological endometritis (CYTO) during artificial insemination (AI); (2) to establish a cutoff point concerning the polymorphonuclear cells (PMN) proportion to diagnose CYTO at AI; (3) to assess the prevalence of CYTO at AI; and (4) to evaluate the effect of CYTO on the pregnancy outcome of that AI. The investigation was performed using 1,625 AI-CT samples harvested from 873 Holstein-Friesian cows from 18 dairy farms in the Flemish region of Belgium. The CT device consisted of adapting a 1.5-cm piece of paper tape on the top of a conventional AI catheter covered with a double guard sheet, allowing an endometrial cytology sample to be taken when performing an AI. A receiving operator characteristic curve was built to assess the threshold level above which the PMN proportion significantly affected the AI success. Multilevel generalized mixed-effect models ...
The finding of granulomas in an endometrial biopsy can be caused by a variety of diseases. In a recent review granulomas were identified in 0.15% of endometrial biopsies.20 Focal granulomas, that is the finding of a single granulomatous focus, were significantly associated with a prior history of cesarean section or dilatation and curettage, and none of the women had any signs of a systemic disorder or infection upon follow-up. In terms of infection, granulomas can be caused by tuberculosis (as described previously), atypical mycobacteria, endemic mycosis, actinomycosis, and parasites.20,21 Noninfectious causes can include a reaction to foreign material, Crohn disease, sarcoidosis, or lymphoma. Granulomas can also be seen postsurgically. Sarcoidosis is associated with well-formed, clearly demarcated epithelioid cell granulomas lacking central necrosis. It occurs usually, but not always, in women with known systemic disease. However, as necrosis is unusual in tuberculous endometritis, endometrial ...
Young mares and mares with healthy reproductive tracts usually conceive easily and carry a foal to term without difficulty. Attempts to get some older mares and mares with reproductive problems pregnant can be very frustrating and costly. In this article I discuss a few of the factors that result in uterine problems. I then describe the diagnosis, treatment and management of mares with post-breeding endometritis, the most common cause of mare infertility. Understanding the basics of how the equine reproductive tract functions will help you to understand the problems that can develop.. THE EQUINE UTERUS. The uterus is a muscular, hollow organ with a lining that is designed to attach to a placenta and provide the nutrients needed from the mare to sustain a foal. The uterus is protected from the outside world by several barriers to infection. The vulva forms a tight seal and helps to keep contaminants out of the vagina and uterus. The cervix is a thick tube (separating vagina from uterus) that ...

TY - JOUR. T1 - Vaginal chlorhexidine during labour for preventing maternal and neonatal infections (excluding Group B Streptococcal and HIV).. AU - Lumbiganon, P.. AU - Thinkhamrop, J.. AU - Thinkhamrop, B.. AU - Tolosa, Jorge. PY - 2004. Y1 - 2004. N2 - BACKGROUND: The incidence of chlorioamnionitis occurs in between 8 to 12 women for every 1000 live births and 96% of the cases of chlorioamnionitis are due to ascending infection. Following spontaneous vaginal delivery, 1% to 4% of women develop postpartum endometritis. The incidence of neonatal sepsis is 0.5% to 1% of all infants born. Maternal vaginal bacteria are the main agents for these infections. It is reasonable to speculate that prevention of maternal and neonatal infections might be possible by washing the vagina and cervix with an antibacterial agent for all women during labour. Chlorhexidine belongs to the class of compounds known as the bis-biguanides. Chlorhexidine has antibacterial action against a wide range of aerobic and ...
ABSTRACT) Delayed uterine clearance (DUC) is a significant problem contributing to subfertility and infertility in the mare, characterized by an accumulation of fluid and inflammatory debris in the uterine lumen following breeding events, venereal disease or an estral cycle. This syndrome is typically seen in older, multiparous mares and mares with poor reproductive tract conformation. The etiopathogenesis of DUC has not been fully elucidated but suggested causes include poor genital conformation, a cranioventrally tilted uterus, defective myometrial contractions, decreased intrauterine immune activity, inappropriate lymphatic drainage or mucus overproduction. The objective of this research was to evaluate gene expression of selected genes in endometrial tissue samples taken from three categories of mares (young fertile [YF], older clinically normal [ON] and older susceptible [OS]). The genes assayed in this research were oxytocin receptor, PGF2á receptor and progesterone receptor. The ...
Infectious morbidity for the mother, consisting primarily of endomyometritis and wound infection remains a leading cause of post operative complications. Infectious morbidity following CD remains among the top five causes of mortality in mothers worldwide, causing a huge burden to the society [9].. In the study by Sullivan et al. [7], on 357 women, decreased total infectious morbidity in the study group was found (RR = 0.4, 95% CI = 0.18 to 0.87). There was a decreased endometritis risk as well in the pre incision group (RR = 0.2, 95% CI = 0.15 to 0.94).There was no increase in neonatal sepsis (P = 0.99), sepsis work up (P = 0.96) or increased hospital stay (P = 0.17). In the present study, the results were almost similar in that, the incidence of SSI was decreased and duration of hospital stay was decreased with antibiotics given Pre-incision than after cord clamping. Unlike the study by Sullivan et al. [7], in the present study, the incidence of endometritis was similar in both groups. ...

Endometritis is one of the major problems in the horse breeding industry. The use of antibiotics for treatment of endometritis in the mare is recommended as best practice. The intrauterine application of antibiotics, however, has been under discussion over the last years because of concerns about its efficacy. The systemic use of antibiotics has been considered more effective because of its better distribution within the uterus. The objective of the present study was to determine the concentration of ceftiofur derivates in serum and endometrial tissue after intramuscular administration. Specifically, the authors tested the hypothesis that ceftiofur concentrations in serum and endometrial tissue remain above the minimum inhibitory concentration (MIC) for common uterine pathogens for 24 h. Nine mares in estrus received a single dose of 2.2 mg/kg ceftiofur hydrochloride intramuscular per kg of body weight. Blood samples and endometrial tissue were obtained immediately before treatment (-1 h) and 2 ...

Metroendometritis is an inflammation of the mucous membrane and the muscular membrane of the uterus due to infection in the uterus. Most often, this disease occurs after childbirth, when the surface of the uterus, in fact, is a wound surface that is not protected with virtually nothing from the infection.. Metroendometritis can also appear after a failed abortion, miscarriage, as a result of trauma to the genitals, removal of the tumor of the uterus, polyps of the uterus. Meteoroid metritis can be caused by typhus, influenza, colds.. The acute form of metroendometritis can be accompanied by severe intoxication of the womans body, paresis of the intestine. In the absence of proper treatment, the disease acquires a chronic course, causes infertility, and sometimes an abscess of the uterus. In the latter case, treatment is possible only by surgical removal of the uterus.. Attention! Cure metroendometrit by using hirudotherapy or folk remedies is impossible.. Met endometritis is preceded by ...

Several interventions are of proven effectiveness in decreasing the frequency of infection after major pelvic surgery. First, patients who have preexisting medical illnesses should be stabilized before surgery. Second, to the greatest extent possible, prolonged preoperative hospitalization should be avoided so that the patients risk of being colonized with hospital-acquired bacteria is reduced. Compared with the patients endogenous bacterial flora, such organisms are more likely to be resistant to commonly used antibiotics. Third, the physician should pay strict attention to those surgical techniques that clearly have been proven to reduce the rate of postoperative infection. For example, the risk of postcesarean endometritis is decreased when the placenta is removed by traction on the umbilical cord rather than by manual extraction.14, 15. Similarly, in obese patients having cesarean delivery, closure of the lower half of the subcutaneous layer reduces the frequency of wound infection, ...
