Compliance issues in Treatment- To treat TB, the WHO recommends the so-called DOTS (Directly Observed Treatment Short-course) strategy, where patients take medication under supervision from health staff. Daily supervision of treatment is too demanding for most of our patients and instead implements self-administered therapy with patient education and support to ensure adherence. In many countries affected by conflict, access to health structures is limited for the population. Conflict interrupts travel and makes people fearful of leaving their shelters to seek assistance. It can also lead to the collapse of existing health systems. In India in 2010 292,972 people needed TB re-treatment because of initial treatment relapse, failure or default ...

Current epidemiological aspects of tuberculosis and the impact of the DOTS strategy in disease control. Rev. Latino-Am. Enfermagem [online]. 2007, vol.15, n.1, pp.171-173. ISSN 1518-8345. http://dx.doi.org/10.1590/S0104-11692007000100025.. This article aims to present the current situation of tuberculosis and how the DOTS (Directly Observed Treatment Short Course) strategy has impacted national tuberculosis control programs worldwide, in Latin America and in Brazil. Data reveal a tendency towards a slow decline in disease rates (1%) around the world in 2003. In Brazil, data indicate a constant downward tendency of approximately 3% a year in incidence levels. The DOTS strategy has been recommended to all countries. Brazil needs to improve its tuberculosis surveillance efforts, particularly in terms of confirming negative sputum smear results at the end of ...
We propose to conduct a randomized, unblinded, clinical trial of a medication adherence intervention in opioid-dependent, HIV-infected participants who are initiating new antiretroviral therapy, and who receive opioid agonist maintenance therapy with methadone or buprenorphine in opioid treatment programs (OTPs) in Baltimore, MD. Randomization will be stratified by study site and prior antiretroviral exposure. Two hundred participants will be randomly assigned 1:1 self-administered antiretroviral therapy (SAT) or directly administered antiretroviral therapy (DAART). Subjects assigned to DAART will take morning doses of antiretroviral therapy with a nurse or medical assistant in a private room at the OTP. DAART subjects will be transferred to self-administered therapy after 12 months. This is a 5 year study and participants will be enrolled between month 6 and month 42 of the study. The maximum follow-up for individual participants will be 18 months. Based on our pilot experience we anticipate ...

The return of our nations biggest killer in the 1900s is re-emerging in its most powerful form ever seen. Tuberculosis (TB) faded off Americas screens in the 1950s as the invention of antibiotics became the problem solver. As problems with the disease became domestically solved, funding for the cause decreased under the assumption that other areas would begin to win the fight against TB as well. They never did. Currently new strains of multi-drug resistant (MDR) TB are now the worlds biggest killer, second only to the AIDS virus. Explanations of how TB returned are present, as well as explanations of the areas in most dire need of attention. The synergistic effects that AIDS and Sexually Transmitted Diseases can have on TB are explained. Current program implementations, domestically and internationally, are discussed with special attention diverted to the Directly Observed Treatment Short-Course (DOTS) program. An example of what the author believes to be an effective research design, with ...

The RNTCP response. The countrys response to this public health scourge has mainly been through the Revised National Tuberculosis Control Programme (RNTCP) launched by the Ministry of Health & Family Welfare in 1997. The strategy recommended by the Ministry is known as the Directly Observed Treatment Short Course (DOTS), as propounded by the World Health Organization. DOTS as a strategy was successfully piloted from 1993-96 before being widely advocated and applied. Since the launch of the programme, much has been achieved and the global benchmark of 70 per cent case detection and 85 per cent cure rate of new smear cases has been reached. There is also focus on Multi-Drug resistant TB (MDR-TB) through the DOTS-Plus services in 17 states and efforts are on to make these services available throughout the country. Treatment for 13.5 million patients has been initiated and nearly 2.4 million lives have been saved. Though its impact is yet to be visibly seen, the RNTCP has begun addressing the issue ...
On the other hand, programmatic studies of the effectiveness of the DOTS strategy have shown high rates of treatment success (2,50 - 52). In practice, the trial design necessary to properly evaluate the contribution of DOT alone to the effectiveness of the overall DOTS strategy requires assessment of the social aspects of patient support that surround DOT (as supervised swallowing). The outcomes of programmatic evaluations of the effectiveness of implementation of the DOTS strategy better reflect the social, behavioural and economic factors related to the patient, the health care services and characteristics of treatment.. Many other interventions have been found to significantly improve adherence. One study found that reminder letters sent to patients who failed to attend clinic, appeared to be of benefit even when patients were illiterate (46). Another study reported that home visits by a health worker, though more labour-intensive, may be more effective than reminder letters for ensuring ...

