CRE, or Carbapenem-Resistant Enterobacteriaceae, is a family of superbug bacteria that have recently evolved a resistance to nearly all antibiotics. An estimated 50% of infections are fatal.. In February 2015, at least 179 people were exposed to CRE after undergoing ERCP duodenoscope procedures at a hospital in Los Angeles. Seven patients developed infections and two patients died. Meanwhile, at least 3 people were infected with CRE and one person died in North Carolina.. In January 2015, officials at Virginia Mason Medical Center in Seattle reported that 35 patients were exposed to CRE after undergoing duodenoscope prodceures in 2012 and 2014. At least 11 patients died after developing infections, but it is not clear that the infection caused all of the deaths.. In mid-2013, an outbreak of CRE at Advocate Lutheran General Hospital in Chicago, Illinois, infected at least 44 patients. Of those patients, 38 had recently undergone an endoscopic procedure. ...

Identifying patients with Crohn s disease with rapid disease progress or high risk of early surgery is crucial to clinical decision making.. Dr Wei Liu and colleagues evaluated the correlation between the L mann index at diagnosis and abdominal surgery in the first year after Crohn s disease diagnosis and to find the risk factors for early surgery.. The team performed a retrospective cohort study at a single tertiary hospital.. Patients diagnosed with Crohn s disease between 2013 and 2015 were included.. The researchers main outcome of interest was the need for an abdominal surgery within 1 year after the L mann index evaluation at diagnosis.. Of 212 eligible patients, the team noted that 48 patients underwent abdominal surgery during follow-up. L mann index was much higher in the surgery group. On tertiles of the L mann index, the researchers observed that frequency of surgery grew as the L mann index increased. ...

Glitazones have been widely used by people with diabetes for more than 10 years. If they do have a beneficial effect in Parkinson s disease we expect that some of the people who have been taking them regularly will have avoided developing signs of Parkinson s disease as a beneficial side effect of the medication. The Clinical Practice Research Datalink is a rich source of data based on electronic health records for more than 11 million United Kingdom citizens and it contains high-quality information on the drugs people have been prescribed and the illnesses they have had. We will use these data to see if people taking glitazones are diagnosed with Parkinson s disease less often than people taking other treatments for diabetes. We will also see if any effect of glitazones changes over long periods of treatment.. Relevance to Diagnosis/Treatment of Parkinson s Disease ...

With colleagues Mark Mintun, M.D., professor of radiology, and David Holtzman, M.D., the Andrew B. and Gretchen P. Jones Professor and head of the Department of Neurology, Fagan studied a group of 24 people that included individuals diagnosed with very mild and mild Alzheimer s disease, and cognitively normal subjects. As expected, in patients with cognitive impairments, believed to be attributable to Alzheimer s disease, researchers found low CSF levels of amyloid beta 42 (A-beta 42), the principal ingredient of the brain plaques that are characteristic of Alzheimer s disease. In the same individuals, brain scans with a new imaging agent that reveals the presence of amyloid plaques in the brain were positive. What scientists didn t anticipate was that three cognitively normal subjects would have both low CSF levels of A-beta 42 and positive results from the brain scans. Fagan stressed that although this aspect of their findings was very intriguing, it doesn t prove that the three normal ...
The team observed that from 48% to 88% of children have experienced at least one corticosteroid course irrespective of the period of diagnosis, and up to 33% became steroid dependent. The researchers found that immunosuppressive preparations were used earlier, and more frequently in newer than older cohorts.. The team noted that more than 33% of children have received biological treatment early in the disease course. A decline in the surgery rate might be observed in more recent compared with older unselected populations. The team found that the relative risk of cancer in childhood-onset Crohn s disease as well as the risk of death seem to have increased. Dr Duricovas team concludes, Childhood-onset Crohn s disease seems to be an aggressive phenotype of the disease.. Compared with older cohorts, a trend towards decreasing surgical rate can be observed in newer cohorts paralleled by an increase in immunomodulator use and biologicals.. Nevertheless, the causative role has yet to be ...
Crohns Disease is an inflammatory disease of primarily the small intestine and colon.. In recent years scientists have hypothesized that low blood vitamin-D levels play a role in the development of autoimmune diseases, including Crohns disease. The purpose of this study is to examine whether vitamin-D is effective in keeping remission in inactive Crohns Disease (double blind placebo controlled trial). The patients who have active disease in the placebo group (reach primary endpoint), will in an open setting receive a higher single dose vitamin-D and the effect will be measured within 4 weeks. ...
Aim of this trial is to investigate the expression and localisation of different amino acid transporters and their regulatory proteins derived from the regulatory proteins of the local renin angiotensin system (RAS) in the intestine. This is investigated on one hand in patients who do not take any drugs interfering with RAS. On the other hand expression analysis is done in patients taking daily RAS-active drugs, like ACE inhibitors or sartanes.. After obtaining informed consent of patients attending the hospital for clarification of gastrointestinal symptoms by gastroduodenoscopy or colonoscopy, 2 biopsies (in addition to biopsies needed for clinical diagnostics) will be taken from each duodenum, jejunum, ileum and descending colon.. Biopsies are investigated anonymously at the Institute of Physiology of the University of Zurich. The mRNA content of amino acid transporters and regulatory proteins, respectively, in the biopsies is analyzed by quantitative PCR. Transport proteins are in addition ...

