Dural tears usually occur in re-operation where the dura is adherent to perineural and peridural scar tissue. This situation as well as dura adherent to periosteum is usually evident on adequate pre-operative MRI scans.. Dural tears are less likely to occur in a primary operative (virgin spine) procedure. They may actually exist prior to surgery where a freely sequestered fragment of disc material has torn a nerve axilla and produced its own tamponade. In this circumstance the removal of the fragment may remove the tamponade and spinal fluid leakage is then seen. Abstract Polymorphism at codon 219 lysine in prion protein (PrP) is considered to affect the clinicopathological features of prion diseases including Creutzfeldt-Jakob disease (CJD) and to have an inhibiting effect on the pathogenesis of these diseases. We describe the first autopsied case of dura mater graft-associated CJD (dCJD) with heterozygosity of lysine at codon 219 in PrP observed in a Japanese subject. Although this case demonstrated the non-plaque type of dCJD and MM1 subgroup of CJD pathologically and biochemically, the patient demonstrated a long incubation period (19.3years), atypical periodic sharp-wave complexes with a dominant rhythm on EEG, partially scattered small deposits of plaque-like PrP along with synaptic type deposits of PrP on immunohistochemistry and an atypical MM1 glycosylation pattern with a relatively increased diglycosylated isoform of proteinase-resistant PrP on western blot analysis (i.e. MM1 variant pattern). Background: A complication of a silastic dural substitute is described, which appeared after 32 years-by far the longest latency period reported in the literature. Methods: Case report and literature review. Results: In 1971, a 20-year old woman suffered from an acute subdural haematoma and a temporal cerebral contusion due to a motorbike accident. She underwent an operation with evacuation of these and the dura was mended with a silastic duraplasty. Thirty-two years later she deteriorated with increased memory problems and dysphasia. CT revealed an expanding haemorrhagic mass around the previous duraplasty, which demanded surgery with removal of the silastic dural implant and evacuation of the haemorrhagic mass. Although the haemorrhagic mass enveloped the silastic implant, a contribution of the acrylate flap cannot be ruled out. Bacteriological cultures revealed Acinetobacter spp. in the CSF. Lisa gave birth to her triplet daughters Alice, Caitlin and Grace at Mater Mothers Hospital in March 2011 at 33 weeks and five days.. At the time, the thought of having three babies at once was daunting for Lisa and husband Tim, but as the girls start Prep this week, 2011 seems a long time ago.. Lisa says she cant thank the hospital enough for the care she and her three beautiful daughters received.. We had a great experience at the Mater who helped us every step of the way. Having multiples is a stressful experience and the staff helped us to feel confident as new time parents.. This study was performed to determine the clinical significance of diffuse dural enhancement (DDE) detected by magnetic resonance (MR) imaging and to typify enhancing patterns related to inflammatory or metastatic causes. The authors retrospectively evaluated the clinical, imaging, and laboratory characteristics of 20 consecutive patients with DDE. Those with DDE and an underlying neoplastic disease (13 patients) were compared to 11 consecutive patients with cytological evidence of neoplastic leptomeningeal metastasis evaluated by MR imaging. The DDE was often associated with an underlying malignancy (13 (65%) of 20 patients) but it coexisted with leptomeningeal metastasis in only one patient. Skull metastases were evident in 10 (77%) of 13 patients and cranial nerve palsies in six (46%) of 13. One of the major challenges in neural engineering is the tissue damage that occurs during insertion of microelectrodes into the central nervous system. The damage occurs as a result of the dimpling effect that is due to the insertion force. A method which can reduce the penetration force can also reduce dimpling and the resulting tissue damage.
There are two objectives in this thesis. One is to measure the penetration force with Michigan electrodes, which are silicon based electrodes. The second is to reduce the penetration force by using mechanical vibrations.
First, the penetration force was measured in the rat brain. To measure the penetration force, the microelectrode was connected to a load transducer. Because dura is a tough membrane to penetrate, it is usually removed during surgery in experimental animals.
