BACKGROUND AND PURPOSE: Patients with myotonic dystrophy type 1 (DM1) have an increased incidence of endocrine dysfunction. In this study, the temporal evolution of endocrine dysfunction in patients with DM1 was investigated. METHODS: Endocrine function was assessed in 68 patients with DM1, in whom endocrine function had been followed, on average, for 8 years. The endocrine function was assessed by measuring the concentration of hormones and metabolites in blood and by validating libido with questionnaires. RESULTS: At baseline, 30 of the 68 patients presented with at least one hormonal dysfunction. When re-evaluated after 8 years, 57 of 68 patients had endocrine dysfunction. Diabetic patients had increased from one to four. At follow-up, hyperparathyroidism occurred in 25% and abnormal thyroid-stimulating hormone in 21%, compared with 14% and 9% at baseline. In rabbits, mean arterial pressure (MAP) increases in response to fat feeding, but does not increase further with progressive weight gain. We documented the progression of adiposity and the alterations in endocrine/cardiovascular function in response to fat feeding in rabbits, to determine whether stabilization of MAP after 3 weeks could be explained by stabilization of neurohormonal factors. Rabbits were fed a control diet or high-fat diet for 9 weeks (n¼23). Fat feeding progressively increased body mass and adiposity. Heart rate (HR) was elevated by week 3 (15±3%) but changed little thereafter. The effects of fat feeding on MAP were dependent on baseline MAP and peaked at 3 weeks. We present two children with rapid onset obesity presenting to hospital with acute respiratory failure requiring admission to paediatric intensive care within a week of each other. The first case is a Caucasian girl who had exhibited hyperphagia and food stealing behaviour from the age of 17 months. Aged three years old, she suffered a respiratory arrest in hospital where she was admitted with acute renal failure. At this point, she weighed 32 kg (BMI 22 kg/m2, 3.4 SDS). Investigations revealed hyperprolactinaemia (8087 mu/l), a failed growth hormone (GH) stimulation test (peak 0.2 ug/l) and hypernatraemia (184 mmol/l). OBJECTIVE To define the prevalence, risk factors, and age at diagnosis of endocrinopathies in beta-thalassemia major (BTM) in Northwest Saudi Arabia. METHODS This retrospective cross-sectional study included patients with BTM attending a combined endocrine-hematology clinic in Al-Madinah, Kingdom of Saudi Arabia from March 2009 to December 2010. Clinical and biochemical data from the initial clinic visits were used to define the prevalence and age of diagnosis of endocrinopathies. Demographic and laboratory variables were analyzed to identify significant risk factors. RESULTS Eighty-one patients (42 males), aged 2-28 years were screened. Thirty-eight of them (46.9%) had at least one endocrinopathy. Of these, 28.9% (11/38) were aged less than 10 years. Hypogonadism was the most common complication detected in 52.7% (19/36) of patients of pubertal age group and 23.4% (19/81), of all cohort followed by short stature in 20.9% (17/81), subclinical hypothyroidism in 14.8% (12/81) and hypoparathyroidism

OBJECTIVE: To determine the major endocrine complications present in patients of Thalassemia major presenting to a tertiary care center in Karachi, Pakistan. METHODS: Records of all thallasemia major patients at a Haematology Department in a tertiary care hospital of Karachi were retrospectively reviewed from May to August 2009 with specific focus on endocrine data and investigations. The patients data was recorded in a questionnaire and analyzed using SPSS v.17, frequencies and percentages along with corresponding 95% confidence interval were computed. RESULTS: Our data revealed that a significantly small percentage of those under care received regular endocrine follow-up. Male hypo-gonadal abnormalities had the highest probability; 100% of the tested patients had decreased levels of testosterone, while 95.2% had raised serum creatinine levels.
