Pioneering health charity GAFFI is today calling on the World Health Organisation (WHO) to help hundreds of thousand of AIDS and HIV positive patients worldwide by including itraconazole on the Essential Medicine List (EML).. GAFFIs application to the WHO, in collaboration with the International Foundation for Dermatology, pinpoints key fungal diseases in AIDS for which itraconazole is crucial. Itraconazole suspension is ~70% effective for fluconazole resistant oral thrush, and is the treatment of choice for eosinophilic folliculitis, a debilitating, itchy rash associated with HIV infection. Patients with Talaromyces marneffei infection (previously called penicilliosis) and common in SE Asia, also respond really well to itraconazole, as do those with coccidioidomycosis and paracoccidioidomycosis in the Americas. Numerous skin fungal infections in adults and children with HIV infection are not adequately treated with the drug, griseofulvin, which remains on the EML.. Dr David Denning, President ...
The WHO Model List of Essential Medicines (EML), published by the World Health Organization (WHO), contains the medications considered to be most effective and safe to meet the most important needs in a health system.[1] The list is frequently used by countries to help develop their own local lists of essential medicine.[1] As of 2016[update], more than 155 countries have created national lists of essential medicines based on the World Health Organizations model list.[2] This includes countries in both the developed and developing world.[1][3] The list is divided into core items and complementary items.[4] The core items are deemed to be the most cost-effective options for key health problems and are usable with little additional health care resources.[4] The complementary items either require additional infrastructure such as specially trained health care providers or diagnostic equipment or have a lower cost-benefit ratio.[4] About 25% of items are in the complementary list.[5] Some ...
For various historical stop-gap reasons, development partners have taken the lead in delivering life-saving commodities in the different countries. In many cases, development partners (mainly UNICEF) are responsible for all or parts of the supply chain for nutrition products. Nutrition products are therefore often seen by government health staff at all levels as external products. Integration of the nutrition products supply chain in the regular supply chain is an important contribution toward normalisation of these products. A key element for normalisation of nutrition products is inclusion on the National Essential Medicine List, which is seen as an important driver for inclusion in other SCM elements.. Whether focusing on optimisation or integration, predictability and availability of funds necessary for correct execution of the different SCM elements continues to be a major obstacle in all assessed countries.. De facto integration has often taken place downstream in the supply chain ...
For example, all women giving birth should receive oxytocin. Oxytocin is recommended by WHO as a priority medicine against postpartum haemorrhage, the leading cause of maternal death. Children should be treated with antibiotics for pneumonia, and zinc and oral rehydration solution for diarrhoea. WHO has published a list of priority medicines for mothers and children that includes these drugs and promotes them through policy statements and clinical guidelines. The problem is not simply one of cost. Many of these products are relatively cheap. Zinc and oral rehydration solutions for diarrhoea cost less than $US 0.50 per treatment dose. Antibiotics such as amoxicillin for pneumonia cost about $US 0.30 per treatment dose. The major obstacles are inefficient procurement and supply systems, poor partner collaboration, and lack of crucial reproductive, maternal, newborn and child health commodities on national essential medicines lists. Solving bottlenecks in these areas would radically help country ...

The EMLc Subcommittee did not endorse the inclusion of this fixed-dose combination for the treatment of tuberculosis on the EMLc. The various fixed-dose combinations were considered as a group, as there is no clinical evidence for any of these combinations in children. However, it is difficult to determine the necessary appropriate combinations and what the strengths of the components should be in FDCs for use in children without examining further data (including pharmacokinetic data, stratified by weight and age). The Subcommittee therefore decided to endorse the lower strength rifampicin + isoniazid combinations (60 mg + 30 mg and 60 mg + 60 mg) and rifampicin + isoniazid + pyrazinamide (60 mg + 30 mg + 150 mg) as probably useful for many children, but requested an urgent review of all clinical evidence to support these and other potential combinations ...

Downloadable! On April 1, 2005, Denmark changed the way references prices, a main determinant of reimbursements for pharmaceutical purchases, are calculated. The previous reference prices, which were based on average EU prices, were substituted to minimum domestic prices. Novel to the literature, we estimate the joint effects of this reform on prices and quantities. Prices decreased more than 26 percent due to the reform, which reduced patient and government expenditures by 3.0 percent and 5.6 percent, respectively, and producer revenues by 5.0 percent. The prices of expensive products decreased more than their cheaper counterparts, resulting in large differences in patient benefits from the reform.

