In July of 2015, she went through what we thought was the worst episode yet. She was admitted to the hospital and by the 5th day test results showed that she was suffering from pancreatitis and complete intestinal blockage caused by an unknown cystic mass next to the her duodenum (first part of the small intestine). It was during surgery one week later that they discovered that she had a duodenal duplication cyst, a very rare congenital anomaly. They were able to partially excise the cyst, avoiding a more complicated surgery. It would take another two weeks for her to heal. The arts and crafts box [Bedside Adventure Kit] that Discovery Arts left for her kept her busy and helped take her mind off any pain or discomfort she was having. A case of pedunculated Brunners gland hamartoma (BGH) of the duodenum causing upper gastrointestinal (GI) hemorrhage is reported. The patient was a 47-year-old man who visited our hospital for further evaluation of tarry stools and shortness of breath. Endoscopic examination of the upper digestive tract revealed a large peduncular polyp with bleeding, about 30 mm in diameter, arising from the wall of the second portion of the duodenum. GI bleeding occurred from the base of the stalk of the polyp. Endoscopic polypectomy was performed. Histological examination of the specimen revealed that the main body of the polyp contained several lobules of mature Brunners gland with areas of cystic dilatation. The surface epithelium consisted of normal duodenal mucosa with areas of focal ulceration. This polyp was diagnosed as a BGH. The symptom of tarry stools resolved after endoscopic resection. Intravesical instillation of Bacillus Calmette-Guérin (BCG) is the treatment of choice for superficial bladder carcinoma. Complications of BCG therapy include local infections and disseminated BCG infection with multiple endorgan complications. We report a case of disseminated, post-treatment BCG infection that initially presented with granulomatous hepatitis and choroiditis. After successful anti-mycobacterial therapy and resolution of the hepatic and ocular abnormalities, the patient developed an acute upper gastrointestinal hemorrhage from an aortoduodenal fistula that required emergency surgery. The resection specimen revealed multifocal, non-caseating granulomas, indicating mycobacterial involvement. This case highlights the varied end organ complications of disseminated BCG infection, and the need for vigilance even in immuno-competent patients with a history of intravesical BCG treatment.

We report a case of right paraduodenal hernia in an adult patient and its emergency diagnosis and management in an acute clinical presentation. A twenty-eight-year-old male patient was admitted in our outside hospitals emergency room after 12 hours of evolution of diffuse abdominal pain, nausea and vomiting. He referred a similar episode several months ago that ceased spontaneously. An abdominal CT scan with intravenous contrast demonstrated an encapsulated cluster of small bowel loops occupying mainly the right upper quadrant, lateral to the duodenum, suspicious for an internal hernia in the context of an intestinal malrotation. The patient underwent a laparotomy, which revealed a large sac containing dilated small bowel loops as shown by radiologic studies. The patient did well in postoperatory and was discharged home on the fourth day after the surgery. In a year follow up the patient remained asymptomatic.
