Dyspepsia, a poorly defined syndrome of episodic or recurrent upper abdominal discomfort after eating, has an estimated prevalence of 20% to 30% in the general population. The pathophysiology of this disorder is unknown. Postulated mechanisms include gastric acid hypersecretion, gastric motility disorder, Helicobacter pylori infection, and psychogenic stress. The investigation of dyspepsia is limited to patients with systemic signs or symptoms that suggest anatomic disease; otherwise, the treatment of dyspepsia is empiric. Results of previous randomized trials have generally failed to show any advantage of antacids or cimetidine over placebo (1), although a more recent trial found a benefit of cimetidine for relief of pain (2). Johannessen and colleagues compared on-demand cimetidine with placebo in treating the symptoms of dyspepsia. Their n-of-1 design addresses a limitation of previous studies: A subgroup of responders may have been lost in the random error associated with a majority of ...

Gastroparesis is defined by the presence of delayed gastric emptying (GE) in the absence of mechanical obstruction. Symptoms that have been attributed to gastroparesis include postprandial fullness, early satiation nausea and vomiting. Gastroprokinetic drugs are the preferred treatment option. A number of problems with the concept of gastroparesis have been identified recently. Major overlap exists with the symptom complex of the functional dyspepsia subtype of postprandial distress syndrome. The distinguishing feature of gastroparesis is delayed GE, but the correlation between delayed emptying and symptom pattern or severity in gastroparesis is modest and the stability of delayed emptying over time is poor. Other pathophysiological mechanisms such as hypersensitivity or impaired accommodation may also underlie symptoms in patients with gastroparesis. Moreover, symptomatic response to prokinetic therapy is variable and cannot be predicted based on the degree of enhancing GE. A number of ...

Upper gastrointestinal endoscopy is the most preferable diagnostic examination for patients over fifty when upper gastrointestinal symptoms appear. However, limited knowledge exists in concerns to the compliance of primary care patients to the doctors recommendations for endoscopy. Patients who visited primary care practices in Greece and experienced upper gastrointestinal symptoms within a 10 days screening study, were referred for an upper endoscopy exam. The patients which refused to complete the endoscopy exam, were interviewed by the use of an open- ended translated and validated questionnaire, the Identification of Dyspepsia in General Population (IDGP) questionnaire. A qualitative thematic analysis grounded on the theory of planned behavior was performed to reveal the reasons for patients refusal, while socio-demographic predictors were also assessed. Nine hundred and ninety two patients were recorded, 159 of them (16%) were found positive for dyspepsia and gastro-esophageal reflux disease
Introduction: One quarter of the population in the Western countries suffer from dyspepsia. Thus, if every patient with dyspepsia were to be endoscoped, then the cost to community and the individual would be substantial and the work load of the endoscopist would be large. Objectives We investigated whether using demographic characteristics and alarm symptoms can accurately predict cancer in patients with dyspepsia in Iran, where upper GI cancers and H. pylori infection are common. Methods All consecutive patients referred to a tertiary gastroenterology clinic in Tehran, Iran, from 2002 to 2009 were invited to participate in this study. Each patient completed a standard questionnaire and underwent upper gastrointestinal endoscopy. Alarm symptoms included in the questionnaire were weight loss, dysphagia, GI bleeding, and persistent vomiting. We used logistic regression models to estimate the diagnostic value of each variable in combination with other ones, and to develop a risk-prediction model. Results A total of 2,847 patients with dyspepsia participated in this study, of whom 87 (3.1%) had upper GI malignancy. Patients reporting at least one of the alarm symptoms constituted 66.7% of cancer patients compared to 38.9% in patients without cancer (p|0.001). Esophageal

In this study, the relation between dyspeptic complaints and six dimensions of functional health was assessed using a cross sectional survey of dyspeptic patients consulting their GP. Results showed that dyspepsia has a clear but limited influence on health status. Furthermore, psychological distress was shown to be a more powerful predictor of the health status of dyspeptic patients in general practice, especially in patients with higher levels of psychological distress. In general, infection withH pylori was not associated with severity of dyspeptic complaints or health status;H pylori prevalence was similar to that found in a group of asymptomatic Dutch labourers.26 The cross sectional design of the study limits interpretation of the results. Firstly, no conclusions can be drawn regarding the causal relation of any association found in this survey; and secondly, the findings can only be generalised towards dyspeptic patients consulting their GP. We were not able to include a control group of ...

