The Pipeline embolization device is a breakthrough and life-saving endovascular treatment for large or giant wide-neck brain aneurysms, said Joe Woody, president, Vascular Therapies, Covidien. Pipeline now offers hope for those patients who have had no other options for treating this often debilitating and even fatal medical condition.. The Pipeline embolization device PMA application was based on the results of the PUFS (Pipeline for uncoilable or failed aneurysms) clinical trial, a single-arm study of large and giant, wide-neck or fusiform aneurysms that included safety and efficacy data on 108 patients.. Brain aneurysms are a silent killer because most show no symptoms over time, said Christine Buckley, executive Director, The Brain Aneurysm Foundation. The Pipeline embolization device now provides patients with a safe and effective treatment of large or giant, wide-neck aneurysms, which remained an unmet clinical need until now.. The Pipeline embolization device has received CE mark ...
Y-configured double stent technique is useful for coil embolization of a bifurcation wide neck aneurysm while preserving the patency of the two important vessels. However, if the important vessels emanating directly from the aneurysm comprised four vessels, two vessels not deployed, Y-stents might not be preserved with the Y-stent technique by itself. We report a case treated with Y-configured double stent-assisted coil embolization with a double microcatheter technique for complex basilar bifurcation aneurysm. A 78-year-old woman presented with a subarachnoid hemorrhage (SAH) of poor grade. CT-angiography showed a wide neck and shallow aneurysm of complex basilar bifurcation involving both posterior cerebral arteries (PCAs) and superior cerebellar arteries (SCAs). In the chronic stage of SAH, Y-configured double stent-assisted coil embolization with a double microcatheter technique was performed. After Y-stent (two Enterprise) deployment from both the PCAs to the basilar artery, coil ...
The pipeline embolization device (PED) is a well recognized treatment for intracranial aneurysms. However, uncertainty remains regarding its effects on flow alteration, which is particularly highlighted by persistently perfused aneurysmal remnants and non-regressing, non-perfused aneurysmal masses. Here we present a 68-year-old woman with an incidental giant fusiform right paraophthalmic aneurysm electively treated with a PED. After lowering her antiplatelet therapy to promote aneurysm thrombosis, she was found to have a progressively enlarging perfused aneurysmal remnant. Angiography revealed PED occlusion, but curiously the development of a peri-construct collateral channel which feeds the aneurysmal remnant, and gives rise to distal branches and contributes to middle cerebral artery flow. The large thrombosed aneurysmal mass showed tiny internal vessels on cone beam CT angiography as well as florid enhancement on MRI, further confirming that apparently thrombosed remnants are biologically ...

MANSFIELD, Mass., Apr 06, 2011 (BUSINESS WIRE) --. Covidien (NYSE:COV), a leading global provider of healthcare products, today announced that it received Premarket Approval (PMA) from the U.S. Food and Drug Administration (FDA) for the Pipeline(R) Embolization Device, indicated for the endovascular treatment of adults (22 years of age or older) with large or giant wide-necked intracranial aneurysms in the internal carotid artery from the petrous to the superior hypophyseal segments.. Pipeline is a new class of embolization device designed to divert blood flow away from the aneurysm in order to provide a complete and durable aneurysm embolization while maintaining patency of the parent vessel.. A cerebral aneurysm is a weak spot in the wall of a blood vessel within the brain, characterized by an abnormal ballooning or widening of the vessel. A brain aneurysm ruptures every 18 minutes and 40% of these cases are fatal. Of those who survive, about 66% suffer some permanent neurological ...
BACKGROUND AND PURPOSE: Utility of the Pipeline Embolization Device extending to the M1 and its clinical and flow consequences at the ICA bifurcation have not been characterized. We analyzed flow modification in cases where a single Pipeline Embolization Device was deployed from the M1 to the distal supraclinoid ICA, covering the A1, for aneurysm treatment. MATERIALS AND METHODS: A1 flow modifications and size regression in postprocedure and follow-up angiography were analyzed. Vessel diameters and ratios of the proximal A1 and M1 segments and the distal ICA were assessed. Relationships between Pipeline Embolization Device nominal diameter and the vessel diameters at landing zones were obtained. Clinical assessments after flow modification were documented. RESULTS: Six of 7 patients demonstrated no change of flow in the anterior cerebral artery/anterior communicating artery complex at immediate postembolization angiography. All patients who underwent follow-up angiography demonstrated size regression of