Health is a fundamental human right. Access to health care, which includes access to essential drugs, is a prerequisite for realizing that right. Essential drugs play a crucial role in many aspects of health care. If available, affordable, of good quality and properly used, drugs can offer a simple, cost-effective answer to many health problems. In many countries drug costs account for a large share of the total health budget. Despite the obvious medical and economic importance of drugs there are still widespread problems with lack of access, poor quality, irrational use and waste. In many settings essential drugs are not used to their full potential.. Lack of access to essential drugs. An increasing number of pharmaceutical products are available in the world market, and there has been rapid growth in both drug consumption and expenditure. However, many people throughout the world cannot obtain the drugs they need, either because they are not available or too expensive, or because there are no ...
You see, in Sri Lanka there is this political monster called the National List. Actually, the original thinking behind the National List was quite honorable. The National List allowed a political party the means to include a person or persons from underrepresented sectors of the population or persons who were in possession of skills and knowledge that were of vital importance to the well-being of the nation. However, in actuality the National List is putty in the hands of party leaders and their parasitic groupies. Many a time, the National List has been used to sneak in unelectable losers who offered unquestioned support to the party hierarchy. This has been the way the cookie crumbled ever since the List was introduced back in the 1980s.. The election of Maithripala Sirisena as President was meant to bring about a new political order. An order based on fairness, transparency and decency. Sirisena promised the country that his cards will always be laid down face up. Sirisenas main groupies in ...

Background: The domain of orthopedic illnesses is grossly underrepresented in the current healthcare scenario especially due to overreliance on mortality rather than the morbidity or dysfunction. Continuous introduction of new drugs in the field of pharmacology requires holistic utilization, effectiveness, and side-effects studies. In addition, inappropriate use of drugs poses a risk of adverse drug reactions (ADRs). Our study aims to analyze the prescription pattern in outpatient department of orthopedics at a tertiary care hospital of Delhi. Methodology: The study was carried out in 210 patients who attended the orthopedics outpatient department for a period of 3 months. Patient demographic details, duration of illness, comorbid conditions, drugs prescribed, ADRs, and usage of complementary and alternative medicine were used to analyze the pattern of drug use. Results: In our study, we observed that vitamins and mineral preparations (20%) was the most commonly prescribed class of drugs ...