2018, Springer Science+Business Media, LLC, part of Springer Nature. The construct of dysphoria has been described inconsistently across a broad range of psychopathology. The term has been used to refer to an irritable state of discontent, but is also thought to incorporate anger, resentment and nonspecific symptoms associated with anxiety and depression, such as tension and unhappiness. The Nepean Dysphoria Scale has been developed to allow assessment of dysphoria, but its factor structure has not yet been investigated in clinical samples. We aimed to determine the latent structure of dysphoria as reflected by the Nepean Dysphoria Scale, using a clinical sample. Adults (N = 206) seeking treatment at a range of mental health services were administered the Nepean Dysphoria Scale. Four putative factor structures were investigated using confirmatory factor analysis: a single-factor model, a hierarchical model, a bifactor model and a four-factor model as identified in previous studies. No model fit ...

There are several things you can do. Try to eat little and often. Milk may relieve acute indigestion pain and heartburn. Watch your posture and allow your stomach to expand while eating; this can be difficult if you are slouching or attempting to eat while lying down. Avoid eating highly spiced food if you arent used to it. Do not lie completely flat; use cushions/pillows to prop you up so that your top half is slightly tilted. You could try propping the head of your bed up by placing some books under the feet of the bed to achieve the same thing while you are asleep. This will help to prevent the acid coming out of your stomach into the oesphagus causing heartburn. If none of the above work, your GP or a doctor at the antenatal clinic may prescribe you some antacid that will neutralize the acidity of your stomach. ...
Your stomach is just one big processing plant: Incoming food is broken down into individual nutrients that are then absorbed into the body. The main food de-constructor for this process is stomach acid, a powerful fluid comparable in strength to battery acid. Alls well as long as it remains in the stomach-but should it escape, it can wreak havoc on other parts of the body, including teeth.. Thats the reality for 1 in 5 Americans with gastroesophageal reflux disease (GERD). Also known as acid reflux, GERD occurs when the ring of muscle at the base of the esophagus-which ordinarily keeps stomach acid contained-weakens to allow it into the esophagus. It can then irritate the esophageal lining, giving rise to the burning sensation of indigestion.. The scenario changes, however, if acid continues up into the mouth. This puts tooth enamel at risk for erosion. The resulting high acidity is enough to dissolve the mineral content of enamel, which could jeopardize the survival of affected ...

I am troubled by Laine and colleagues conclusion that there is little support for the use of H. pylori eradication therapy in patients with nonulcer dyspepsia (1). Since infected persons have a two- to sixfold increased risk for gastric cancer and certain types of lymphoma, isnt it reasonable to therefore offer curative therapy? Do we really have longitudinal studies of sufficient duration to know the actual long-term risks associated with continued H. pylori infection? In the same issue, the findings of Ohkusa and associates that precancerous gastric lesions improved after eradication of H. pylori(2) certainly support the premise of long-term benefit ...

Functional dyspepsia (FD), a gastrointestinal disorder characterized by pain or a burning sensation in the epigastrium, postprandial fullness, early satiety, bloating and nausea, used to be treated with prokinetics such as metoclopramide, cisapride and domperidone. Unfortunately, these drugs have severe side effects, including cardiac and sudden death, and consequently are now rarely used or have been withdrawn from the market. However, botanicals could be a source of natural prokinetics which do not have these adverse effects. Perilla frutescens leaf extract is a possible candidate, with some data indicating a clinical role in FD. As with any botanical, perilla extract normally consists of a complex mixture of active ingredients characterized by different chemical and physical properties, including varying degrees of solubility in water and/or alcohol. Consequently, selection of the correct solvents is very important for the preparation of standardized botanical extracts. We have therefore ...

Introduction and aims. Helicobacter pylori (H. pylori) colonization is a well-recognized risk factor for the development of gastric disorders in the human stomach,1-3 even though most infected persons will remain asymptomatic throughout their lives. The seroprevalence of H. pylori infection in Mexico was reported at 66% and age was the strongest risk factor for infection.4 According to the fourth Mexican consensus on H. pylori,1 the association between functional dyspepsia and H. pylori infection is controversial. Several studies have suggested that the genetic variability of H. pylori and the host,5 as well as environmental factors, determine clinical outcome.6H. pylori is genetically diverse and type I strains, which are cytotoxin-associated gene A-positive (cagA+) and secrete the vacuolating cytotoxin A (VacA), are the most virulent.7,8 They are associated with duodenal ulceration,9,10 abdominal pain, bleeding, active gastritis,11 symptoms of dyspepsia,12,13 DNA damage in the gastric ...