Stent-assisted coil embolization is very effective for treating fusiform and wide-neck aneurysms. This technique enables improved packing density with a relative low risk of coils herniating into the parent artery, which may also be beneficial for small to medium aneurysms.35 In addition, a fully deployed stent may function as a scaffold for endothelial growth.7. The standard technique for follow-up imaging after (stent-assisted) coil embolization of intracranial aneurysms is DSA, which only provides a 2D projection of the vascular anatomy and implants. Diagnostic interpretation is therefore determined by the projection angle of the x-ray source and may not fully disclose adjacent vascular anatomy and potential clot formation, stent-wall apposition, stent herniation, recanalization, intimal tissue growth, or hyperplasia. In situ-acquired high-resolution contrast-enhanced CBCT (VasoCT) allows visualization of vascular implants and host arteries with 3D spatial information. However, the presence ...
This study was a detailed analysis of the healing response of elastase-induced aneurysms in rabbits after they were densely packed with a new platinum coil system. The angiographic findings in our study demonstrated a stable coil configuration in most of the cases, with slight compaction seen in 6% of the aneurysms. This finding concurs with those of a clinical series (16) that had similar rates of compaction among aneurysms with a diameter less than 10 mm, such as those used in our study. The histologic findings were highly concordant within each study group, as long as 12 weeks after embolization. These findings indicate that aneurysmal domes remain nearly completely filled with unorganized blood for longer than 4 weeks after embolization. At 12 weeks after embolization, connective tissue replaced most of the blood products in the dome, but the exact type of tissue remains impossible to characterize with the embedding and staining techniques used in our study. Fibrin represents the predominant ...

BACKGROUND AND PURPOSE: Thrombus formation during endovascular embolization of intracranial aneurysms occurs in 2.9%-6% of patients. Use of IIb/IIIA inhibitors such as abciximab or eptifibatide intravenously has been reported in management of this complication. Because the intra-arterial infusion of IIb/IIIA inhibitors may require lower doses to achieve thrombolysis, it may reduce the risk of haemorrhage. Therefore, we retrospectively analyze our database and review the literature. METHODS: This is a retrospective analysis of a prospectively acquired database of patients with ruptured or unruptured aneurysm treated intra-arterially for thrombus formation during endovascular coil embolization between July 2005 and August 2008. Patient demographics, aneurysmal characteristics, procedural, clinical outcome and complications were recorded. RESULTS: From July 2005 to August 2008, out of 184 patients who underwent coil embolization, 19 patients (15 smokers, 14 female, mean age 52) developed intraprocedural

We report the first European case of cerebral iodized lipid embolism post transcatheter arterial embolization for hepatocellular carcinoma. Lipiodol emboli and corresponding multifocal brain ischemia were documented with computed tomography (CT) and magnetic resonance (MR) in the acutely symptomatic patient. Transcranial Doppler sonography with contrast indicated a right-to-left shunt, while on a follow-up CT scan lipiodol embolization was detected in both lungs. Dilated pulmonary vessels and thick vascular channels were seen in the vicinity of the right diaphragm suggestive of pulmonary arteriovenous shunt. The patient symptoms regressed with supportive care alone, but he died 5 months later due to hepatic failure unrelated to the procedure.
This detachable coil system leverages design elements from Codmans Trufill DCS Orbit coils, but adds new features including stretch resistance, the soft, complex Xtrasoft Finishing coil, and the stable, conformable Frame coil.. Each coil has its own unique and random, complex shape. According to Van Rooij WJ et al, the use of complex, random loop coils have been shown to result in significantly higher packing than helical coils and complex coils with pre-determined shapes.. The Orbit Galaxy family of coils leverages proven and reliable coil technology with important new developments. The conformability and softness of these stretch resistant coils enables physicians to confidently fill an aneurysm very densely from the periphery to the core, said Elad I Levy, professor of Neurosurgery, University at Buffalo Neurosurgery, State University of New York, USA. Having a complete set of coils in one system helps streamline the procedure.. Endovascular coils are inserted directly into an aneurysm ...