Alzheimer´s disease is the most common form of dementia, predominantly in the age group of 65-85 years. There are currently 48 million known patients with Alzheimer´s disease worldwide. This number is expected to double by 2050.. Our newest work, which was done in a collaboration with Dr. Gert von Helden from the Fritz Haber Institute of the Max Planck Society, was published in the current issue of Nature Chemistry and makes a major step forward in analyzing the biochemical causes of Alzheimer´s and Parkinson´s disease. The results could enable the development of new medications fighting the causes of both diseases.. The causes of neurodegenerative diseases like Parkinson´s and Alzheimer´s disease are not fully understood to this day. In both cases, a biochemical disorder can be observed during the course of the disease, but the exact mechanism is unknown. Water-soluble proteins undergo a spontaneous transition into insoluble amyloid fibrils. In the case of Alzheimer´s disease, protein ...
March 5, 2015 - Cedars-Sinai Medical Center in Los Angeles has reported that four patients were infected with an antibiotic-resistant superbug after undergoing procedures with a duodenoscope that is very difficult to sterilize.. Cedars said 67 other patients may have been exposed, including 1 patient who died. They are still investigating whether the infection contributed to the death, according to the LA Times.. Patients may have been exposed from one Olympus duodenoscope in use from August 2014 to mid-February, according to the hospital.. The announcement came just two weeks after UCLAs Ronald Reagan Medical Center reported that the same Olympus scope was responsible for exposing 179 patients to the bacteria, including seven who were infected and two who died.. Another hospital, Virginia Mason Medical Center in Seattle, blamed the scope on over 30 infections and at least 11 deaths since 2012.. Earlier this week, the FDA reported that the scope had been on the market since 2010 without ...

Parkinson s disease occurs both sporadically and as a result of inheritance of single gene mutations. One of the most common neurodegenerative disorders, it is associated with the progressive and selective loss of a specific population of neurons in the brain, the dopaminergic neurons of the substantia nigra pars compacta . Exposure to several common environmental toxins, thought to injure neurons through oxidative damage, has been shown to be associated with sporadic forms of Parkinson s disease. During the past decade, researchers have also made remarkable progress in identifying genes responsible for inherited forms of Parkinson s disease, with the expectation that understanding the function of these genes will elucidate mechanisms behind sporadic Parkinson s disease. Past work had shown that one form of familial Parkinson s disease results from a loss of function of a gene called DJ-1 ...
TY - CHAP. T1 - Endoscopic diagnosis and resection therapy of the Tumor of the major duodenal papilla. AU - Itoh, Akihiro. AU - Hirooka, Yoshiki. AU - Goto, Hidemi. PY - 2008/12/1. Y1 - 2008/12/1. N2 - Endoscopic ultrasonography and intraductal ultrasonography (IDUS) are useful for diagnosing tumor extension of the major duodenal papilla. Especially, IDUS makes it possible to diagnose early cancer that has never been correctly diagnosed by other modalities. On the other hand, we think that endoscopic resection therapy of the tumor that is benign adenoma or early cancer may be initially selected as a curative treatment similar to, endoscopic mucosal resection for early gastric or colonic cancer. Between 1993 and 2007, 91 patients with tumor of the duodenal major papilla underwent, endoscopic resection therapy at our institute. The indication for this therapy was defined as a case with adenoma or early cancer without infiltration into the bile and pancreatic ducts. The tumors were resected in a ...
The first thing to do in young people is to research the Helicobacter Pylori. The bacterium can be tested by faecal, blood or breath tests (urea Breath Test). The breath test is performed by making the patient drink a particular solution containing a marked urea with a carbon isotope (non-radioactive), therefore the patient must breathe inside a machine or in a tube. The bacterium is able to split the urea into water and carbon dioxide, if in the patients exhaled there is the presence of carbon dioxide marked means that there is infection with Helicobacter Pylori.. For those symptomatic subjects who do not respond to therapy and are more than 45 years old, an esophago-gastro-duodenoscopy or EGDS is recommended. In these cases, in fact, the same symptoms of gastritis could be the expression of other and much more serious diseases, including tumors. To treat gastritis, it is advisable to reduce stomach acidity with Anti-H2 drugs (ranitidine type) or proton pump inhibitors (omeprazole type), or ...