Challenge TB is the primary mechanism for implementing USAIDs vision of a world free of TB and its global End TB Strategy. This includes a decline in TB mortality by 35 percent and a 20 percent reduction in incidence compared to 2015 levels, and no more affected families facing catastrophic costs due to TB by 2019. To achieve this vision, the project complements existing and planned efforts to provide global technical leadership and support to national tuberculosis programs and other in-country partners. ...

The first edition of Guidance for national tuberculosis programmes on the management of tuberculosis in children was published in 2006. It resulted in the revision or development of guidelines for child TB management by national TB programmes in many TB-endemic countries. Now, however, newly published evidence and new recommendations have made it necessary to update the original 2006 guidance.

Collaboration between the Indian Revised National Tuberculosis Control Programme and private practitioners in India improved the case notification of tuberculosis while maintaining good treatment outcomes. Compared with historical surveillance data from the same project areas in a limited number of projects involving private practitioners, higher mean case notification rates were observed after implementation of the public-private mix project. Furthermore, limited data from non-governmental organisations and private industry medical services showed their ability to work with the Indian tuberculosis control programme and successfully diagnose and treat large numbers of patients with tuberculosis. A strong public sector tuberculosis control programme proved critical for provision of necessary advocacy, training, and supervision. These findings highlight the feasibility of building public-private collaborations and the considerable potential they hold for improving tuberculosis control in ...
REMICADE is unique among available anti-TNF biologic therapies. Unlike self-administered therapies that require patients to inject themselves frequently, REMICADE is the only anti-TNF biologic administered directly by caregivers in the clinic or office setting. In RA, CD, and psoriatic arthritis, REMICADE is a two-hour infusion administered every eight weeks, following a standard induction regimen that requires treatment at weeks 0, 2 and 6. As a result, REMICADE patients may require as few as six treatments each year. In AS, REMICADE is a two-hour infusion (5 mg/kg) administered every six weeks, following a standard induction regimen that requires treatment at weeks 0, 2 and 6. The safety and efficacy of REMICADE have been well established in clinical trials over the past 12 years and through commercial experience with over a half a million patients treated worldwide ...

Successfully combating the tuberculosis (TB) epidemic requires that national TB programs (NTPs) prevent new infections and ensure that current patients are cured. Although the treatment for drug-sensitive TB is very effective, curing the disease requires that patients adhere to a strict daily regimen of multiple pills for six to nine months. Adding to the challenge is the fact that treatment for drug-resistant TB is longer, more toxic, and less effective.. All medicines carry some risk of adverse events, and anti-TB medicines are no exception. In addition to threatening the health of patients, adverse events, if not well managed, may also result in individuals stopping their treatment early. Patients who prematurely discontinue treatment may remain sick, develop resistance to the medicines, and spread TB to others in their community.. To support NTPs and health professionals efforts to meet treatment goals and improve the safety of anti-TB medicines, the US Agency for International Development ...
In the past six years of working for the TB Program of Health Service Region 8, TBNet has proven to be a valuable weapon in that battle against the spread of TB. The patients have and are now receiving the continuity of care that was lacking in those earlier years. Thank you for being a reliable resource.
All medicines carry some risk of adverse events; although certain risks are identified when medicines are tested during clinical trials, others arent recognized until after the medicine is on the market and has been used in real world settings. Adverse events not only endanger the health of patients, but if not well managed, they may also result in patients stopping their treatment early. For tuberculosis (TB), premature discontinuation of medicines has serious consequences for both the individual and their community. Patients who stop their treatment early remain sick, develop resistance to the medicines, and risk spreading TB to others in their community.. To help patients complete their full course of TB treatment, the United States Agency for International Development (USAID)-funded Systems for Improved Access to Pharmaceuticals and Services (SIAPS) Program created a guide for national TB programs (NTP) on minimizing medicine risks. The guide Preventing and Minimizing Risks Associated ...

Research shows the person youre supporting benefits most when you are healthy and your life is balanced. With this in mind, supporters are encouraged to take time and care tending to their own health and spirit.. When someone we care about is diagnosed with cancer, we tend to focus our attention on their condition. We may feel sad, fearful or angry, all stemming from concern and uncertainty. As supporters, we may feel alone in our emotions, and discount our experience as relatively not important. Yet acknowledging the changes created in our lives and our relationships will better equip us in providing meaningful support.. One of the best ways to gain support as a supporter is to talk to others who share similar experiences. Open communication in a safe environment can help supporters feel stronger, more involved, and in more control of any challenges they may be facing. In this way, supporters can share resources and tips theyve learned along the way.. ...