A new digital angiography flat panel system reduces the radiation dose to patients undergoing interventional treatment for liver cancer by about one-fourth, a new study shows. The study compared the radiation dose to patients skin during transcatheter arterial embolization, a procedure that blocks the flow of a blood to a tumor. Doses were assessed for 12 patients using a new angiography unit with a digital flat-panel system and 12 using a conventional unit for angiographic imaging. The maximal skin dose to the patients ranged from 510-1,882 mGy using the conventional unit, said Shigeru Suzuki, MD of Teikyo University School of Medicine in Tokyo, Japan. Dr. Suzuki is the lead author of the study. That compares to 130-467 mGy for the flat panel system, he said. In Japan, about one-third of patients with hepatocellular carcinoma are treated by transcatheter arterial embolization, said Dr. Suzuki. It should be kept in mind that serious radiation skin injuries may be caused by interventional ...

The PED is a flexible, braided wire mesh tube which can be placed within the internal carotid artery in the brain, blocking off large or wide-necked aneurysms. The device can also reduce the likelihood that an aneurysm will rupture. The Pipeline® Embolization Device offers neurointerventional surgeons a minimally invasive alternative to open surgery or placement of materials such as coils or a liquid embolic when treating carotid artery brain aneurysms, said Avi Setton, MD, chief of interventional neuroradiology at North Shore University Hospital. While coiling can be effective for small brain aneurysms, it does not always work on larger ones. The PED device offers patients a better chance of long-lasting aneurysm occlusion and low complication rates.. In the endovascular procedure, the PED is threaded up through a catheter placed in a blood vessel in the leg, up to the brain. The PED is implanted across the neck of the aneurysm, cutting off blood flow. The blood that remains in the ...

AU - Echeverri, Jose. AU - Movahed, Mohammad Reza. PY - 2012/12/1. Y1 - 2012/12/1. N2 - The transradial approach for percutaneous coronary intervention (PCI) has gained increasing popularity as an alternative site for coronary angiography and intervention. The transradial approach has safely been used in patients with contraindications to the transfemoral approach. Despite its overall lower complication rates compared with the transfemoral approach, the transradial approach has its own inherent complications. The risk of arterial perforation with the transradial approach is less than 1%, primarily involving the radial artery with the incidence of compartment syndrome at approximately 0.004%. In addition, the risk of significant bleeding requiring transfusion with transradial approach is extremely rare, occurring ...

AU - Lanzino, Giuseppe. AU - Guterman, Lee R.. AU - Hopkins, L. Nelson. PY - 1998/7. Y1 - 1998/7. N2 - Guglielmi detachable coil (GDC) treatment for complicated cerebral aneurysms is an attractive option that has become widely accepted in recent years. This technique is usually considered only if the patient harbors an aneurysm that is not a good candidate for surgical clipping. However, the definition of surgical candidate varies among institutions, and many patients worldwide are being treated with GDCs as primary therapy. Although most centers currently perform follow-up angiography at 6 months to 1 year, others do not routinely perform it after an initially good result. The authors present a case that indicates longer follow up may be necessary and illustrates some of the pitfalls of GDC treatment. ...
Juvenile nasopharyngeal angiofibroma (JNA) is a rare benign neoplasm that occurs almost exclusively in the nasopharynx of adolescent males. Surgery remains the primary treatment of choice. JNA has always presented a management challenge to surgeons because of its vascular nature, site of occurrence, and local tissue destruction. The surgical approaches are either standard open method which include external or intraoral incisions, or the recent advanced approach, i.e. via using the endonasal endoscope. It is widely accepted that the use of preoperative angiographic embolization reduces the occurrence of intraoperative bleeding and facilitates tumour removal. However, angiographic embolization is not available at all centres. The purpose of this article is to present our experience with five patients diagnosed with JNA who were resected without embolization, using various surgical approaches. Two tumours were removed via endonasal endoscopic surgery. None of the tumours were embolized prior to ...
Extracranial vertebral artery aneurysms are extremely rare and account only for 0.5% of all aneurysms. Most extracranial vertebral artery aneurysms are located in the V3 segment followed by the V1 segment.7 These aneurysms are diagnosed secondary to an embolic infarct or incidentally as a palpable mass. Patients with connective tissue disorders, including Ehlers-Danlos syndrome, Marfan syndrome and neurofibromatosis type I are at higher risk of developing extracranial vertebral artery aneurysms. A ruptured vertebral artery aneurysm with local pain and haematoma are often found in this particular group of patients,.3 4 8 In contrast, our case report presents a young patient without any trauma who presented with sudden onset of severe supraclavicular pain. CT angiography and DSA are the standard tools to diagnose and reveal the anatomy of vasculature and to plan treatment. Treatment options include ligation, isolation, balloon embolisation, onyx embolisation and coil embolisation.6 9-11 There is ...

