We conducted a retrospective cohort study (N = 477) to determine if use of intravenous tranexamic acid (TXA) improves early ambulation and reduces blood loss after total knee arthroplasty and anterior and posterior total hip arthroplasty. Mean (SD) patient age was 66.5 (10.1) years. For all 3 procedures, early ambulation was significantly better in the TXA group than in the no-TXA group at postoperative days 1 and 2, and, over time, reductions in hemoglobin and hematocrit were statistically significantly lower in the TXA group than in the no-TXA group. TXA groups required fewer postoperative transfusions. Incidence of postoperative venous thromboembolism was similar between all groups. TXA use improves early ambulation after total joint arthroplasty.

The general risk comes from the immobility of lying motionless on an operating table for a period of time (venous circulation in the legs depends much on muscular action, which milks blood north with the assist of one-way valves in the veins.) There was a time when such clots were an unrare complication of surgery; in fact, years ago I lost a couple of patients to it, despite doing everything we knew to do in those times. Now, because of several measures, the complication -- at least at a level where its noticeable -- is quite rare. Abdominal drainage after donor hepatectomy is a routine practice in most of the LDLT centres. But the routine abdominal drainage after donor hepatectomy is questioned as the drain increases the postoperative discomfort and also it hampers early ambulation of donor in postoperative period. At our centre we have done 142 liver transplant out of which 19 were DDLT whereas 123 were LDLT. We have done one dual lobe liver transplant. Retrospective study was performed in 124 donors. Drains were placed selectively with the surgeon deciding based on donor age, remnant volume, and personal preference. We have placed abdominal drain in 95 liver donors whereas abdominal drain was not placed in 29 donors. We defined early mobilisation as any early, active exercise during invasive ventilation, and it was not common in our cohort despite the fact that physiotherapy services were available to all ICU patients included in the study as part of standard ICU care. Introduction - Mobilisation of patients in the intensive care unit (ICU) is an area of growing research. Currently, there is little data on baseline mobilisation practises and the barriers to them for patients of all admission diagnoses. Methods - The objectives of the study were to (1) quantify and benchmark baseline levels of mobilisation in Australian and Scottish ICUs, (2) compare mobilisation practises between Australian and Scottish ICUs and (3) identify barriers to mobilisation in Australian and Scottish ICUs. We conducted a prospective, observational, cohort study with a 4-week inception period. Patients were censored for follow-up upon ICU discharge or after 28 days, whichever occurred first. Patients were included if they were ,18 years of age, admitted to an ICU and received mechanical ventilation in the ICU. Survival of critically unwell patients has improved in the last decade due to advances in critical care medicine. Some of these survivors develop cognitive, psychiatric and /or physical disability after treatment in intensive care unit (ICU), which is now recognized as post intensive care syndrome (PICS). Given the limited awareness about PICS in the medical faculty this aspect is often overlooked which may lead to reduced quality of life and cause a lot of suffering of these patients and their families. Efforts should be directed towards preventing PICS by minimizing sedation and early mobilization during ICU.All critical care survivors should be evaluated for PICS and those having signs and symptoms of it should be managed by a multidisciplinary team which includes critical care physician, neuro-psychiatrist, physiotherapist and respiratory therapist,with the use of pharmacological and non-apharmacological interventions.
