Although primary duodenal adenocarcinoma (DA) is a rare malignancy representing ~ 0.5% of all gastrointestinal cancers, the incidence of DA is more frequent in Lynch syndrome. Because of its rarity, treatment strategies or optimal chemotherapeutic regimens have not been clearly defined for advanced DA. A 72-year-old woman with Lynch syndrome visited our hospital with a right upper abdominal pain. Computed tomography (CT) showed wall thickness with enhancement in the second portion of the duodenum and adjacent abdominal wall, which suggested direct tumor invasion to the abdominal wall. Upper gastrointestinal endoscopy (UGE) showed a large ulcerative tumor in the second portion of the duodenum, and histological analysis revealed a poorly differentiated adenocarcinoma. A cT4N0M0, cStage IIB (Union for International Control Cancer TNM staging) DA was diagnosed. After three courses of chemotherapy with S-1 and oxaliplatin (SOX), follow-up CT and UGE showed shrinkage of the duodenal tumor. Therefore, the

Division of hepatobiliary and pancreas, Department of Surgery, Yonsei University College of Medicine, Seoul, Korea.. Introduction: Although organ-preserving operation for benign and borderline duodenal tumors has been regarded as reasonable strategy, laparoscopic pancreas-sparing subtotal duodenectomy(LapPSSD) was rarely reported. The aim of this study was to compare the perioperative surgical outcomes laparoscopic approach with open surgery.. Method: Between March 2010 and June 2013, 7 consecutive LapPSSD for duodenal gastrointestinal stromal tumor (GIST) were performed. The demographics and surgical outcomes of LapPSSD were compared with the dababase of 7 patients who underwent open PSSD during same period. Subtotal duodenectomy was defined as distal duodenal resection of ampulla of Vater to first portion of jejunum. All patients underwent side-to-side duodenojejunostomy after ...
This will be a single-center, phase-II, six-month-long, placebo-controlled, double blinded, randomized trial of the epidermal growth factor receptor (EGFR) inhibitor, erlotinib (Tarceva) and the cyclooxygenase (COX-2) inhibitor, sulindac in patients with familial adenomatous polyposis (FAP) or attenuated FAP. FAP is an autosomal dominant inherited colon cancer predisposition with a 100% risk of colon cancer in the absence of preventive care (endoscopy and surgery). Efficacious chemoprevention for duodenal adenomas is an unmet clinical need in FAP patients that would reduce the morbidity from duodenectomy and risk of duodenal adenocarcinoma. Currently the only Food and Drug Administration (FDA)-approved chemopreventive agent is celecoxib which results in a modest reduction of duodenal and colorectal polyps and is associated with cardiac toxicity at effective doses. If it can be shown that combinatorial inhibition of COX-2 and EGFR activity leads to successful regression in duodenal adenomatous ...
Duodenal cancer is a type of cancer in the small intestine. It is something like gastric cancer and colorectal cancer. Familial adenomatous polyposis (FAP), Gardner Syndrome, Lynch Syndrome, Muir-Torre syndrome, Celiac sprue, Puetz-Jeghers, Crohns Disease and Juvenile Polyposis Syndrome are risk factors for having this cancer.[1] The duodenum is the first part of the small intestine. It is located between the stomach and the jejunum. After foods combine with stomach acid, they go down into the duodenum where they mix with bile from the gall bladder and digestive juices from the pancreas. ...

Myeloid Sarcoma (MS), a rare extra hematopoietic carcinoma composed of blast cells, is located primarily in extramedullary sites such as skin, soft tissue, lymph nodes, and bone. MS usually presents in the setting of coexisting acute myeloid leukemia (AML) and myeloproliferative disorders. Gastrointestinal involvement (GI) is extremely rare from nonspecific abdominal symptoms to obstruction. Eight cases of myeloid sarcoma involving the duodenum including the current case have been reported, overall mean age being 40 years (range 17–71) and M : F ratio 7 : 1. The prognosis of patients with |i|de novo|/i| MS cases has been reported to be better than those who have a coexisting leukemia. MS is a rare extramedullary tumor, which should be considered in the differential diagnosis of a soft tissue mass involving the duodenum, especially if there is a coexisting hematological disorder. |i|De novo|/i| cases often progress to AML, and current therapy involves Daunorubicin- and

A technique to read a stored state in a magnetoresistive random access memory (MRAM) device, such as a giant magneto-resistance (GMR) MRAM device or a tunneling magneto-resistance (TMR) device uses a bit line in an MRAM device that is segmented into a first portion and a second portion. An interface circuit compares the resistance of a first portion and a second portion of a first bit line to the resistance of a first portion and a second portion of a second bit line to determine the logical state of a cell in the first bit line. The interface circuit includes a reset circuit that selectively couples the outputs of the interface circuit together. A subsequent decoupling of the outputs allows cross-coupling within the interface circuit to latch the outputs to a logical state corresponding to the stored magnetic state, thereby allowing the stored state of a cell to be read.