A Guglielmi detachable coil (GDC) is a medical device made of platinum used in the treatment of brain aneurysms. It led to a revolutionary advance in the field of endovascular minimally-invasive surgery for the occlusion of cerebral vascular disease. Invented by Dr. Guido Guglielmi, Italian neurosurgeon and professor of UCLA interventional neuroradiology in 1990, and gradually introduced in the later 1990s as an alternative to invasive surgical clipping. In September 1995, the Guglielmi Detachable Coil (GDC) system received formal approval from the U.S. Food and Drug Administration (FDA) for treatment of surgically high-risk intracranial aneurysms. Due to advances and patent rights, not all coils used today are GDCs, however they are modeled on the same mechanism. These coils have special physical and electrolytic properties: the material is a high shape-memory alloy, allowing it to easily bend and regain its original coil shape. These platinum coils are fairly soft and adaptable to the shape ...
Endovascular treatment of intracranial aneurysms with detachable coils has been widely used since the introduction of GDCs in 1991 and has been proven to be effective in preventing rebleeding after aneurysmal rupture. The clinical and angiographic results of endovascular coil occlusion of intracranial aneurysms are positive, with an initial and final overall complete occlusion rate of 35.9%-76.8% and 38.3%- 87.8%. In the mid- and long-term, however, aneurysm recanalization may occur in as many as one-third of cases.. The natural history of aneurysm recurrence after coil treatment is often benign, but bleeding from incompletely coiled aneurysms is a well-documented threat, moreover, the degree of aneurysm occlusion after treatment was strongly associated with risk of rerupture. Even if 100% occlusion of the aneurysms after the initial treatment was obtained on immediate postembolization angiography, there was still a relatively high recanalization rate (26.4%) on long-term follow-up angiography. ...

AU - Karwowski, John K.. AU - McKinsey, James F.. AU - Vouyouka, Ageliki G.. PY - 2010/1. Y1 - 2010/1. N2 - Persistent sciatic artery (PSA) is a rare vascular anomaly present in 0.025% to 0.05% of the population. They are particularly prone to aneurysmal degeneration, potentially leading to distal ischemia, sciatic neuropathy, or rarely rupture. Here, we describe a case of a ruptured PSA aneurysm managed by endovascular embolization. A 70-year-old man initially presented with acute left lower extremity ischemia. He was found to have a popliteal embolus originating from a complete persistent sciatic artery aneurysm. He underwent thrombolysis followed by a femoropopliteal bypass and ligation of the proximal popliteal artery to exclude the PSA. Four weeks later he re-presented with severe pain, a pulsatile ...
DISCUSSION The use of an alternative technique to surgery with a lower morbidity and shorter hospital stays represents a significant medical advance, because women who do not wish to undergo a hysterectomy for a fibroid tumor have now access to different less-invasive alternatives7. The embolization technique involves a selective blockade of the blood flow of the vessels of the fibroid with embolic agents through catheters. These embolic agents can be classified into different types8, 9, 10, and the literature does not describe any problems related to any possible reactions they might cause. The insertion of catheters does not require general anesthesia, and it can be done with local anesthesia and a mild sedation11, although the insertion of an epidural catheter allows for a proper analgesia and the control of pain after the embolization. The size of the fibroids that underwent embolization was variable, although they were usually of less than 10 cm. The effectiveness of the embolization of ...

Abstract: BACKGROUND AND PURPOSE: Self-expandable stents are an effective tool for coil embolization of wide-neck intracranial aneurysms. The purpose of this study was to assess the feasibility and results of bailout stent positioning during rescue situations after deployment of ≥1 coil.. MATERIALS AND METHODS: Among 318 aneurysms treated by coil embolization in 267 patients, 16 patients who were treated by bailout stent deployment were retrospectively reviewed. Bailout procedures were performed to relieve potential parent artery compromise caused by the protruded coil loops or to prevent migration of the unstable coil basket. The size/location of the aneurysm, technical feasibility, successful stabilization rate, and procedure-related complications were evaluated.. RESULTS: The locations of the aneurysms were the internal carotid artery (n = 12) and basilar artery (n = 4). The mean aneurysm size was 8.3 mm (range, 3.5-19.4 mm) with hemorrhagic presentation in 3 patients. Relief/prevention of ...