Pain management in patients with Parkinson's disease: challenges and solutions Orjan Skogar,1,2 Johan Lokk2 1Academy for Health and Care (FUTURUM), Region Jönköping County, Jönköping, 2Department of Neurobiology, Care Sciences and Society, Karolinska Institutet, Huddinge, Stockholm, Sweden Abstract: This review focuses on the diagnosis and management of Parkinson-related pain which is one of the more frequently reported nonmotor symptoms in Parkinson’s disease (PD), which is the second most common neurodegenerative disease after Alzheimer’s disease. Pain is ranked high by patients as a troublesome symptom in all stages of the disease. In early-stage PD, pain is rated as the most bothersome symptom. Knowledge of the correct diagnosis of pain origin and possible methods of treatments for pain relief in PD is of great importance. The symptoms have a great negative impact on health-related quality of life. Separating PD-related pain from pain of other origins is an
I︉ have been with my husband for almost 12 years and we have been married for 6 years. He did not have any symptoms for the first 2 years we were dating but even before that he often complained about being in pain. So essentially the entire time we have been together he has had physical issues. I︉ have tried to be a good companion throughout this journey but it is so hard on me sometimes. He had his ileostomy in place for 7 years and had his colon removed last year. We had many complications with this last year and he was in and out of the hospital for the entire year. He has now been out of work for 15 months (which I︉ support because he is definitely not in a state to hold down a job.) but it is draining. I︉ work a demanding full time job and it is hard to come home everyday to someone who has spent their whole day watching tv. I︉ literally do all the cooking, cleaning and bill paying and try to keep on a smiling face but I︉ am nearing the end of sanity. I︉ wanted to have ...

Neoplastic PTLD. Case 4.. Posttransplant Hodgkins Disease This young male received a liver transplant for relief of sclerosing cholangitis. His underlying disease was a complication of Crohn s disease which had been diagnosed 8 years earlier. His family history was positive for ulcerative colitis, Hodgkin s disease, leukemia, and diabetes in close family members. Following uncomplicated liver transplant, he was discharged under Cyclosporine/steroid immunosuppression. His only complication was a tooth abscess two months posttransplant. Five months after transplant he complained of throbbing flank pain, and developed fever to 103F without chills, anorexia and fatigue. Ultrasonography revealed paraaortic nodes in the distal aorta and pelvis, with no adenopathy elsewhere. Urine was positive for CMV. A lymph node biopsy was consistent with mixed cellularity Hodgkin s disease. A trial of reduced imunosuppression was initiated. The patient returned to his home state and was apparently well for another ...

The bacterial community (microbiota) that inhabits the gut of humans appears to be an important source of antigens that drive the chronic immunological processes characteristic of Crohn's disease (CD) and ulcerative colitis (UC). Most of the members of the microbiota have not yet been cultured, but nucleic-acid-based methods of detection and enumeration can provide information about the community. This investigation used these methods to obtain information about the bacteria associated with mucosal surfaces in the gut of 20 CD and 15 UC patients. Biopsies were collected from inflamed and non-inflamed sites in the intestines of newly diagnosed, untreated patients. Biopsies were also collected from several intestinal sites of 14 healthy subjects. The bacterial collections associated with the biopsies were analysed by generating PCR/denaturing gradient gel electrophoresis (DGGE) profiles, the preparation of 16S rRNA gene clone libraries, and qualitative PCR to detect specific groups of bacteria. Summary The production, purification and properties of the A antigen induced by Marek's disease virus in tissue culture were investigated. It was found that although virus multiplied on passage in chick kidney, chick embryo and duck embryo cells the antigen was produced only in chick kidney and duck embryo cultures. The results suggested that the A antigen produced by duck embryo cells was a glycoprotein having a mol. wt. in the range 70 000 to 90 000 and was heterogeneous in charge (pI 4.5 to 5.5). A 20-fold purification was achieved by electrophoresis in 5% acrylamide gel with a recovery of 45%. An equally good purification was also possible by chromatography on DEAE-Sephadex A50 and Sephadex G200, although the recovery was only 20% in this case.

Crohns disease (CD) is a heterogeneous disorder characterized by diverse clinical phenotypes. Childhood-onset CD has been described as a more aggressive phenotype. Genetic and immune factors may influence disease phenotype and clinical course. We examined the association of immune responses to microbial antigens with disease behavior and prospectively determined the influence of immune reactivity on disease progression in pediatric CD patients.Sera were collected from 196 pediatric CD cases and tested for immune responses: anti-I2, anti-outer membrane protein C (anti-OmpC), anti-CBir1 flagellin (anti-CBir1), and anti-Saccharomyces-cerevisiae (ASCA) using ELISA. Associations between immune responses and clinical phenotype were evaluated.Fifty-eight patients (28%) developed internal penetrating and/or stricturing (IP/S) disease after a median follow-up of 18 months. Both anti-OmpC (p , 0.0006) and anti-I2 (p , 0.003) were associated with IP/S disease. The frequency of IP/S disease increased with ...