Abstract:. Introduction: Assessment of drug use pattern using the World Health Organization (WHO) drug use indicators is a vital step to promote rational drug usage. The objective of the study was to assess the prescription format and adherence to WHO core prescribing indicators in urban health centre, Chetla.. Methods: A descriptive, cross-sectional study was conducted to determine the current prescribing practices at Urban Health Centre, Chetla. Total 360 new prescriptions prescribed by doctors at outpatient department (OPD) were selected on 2 working days per week for a period of 3 weeks, i.e. total 6 working days using census method.. Result: The average number of drugs prescribed per encounter or mean was 2.14(SD 1.058) and the percentage of prescriptions with an antibiotic or injection was 39.4% and 8.6% respectively. The percentage of drugs prescribed by generic name and from an essential drug list was 69.3% and 85.5% respectively. Prescriptions prescribed with fixed dose combination were ...
EDITORIAL. The Global Antibiotic Resistance Partnership (GARP). Antimicrobial resistance (AMR) is an important public health concern shared by developed and developing countries. In developing countries the burden of infectious diseases is greater and exacerbated by limited access to, and availability and affordability of, antimicrobials required to treat infections caused by AMR organisms. With drugs not listed on the essential drugs list (EDL), problems of increased morbidity, costs of extended hospitalisation and mortality are extremely serious. The problem of susceptibility to and spread of infections caused by multidrug-resistant (MDR) infectious agents is fuelled by factors such as limited access to clean water and sanitation to ensure personal hygiene, malnutrition, and the HIV/TB epidemic.. AMR is a consequence of complex interactions of many factors, including inappropriate use (clinical indication, choice, administration and dosing) and poor quality of antimicrobials, inadequate ...
There is some risk in tying a procurement offices reimbursement to the value of purchases by user facilities, as this may create an incentive for the procurement office to increase, rather than decrease, prices and purchases. Therefore, if this sort of approach is used, checks and balances must be put in place, such as a requirement that all major procurement decisions be made by user representatives.. 8. Procurement should be effected in the largest possible quantities in order to achieve economies of scale; this applies to both centralised and decentralised systems.. Justification and explanation. Larger procurement volume makes favourable prices and contract terms more likely, by increasing suppliers interest in bidding and by providing them with an incentive to offer a competitive price.. Practical aspects. A higher volume for single items may be achieved through pooling of procurement volume from many facilities or from several States or countries, by restriction of the drug list or by ...
To obtain preliminary data on the drug supply management system in Ethiopia, selected facilities were assessed for the availability of essential drugs and commodities for malaria, TB and HIV. Of the 48 surveyed hospitals and health centers, 9 (19%), 9 (19%), and 10 (21%) did not have malaria, TB, or HIV drugs, respectively. Similarly, of 27 health posts, 9 (33%) and 6 (22%) did not have rapid diagnostic tests and antimalarial drugs, respectively. The findings indicated an inadequate availability of essential drugs and commodities in the surveyed facilities as well as weaknesses in human resources and training. Assessments of commodity supply chains to ensure operational program success and impact are important.. ...
When viewed sequentially, patients obtained only 61% of their prescribed drugs in the public pharmacy, and only 18% on average understood their use. Such findings suggest that a mere 11% of drugs prescribed through health care centers were both received and had their proper use understood by the patient.. Indicators of the health services. The essential drug list was available in 60% of the centers, but even when available it was not often distributed to prescribers. Of the 40 key drugs studied (Annex), an average of 83.2% was in stock, but interruptions in the supply of widely used drugs such as hypertensive, diabetes mental illness or asthma drugs were common. Stocks ranged from 67.5% to 90%. Only two out of 15 pharmacies had a on-site pharmacist though this is legally required.. Prescribing indicators. For 450 patients interviewed, a total of 1,024 drugs were prescribed with a mean of 2.3 drugs per patient. About one in four prescriptions (26.4%) included an antibiotic and 7.5% an injectable ...
In Uganda, management of tuberculosis (TB) medicines is fully decentralized. Like other government and private programs, the Ministry of Healths (MoH) National Tuberculosis and Leprosy Program (NTLP) procured and supplied tuberculosis (TB) medicines independently to district stores. As a result, parallel supply chain systems emerged alongside the main essential medicines system: Resources and costs were duplicated, overall medicine orders and supplies lacked coordination (which led to expired medicines or stock outs), and at facilities, health workers spent more time making independent orders for different medicines.. The Uganda MoH passed a policy in 2011 to integrate and harmonize supply chain management activities of essential medicines including TB medicines. The harmonization aimed at having one reliable system where planning, procurement, distribution and monitoring of medicines availability can be easily tracked. Following the directive, NTLP shifted responsibilities of procurement, ...

Crisis pregnancy centers (CPCs) are not new to the American cultural landscape. For over three decades, they have provided pregnancy options counseling from an antiabortion perspective. But CPCs are relatively new to the political scene. The last few years have seen a sudden groundswell in both federal and state legislation designed to support and promote them. To date, such efforts have largely focused on two goals: securing direct or indirect government subsidies for CPCs and raising their profile and stature in the public eye.
Ensuring that needed essential medicines and vaccines are available is critical for the success of any disease control program. A great deal is known about what works and what does not work. Careful selection, procurement from prequalified suppliers, proper storage and distribution using secure reliable channels, and assurance of rational use and correct dispensing are all critical components of any drug and vaccine supply system. Ensuring that adequate funds are available to pay for the procurement, distribution, and quality assurance of all medicines and vaccines is equally critical. Depending on the circumstances, either the public or the private sector or a combination of both can efficiently deliver quality-assured medicines and vaccines. The experience of a number of countries and programs has demonstrated that essential medicines and vaccines can be reliably delivered to poor people using the approaches described in this chapter.

However, over the past financial years, anecdotal reports of the EM kit revision and procurement planning processes have shown; a) differences in commodity consumption amongst facilities at the same level of care; (b) Lack of up to date stock data during meetings and therefore, skewed estimates of demand of products; and (c) Budget inadequacies resulting in under-quantification. These limitations made the quantification process a less effective one to predict actual demand of essential medicines for the country.. ...