Periampullary cancers encompass a mixture of cancers but in general are separated into four subtypes: cancer in the head of the pancreas, distal bile duct cancer, true ampullary cancer, and duodenal cancer. These cancers arise in the vicinity of the ampulla of Vater and are differentiated by their histologic origins (pancreatic, distal bile duct, ampulla of Vater, or duodenum). While pancreatic adenocarcinoma makes up the majority of resected periampullary cancers at 62%, ampullary cancer accounts for 19%, distal bile duct cancer 12%, and duodenal cancer 7% of resected periampullary cancers.1 Although preoperative assessment with imaging and biopsy can distinguish one subtype from the other, often times the tumor origin may be undetermined preoperatively. Moreover, duodenal cancer in the periampullary region as well as intestinal-type ampullary cancer behave in a similar fashion, whereas distal bile duct cancer and pancreaticobiliary-type ampullary cancer behave similar to one another. While 56% ...
The male predominance of the two main histologic malignancies of the small bowel cancer may reflect a role of sex hormones which will be examined in this study. This was a nationwide population-based nested case-control study, based on a cohort of subjects born between 1932 and 2008, as identified in the Swedish Multi-Generation Register. For each case of small bowel cancer, 10 age- and sex-matched controls were randomly selected. Number of children and age at having the first child were analyzed in relation to the risk of small bowel cancer using conditional logistic regression, providing odds ratios (ORs) and 95 % confidence intervals (CIs). A total of 632 female cases and 894 male cases of small bowel cancer were included. No overall increased risk of small bowel cancer was found in parous compared to non-parous women (OR = 1.02, 95 % CI 0.67-1.54). There was no association between age at first birth and small bowel cancer (, 30 years of age vs , 20 years; OR = 1.04, 95 % CI 0.72-1.50). No ...

Tamoxifen treatment has previously been reported to confer life-prolonging effects in patients with advanced pancreatic cancer, and most evidently so in women. None of these trials did however include biomarkers, and the relevance of female hormone signaling in pancreatic or other periampullary adenocarcinoma remains largely unexplored. The aim of this study was to examine the extent and potential clinical significance of estrogen receptor-α (ER) and progesterone receptor (PR) expression in pancreatic and other periampullary cancers. ER and PR expression was examined using immunohistochemistry on tissue microarrays with primary tumors from a retrospective consecutive cohort of 175 patients with resected periampullary adenocarcinoma, with long-term clinical follow-up. Non-parametric and Chi square tests were applied to examine the associations of stromal ER and PR expression with patient and tumor characteristics. Kaplan-Meier analysis and log rank test were applied to illustrate survival differences in
Abstract. Thirteen-week and 2-year drinking water studies conducted by the National Toxicology Program (NTP) reported that hexavalent chromium (Cr(VI)) induced diffuse epithelial hyperplasia in the duodenum of B6C3F1 mice but not F344 rats. In the 2-year study, Cr(VI) exposure was additionally associated with duodenal adenomas and carcinomas in mice only. Subsequent 13-week Cr(VI) studies conducted by another group demonstrated non-neoplastic duodenal lesions in B6C3F1 mice similar to those of the NTP study as well as mild duodenal hyperplasia in F344 rats. Because intestinal lesions in mice are the basis for proposed safety standards for Cr(VI), and the histopathology data are relevant to the mode of action, consistency (an important Hill criterion for causality) was assessed across the aforementioned studies. Two veterinary pathologists applied uniform diagnostic criteria to the duodenal lesions in rats and mice from the 4 repeated-dose studies. Comparable non-neoplastic intestinal lesions ...

When cancer cells break away from the primary (original) tumor and travel through the lymph or blood to other places in the body, another (secondary) tumor may form. This process is called metastasis. The secondary (metastatic) tumor is the same type of cancer as the primary tumor. For example, if breast cancer spreads to the bones, the cancer cells in the bones are actually breast cancer cells. The disease is metastatic breast cancer, not bone cancer.. Small bowel cancer is grouped according to whether or not the tumor can be completely removed by surgery.. Treatment depends on whether the tumor can be removed by surgery and if the cancer is being treated as a primary tumor or is metastatic cancer.. ...
