Successful treatment of tuberculosis (TB) is one of the key indicators of a TB Control Programs performance and essential to containing the emergence of anti-TB drug resistance. Multi-drug resistant TB (MDR-TB) treatment requires medicines that are expensive, involve longer treatment regimens, are toxic, and can cause patients to have severe side effects. However, international guidelines and national regulations for the diagnosis and treatment of MDR-TB seldom provide unbiased sources of information on monitoring drug use, side effects, or indicators to assess the quality of prescribing according to the regulations.. For proper and impartial monitoring of rational drug use, drug utilization reviews (DUR) are used to identify common problems such as inappropriate product selection, incorrect dosing, avoidable adverse drug reactions, and errors in drug dispensing and administration (see Managing Drug Supply 3: Chapter 30, PDF). Under a prior MSH project, an indicator based guide was developed ...

N2 - Community pharmacists are encountering on-line drug utilization review (DUR) messages with increasing frequency. DUR messages are sent by third party claims processors and generated by pharmacies in-house computer systems. The purpose of this study was to ascertain community pharmacists attitudes toward on-line DUR messages and the actions most often taken by pharmacists after receiving these messages. A 28-item questionnaire was mailed to a random sample of 1,500 community pharmacies throughout the United States. Four hundred twenty-seven questionnaires were returned and analyzed (a 28.5% response rate). The majority of respondents were male pharmacists practicing in independent community pharmacies in cities with populations of less than 50,000. The low response rate from chain pharmacies was an important limitation of this ...

Another large PBM, Express Scripts, also reports on the growing utilization of step therapy. According to the companys 2004 Drug Trend Report, from December 2002 to January 2005, the number of members enrolled in a plan with at least one step-therapy program nearly tripled, going from 4.5 million to 13.2 million. Therapeutic classes are referred to as step-therapy modules. Each module focuses on utilization in a single therapy class, such as antihypertensive drugs, or in a subclass, such as nonsedating antihistamines. The average number of step-therapy modules per Express Scripts client using step-therapy programs increased from 2.5 to 7.2 between 2002 and 2005.. According to a separate study by Hewitt Associates, 27 percent of employers surveyed in early 2005 had either implemented step therapy or were in the process of adopting it for at least one therapeutic class. An additional 38 percent were considering step-therapy programs for their employees.. "The growth in step-therapy programs is ...

Objective: One of the main problems facing public health providers and administrators in many countries is ensuring the rational use of high-cost drugs. In this regard, on-going process of medication use evaluation can be considered as a useful tool. In this study, we evaluated certain usage aspects of a highly-cost medication, that is, recombinant growth hormone (GH). Methods: This cross-sectional study conducted from August 2012 to August 2014. Children receiving GH ± gonadotropin releasing hormone (GnRH) analogs were included in the study. A researcher-designed checklist was developed to evaluate the GH utilization in these patients. Baseline demographic characteristics and background clinical and growth data, as well as any aspects of drug therapy including indications, dosing, monitoring, and discontinuation were collected from the patients medical records. Findings: Seventy children receiving GH entered the study, of which 23 patients (32.85%) received GH and GnRH analogs simultaneously. ...

Antimicrobial therapy is a cornerstone of therapy in critically ill patients; however, the wide use of antibiotics has resulted in increased antimicrobial resistance and outbreaks of resistant disease. To counter this, many hospitals have instituted antimicrobial stewardship programmes as a way to reduce the inappropriate use of antibiotics. However, uptake of antimicrobial stewardship programmes has been variable, as many clinicians fear that they may put individual patients at risk of treatment failure. In this paper, I argue that antimicrobial stewardship programmes are indeed a form of bedside rationing, and explore the risks and benefits of such programmes for individual patients in the intensive care unit, and the critically ill population in general. Using Norman Daniels Accountability for Reasonableness as a framework for evaluating resource allocation policies, I conclude that antimicrobial stewardship programmes are an ethically sound form of bedside rationing. ...

A medical cost containment system for ensuring that the anticipated cost savings from utilization review (UR) agreements are actually realized. The UR agreements are essentially contractual agreements that specify the type and quantity of medical treatments relating to a specific claim resulting from a specific injury. Each UR agreement comprises a claim number, a procedure code describing the particular medical service authorized, and some indication as to dates or quantity of service authorized. All of the UR agreements are stored in a UR database. When a medical bill is received by the insurance company, the bill is entered in to the computer. The cost containment system searches all UR agreements in the UR database which have the same claim number as the claim number on the bill. For each item in the medical bill, the system finds the UR agreement which most closely matches the procedure code in the line item and various other criteria, such as the dates of treatment. The system then checks to

Antimicrobial stewardship program (ASP) is a distinguished method to improve the prescription and efficacy of antibiotics. The efficacy of ASP and conventional methods was compared to measure the effectiveness of meropenem (MPM) and vancomycin (VMN) antibiotics in pediatric patients. In an interventional quasi-experimental study, 135 children admitted in Childrens Hospital affiliated to University of Medical Sciences in time periods of 2014-2015 and 2015-2016 were assessed. The conventional and ASP methods in 2014-2015 and 2015-2016 were respectively utilized to provide the best antimicrobial therapy of MPM and VMN antibiotics in patient children. The data of mortality rate (MR), antibiotic prescription (AP), antibiotic dose (ADe), antibiotic duration (ADn), length of hospital stay (LOHS), and blood cultures (BCs) were compared across the years using the Chi square, independent t test, and Fishers exact test. The levels of MR, AP, ADe, ADn, LOHS, and positive BCs using the ASP method in 2015-2016 were
