A Phase I clinical trial of N-(phosphonacetyl)-l-aspartate, an antimetabolite which inhibits a key enzyme in the de novo pathway of pyrimidine biosynthesis, was conducted. N-(Phosphonacetyl)-l-aspartate was given as an i.v. 15-min infusion once daily for five days; cycles of treatment were repeated every three weeks. Thirty-four patients received treatment. Dose-limiting toxicity was observed at 1500 to 2000 mg/sq m/day and was manifested by skin rash, diarrhea, and stomatitis. Rash and diarrhea usually began during the first week of treatment and persisted up to Day 17 of a cycle of therapy. No consistent hematopoietic, hepatic, or renal toxicity was observed. One partial response in a patient with colon carcinoma was seen and continues at more than eight months. Stable disease was observed in three patients with colon carcinoma, two patients with hypernephroma, one patient with pancreatic carcinoma, and one patient with melanoma. The predictability and reversibility of toxicity and the ...

N2 - Intrinsic drug resistance of pancreatic ductal adenocarcinoma (PDAC) warrants studies using models that are more clinically relevant for identifying novel resistance mechanisms as well as for drug development. Tumor spheroids (TS) mimic in vivo tumor conditions associated with multicellular resistance and represent a promising model for efficient drug screening, however, pancreatic cancer cells often fail to form spheroids using conventional methods such as liquid overlay. This study describes the induction of TS of human pancreatic cancer cells (Panc-1, Aspc-1, Capan-2) in concave polydimethylsiloxane (PDMS) microwell plates and evaluation of their usefulness as ...
The efficacy of recombinant leukocyte A interferon (rIFN-alpha A [Roferon-A, Hoffman-La Roche, Nutley, NJ]) treatment of Kaposis sarcoma in patients with acquired immunodeficiency syndrome was evaluated in sequential trials using high doses (36 X 10(6) units) and low doses (3 X 10(6) units) of interferon. A major response was seen in 38% of patients treated at the high dose, with a median response duration of 18 months. At the low dose, the major response rate was 3%; dose escalation to 36 X 10(6) units resulted in an additional major response rate of 17% in low-dose nonresponders, with a median response duration of 10 months. Four of 11 patients who achieved a complete response remain free of disease, whereas all partial responders have shown disease progression. Unacceptable toxicity occurred in 27% of patients initially treated at the high dose and only in 10% of those who had progressive dose escalation up to 36 X 10(6) units. Prior opportunistic infections correlated negatively with ...

There are two types of alpha receptors: alpha-1 and alpha-2. Alpha receptors result in a variety of actions depending on the type as well as location in various organs and tissues. Smooth muscle contraction is the primary action of alpha-1 receptors. While these receptors act on both arteries and veins, their action is more pronounced in small arteries and arterioles known as resistance vessels. These vessels play an important role in blood pressure, mean arterial pressure, and systemic vascular resistance. Alpha-2 receptors, on the other hand, tend to be located on presynaptic neurons and mediate reduced sympathetic outflow and vasodilation - an effect opposite to that of alpha-1 receptors. These concepts will be discussed in more detail in the next section.. Alpha-1 adrenoceptors are Gq coupled receptors that stimulate phospholipase C [PLC] increasing the production of the second messengers inositol triphosphate (IP) and diacylglycerol (DAG). Alpha-1 stimulation results in smooth muscle ...

Technically purchasing prescription drugs in Canada and bringing them into the US is illegal. The Food Drug and Cosmetic Act prohibits importing drugs from Canada or any other country since the US cant guarantee sure the safety of the medication.. However, the FDA has created an exception for people like Rehberger.. It will look the other way when the product is intended for the patients own use and is not more than a three-month supply.. That is not something the FDA is going to go after them for, said Dr. Janet Woodcock, the FDAs director of drug evaluation and research.. For Rehberger, the jaunt across the border is well worth his time.. You dont need a prescription. You dont need anything. Just tell them what you need, Rehberger said.. The two-month supply of insulin cost Rehberger $116.65.. The price represents a 92% savings compared to the $1,508.94 he might have spent in the US.. Yeah, its well worth the trip, Rehberger said. For the money I save on the insulin, maybe I ...

The U.S. FDA recently approved a new drug, Taltz (ixekizumab), for the treatment of patients suffering from moderate-to-severe plaque psoriasis.. Psoriasis is a long-lasting autoimmune disease characterized by patches of abnormal skin such as red patches or flaking. It occurs more frequently in people who have a family history of the condition. The disease usually begins in people between the ages of 15-35.. The most frequent type of psoriasis, plaque psoriasis, or psoriasis vulgaris, makes up about 90 percent of cases. It typically presents with red patches with white scales on top. Areas of the body most commonly affected are the back of the forearms, shins, around the belly button, and the scalp.. Todays approval provides patients suffering from plaque psoriasis with another important treatment option to help relieve the skin irritation and discomfort from the condition, stated Julie Beitz, director of the Office of Drug Evaluation III in the FDAs Center for Drug Evaluation and ...

Sixteen patients with advanced cancer were treated with recombinant-DNA-produced pure leukocyte A interferon (IFLrA) intramuscularly in doses ranging from 3 to 198 X 106 units, with interval periods of 72 to 96 hours between doses. At the two lowest doses of 3 and 9 million units, there was a cross-over evaluation between IFLrA and partially pure leukocyte interferon (IFN-C) produced from human cells. The maximum observed serum concentration of IFLrA measured by enzyme immunoassay and bioassay increased with increasing doses. The mean serum concentrations of IFLrA and IFN-C were similar. Clinical effects produced by IFLrA and IFN-C were similar, including fever, chills, myalgias, headache, fatigue, and reversible leukopenia and granulocytopenia. Eight patients had transient and mild numbness of the hands or feet, or both. Three patients developed low titers of antibody to IFLrA. Seven of 16 patients showed objective evidence of tumor regression during the study. ...
As cancer cells have a hyperpolarized mitochondrial membrane and deficiency in Kv channel expression, it was postulated that the reversal of this observation may increase apoptosis and inhibit tumor growth. Bonnet and colleagues reported that the administration of DCA led to the switch from glycolysis to oxidative phosphorylation in the Krebs Cycle through inhibition of PDHK. This was associated with an increase in the production of reactive oxygen species and a decrease in hyperpolarization of the inner mitochondrial membrane, leading to efflux of proapoptotic proteins and apoptosis as measured by increased in TUNEL-positive cells. In addition, DCA also decreased the expression of survivin, an anti-apoptotic protein. DCA upregulated the expression of Kv channels in cancer cells, leading to efflux of potassium ions and further increased the proapoptotic effect of DCA. Such change in energy metabolism and apoptosis was not observed in normal cells. DCA was also shown to inhibit tumor growth both ...

June 2013:. There are numerous reasons for me to be very pleased to re-introduce Dr. Eamonn Molloy to the VF readership. First, Eamonn is among a still small group of doctors world-wide who are dedicated to the care of patients with vasculitis. Indeed, he is the only formally trained vasculitis specialist in all of Ireland. Second, I know him to be a superb care provider. We would like to believe that is in large measure related to learning his vasculitis skills over 3 years in our Center. But that would be over-stating our impact. Eamonn came to Cleveland already quite accomplished and only his parents can take credit for his IQ and charming personality. During his time with us, we came to know him as a most effective teacher and predicted the impact he is now having in training other doctors. Add to that, Eamonn is also an excellent researcher, who earned an additional degree in Clinical Research during his stay in the US. Dr. Molloys vasculitis research interests include new drug evaluation ...