Medtronic (NYSE:MDT) said today it won expanded FDA approval for its Pipeline Flex embolization device.. The device is now indicated for patients with small or medium, wide-necked brain aneurysms in the territory from the petrous to the terminus of the internal carotid artery, the Fridley, Minn.-based company said.. The Pipeline Flex was previously cleared for the endovascular treatment of adults with large or giant wide-necked intracranial aneurysms from the petrous to the superior hypophyseal segments, Medtronic said.. Clearance came based on data from the 141-patient PREMIER trial of the device, which reported a one-year average occlusion rate of 76.7% with the use of 1.1 device per subject on average and an average 2.2% rate of major stroke or neurological death, the company said.. Premier is another landmark study with Pipeline and moves the bar on the safe treatment of wide-necked brain aneurysms. This data changes the way we, as physicians, think about using Pipeline Flex to treat our ...
OBJECT: Endovascular treatment of very small aneurysms poses a significant technical challenge for endovascular therapists. The authors review their experience with a series of patients who had intracranial aneurysms smaller than 3 mm in diameter. METHODS: Between 1995 and 2006, 97 very small aneurysms (defined for purposes of this study as | 3 mm in diameter) were diagnosed in 94 patients who were subsequently referred for endovascular treatment. All patients presented after subarachnoid hemorrhage, which was attributed to the very small aneurysms in 85 patients. The authors reviewed the endovascular treatment, the clinical and angiographic results of the embolization, and the complications. RESULTS: Five (5.2%) of the 97 endovascular procedures failed, and these patients underwent craniotomy and clip ligation. Of the 92 aneurysms successfully treated by coil embolization, 64 (69.6%) were completely occluded and 28 (30.4%) showed minor residual filling or neck remnants on the immediate postembolization

Introduction: Flow diverters (FDs) aim to treat intracranial aneurysms by altering intra-aneurysmal hemodynamics. Reports have suggested aneurysm and parent artery shape may affect flow reduction in FD-treatment. The purpose of this study is to gain insight into the way in which aneurysm shape and parent artery curvature influence the ability of FDs to redirect flow.. Hypothesis: Aneurysm dome size and parent artery curvature affect FD-induced flow reduction within an aneurysm.. Methods: FD models constructed based on the Pipeline Embolization Device with 35% area coverage, 30 um strand diameter, and 4 mm nominal diameter were implemented for hemodynamic simulation analysis. The flow reduction effects were tested using aneurysm models featuring different dome sizes and parent artery curvatures. Aneurysm blood flow was analyzed before and after FD stenting in regions of the aneurysm neck, body, and dome.. Results: We found that aneurysms with higher parent artery curvature had increased systole ...

Recent advances in endovascular techniques have led to increasing rates of anatomical cure of AVMs.1,5,9,11,16 Familiarization of practitioners with the properties of Onyx and contemporary evolution of endovascular technology have afforded safety for prolonged injection time of liquid embolics.1,8 Nevertheless, for accomplishing a successful AVM embolization, the close approach to the nidus remains the major determinant and must be pursued.11 It allows the embolic agent to diffuse into the vascular apparatus of the shunt, sparing normal brain supply branches.1,5,9,11,12,16. In some instances, especially in the deepest portion of the AVM, recurrent configuration and unfavorable angled loops at the origin of the feeders lead to unstable microcatheter navigation.7,9,15 Attempts to gain distal access result in prolapse of the system into the parent vessel.19 The struggling microcatheter navigation usually requires multiple attempts and excessive wire exchanges, leading to distal wire perforation in ...
Transcatheter closure of PDAs is rapidly becoming the treatment of choice at most centers in larger infants, children, and adults. Since the initial descriptions of PDA closure with Gianturco coils, there have been various techniques for coil delivery reported to achieve greater coil stability during closure (4-6). The most frequent complication using Gianturco coils has been coil embolization in up to 10% of cases (7). Detachable coils were created to address the lack of control during implantation and release of the Gianturco coils, with a decrease in reported embolization rates (7). Despite high success rates for ductal closure with coils, these devices were created for occlusion of other vascular structures and were not made to conform to conical shape of most PDAs (type A). Several devices have been developed specifically for PDA closure. The Nit-Occlud (PFM Medical, Carlsbad, California) coil occlusion system is composed of a stainless steel coil that, once released, assumes a biconical ...