Treatment options are rather similar for heartburn and indigestion.. Antacids can be use when necessary to neutralise acid in the stomach when necessary. Examples of antacids include sodium bicarbonate, calcium carbonate, magnesium carbonate and aluminium hydroxide. Patients on a sodium-restricted diet, such as those with heart problems, should avoid sodium-containing antacids. Common side effects from antacids include bloating, diarrhoea from calcium and magnesium-containing antacids and constipation from aluminium-containing antacids. Because antacids can interfere with some medications, check with your pharmacist or doctor before starting treatment.. Domperidone is a medication known as a prokinetic. This review presents a critical evaluation of the role of Helicobacter pylori eradication in the management of peptic ulcer disease and non-ulcer dyspepsia. On current evidence, H. pylori eradication therapy seems likely to emerge as the most rational and cost-effective treatment for duodenal ulcer. The role of H. pylori eradication in the treatment of gastric ulcer and non-ulcer dyspepsia is unclear and requires further study. The emerging problem of antibiotic resistance in H. pylori is of major clinical importance and a prime cause of treatment failure. There is increasing evidence of a link between H. pylori and gastric cancer but it is premature to recommend large-scale eradication of H. pylori as a valid strategy for the primary prevention of gastric cancer. The search continues for the ideal H. pylori eradication regimen.. ...

AIM: To study the different endocrine cell types in the oxyntic mucosa of patients with irritable bowel syndrome (IBS).. METHODS: Seventy-six patients with IBS were included in the study (62 females and 14 males; mean age 32 years, range 18-55 years), of which 40 also fulfilled the Rome III criteria for functional dyspepsia (FDP). Of the entire IBS cohort, 26 had diarrhea as the predominant symptom (IBS-D), 21 had a mixture of diarrhea and constipation (IBS-M), and 29 had constipation as the predominant symptom (IBS-C). Forty-three age and sex-matched healthy volunteers without any gastrointestinal complaints served as controls. The patients were asked to complete the Birmingham IBS symptom questionnaire. Both the patients and controls underwent a standard gastroscopy, during which three biopsy samples were taken from the corpus. Sections from these biopsy samples were immunostained using the avidin-biotin complex (ABC) method, for ghrelin, serotonin, somatostatin and histamine. The densities of ...

Helicobacter pylori is the cause of most peptic ulcer disease and a primary risk factor for gastric cancer. Eradication of the organism results in ulcer healing and reduces the risk of ulcer recurrence and complications. Testing and treatment have no clear value in patients with documented nonulcer dyspepsia; however, a test-and-treat strategy is recommended but for patients with undifferentiated dyspepsia who have not undergone endoscopy. In the office setting, initial serology testing is practical and affordable, with endoscopy reserved for use in patients with alarm symptoms for ulcer complications or cancer, or those who do not respond to treatment. Treatment involves 10- to 14-day multidrug regimens including antibiotics and acid suppressants, combined with education about avoidance of other ulcer-causing factors and the need for close follow-up. Follow-up testing (i.e., urea breath or stool antigen test) is recommended for patients who do not respond to therapy or those with a history of ulcer
OBJECTIVE: To determine (a) the advantages and disadvantages of treatment options for the eradication of Helicobacter pylori and (b) whether eradication of H. pylori is indicated in patients with duodenal ulcer, nonucler dyspepsia and gastric cancer. Pancreatic rest in a young woman: a rare cause of dyspepsia is an article from Gastroenterology & Hepatology: Open Access for MedCrave Group. Ectopic pancreatic tissue or heterotopic pancreatic tissue, also known as a pancreatic rest, is an uncommon congenital anomaly defined as displacement of small amount of pancreas during embryonic development, resulting in the formation of a nodule located far from the pancreas and without any connection via vascular or anatomical means. It can be seen in any part of the gastrointestinal tract with larger percentages found in stomach followed by small intestine especially in duodenum and jejunum. It may remain silent over period of years or present clinically mimicking symptoms of acid peptic disease, ulcer, upper gastrointestinal tract bleeding and obstruction. Infrequently, it may lead to pancreatitis or pancreatic cancer. Radiological appearances are variable ranging from small lesion to large masses. Histopathological diagnosis remains the gold standard.
