Coronary artery ectasia is a rare disease that occurs in only 0.3-4.9% of people in North America. Coronary artery ectasia is characterized by the enlargement of a coronary artery to 1.5 times or more than its normal diameter. The disease is commonly asymptomatic and is normally discovered when performing tests for other conditions such as coronary artery disease, stable angina and other acute coronary syndromes. Coronary artery ectasia occurs 4 times more frequently in males than in females and in people who have risk factors for heart disease such as smokers. While the disease is commonly found in patients with atherosclerosis and coronary artery disease, it can occur by itself and in both cases it can cause health problems. The disease can cause the heart tissue to be deprived of blood and die due to decreased blood flow, and blockages due to blood clots or spasms of the blood vessel. This blood flow disruption can cause permanent damage to the muscle if the deprivation is prolonged. Coronary ...
Objective: Dilation of one or more coronary artery segments to a diameter at least 1.5 times that of a normal adjacent segment is referred to as coronary artery ectasia (CAE). Adropin is a protein involved in endothelial function and is shown to have a protective effect on the regulation of cardiac functions. Atherosclerosis and endothelial dysfunction play an important role in the development of CAE. The aim of this study was to investigate the association between serum adropin levels and isolated CAE ...
Objective: The present study examined temporal and spatial patterns of extracellular matrix and smooth muscle cell changes in the ascending aorta with bicuspid and tricuspid aortic valve stenosis. Methods: Wall specimens were retrieved from both the greater and the lesser curvature ("convexity" and "concavity") of 14 nonaneurysmal and 12 aneurysmal aortas (aortic ratios 1.2 and 1.5, respectively) and from 3 heart donors (normal). Immunochemistry was performed for detection of apoptotic (terminal deoxynucleotidyl transferase-mediated dUTP nick end labelling [TUNEL]-positive) and proliferating(Ki-67-positive) smooth muscle cells and for semiquantification of matrix proteins (collagens, fibronectin, tenascin, laminin). Co-immunoprecipitation assessed the extent of Bcl-2-modifying factor binding to Bcl-2, indicating a matrixderived cytoskeleton-mediated proapoptotic signaling. Polymerase chain reaction allowed for quantification of messenger RNA expression for Bcl-2. Results: In both bicuspid and ...
Approach and Results-Aortic senescence strongly correlates with aortic root dilatation rate in MFS mice. However, although resveratrol inhibits aortic dilatation, it only shows a trend toward reduced aortic senescence. Resveratrol enhances nuclear localization of sirtuin-1 in the vessel wall and, in contrast to losartan, does not affect leukocyte infiltration nor activation of SMAD2 and extracellular signal-regulated kinases 1/2. Interestingly, specific sirtuin-1 activation (SRT1720) or inhibition (sirtinol) in MFS mice does not affect aortic root dilatation rate although senescence is changed. Resveratrol reduces aortic elastin breaks and decreases micro-RNA-29b expression coinciding with enhanced antiapoptotic Bcl-2 expression and decreased number of terminal deoxynucleotidyl transferase dUTP nick-end labeling-positive cells. In cultured smooth muscle cells, the resveratrol effect on micro-RNA-29b downregulation is endothelial cell and nuclear factor κB dependent. ...

Autori: Ginghina C, Popescu BA, Enache R, Ungureanu C, Deleanu D, Platon P. Editorial: Journal of Cardiovascular Medicine, 9 (7), p.747-750, 2008.. Rezumat:. Dilatation of the pulmonary artery may lead to the compression of adjacent structures. Of those, the extrinsic compression of the left main coronary artery is the most worrisome. We present the case of a 48-year-old woman who was diagnosed with pulmonary artery dilatation due to severe, thromboembolic pulmonary hypertension. She also had angina and coronary angiography revealed a 70% ostial stenosis of the left main coronary artery. The presence of this isolated lesion in a young woman without risk factors for atherosclerosis suggests extrinsic compression of the left main coronary artery by the dilated pulmonary artery as the likely mechanism. The patient underwent direct stenting of the left main coronary stenosis with a good result.. Cuvinte cheie: angina, extrinsic compression, pulmonary artery dilatation, stenting, the left main ...

Various causes of abnormal sensations in the laryngopharynx have been considered. In our case, a pulsating bulge in the posterior wall of the left oropharynx was detected by inspection of the oral cavity, and we additionally performed imaging tests leading to a diagnosis of jugular phlebectasia. Because jugular phlebectasia frequently develops in childhood and on the right side, congenital thinning of the carotid sheaths and the presence of an anatomic venous perfusion impairment have been suggested.2,3 Although rare, there are reports of this disease in adults.1,4 Acquired factors such as inflammation, injury, and the state of Valsalva strain are considered to contribute to its development in adults.2. In many cases with jugular phlebectasia, imaging tests such as computed tomography, MRI, and ultrasound show characteristic findings leading to a definitive diagnosis. Laryngeal diverticulum, laryngocele, and superior mediastinal tumors and cysts are important mass lesions that show size changes ...
Objectives: Aortic root dilatation is the main cause of morbidity and mortality in Marfan syndrome (MFS), a disorder caused by mutations in the gene encoding fibrillin-1 and consequent dysregulation of TGF-beta signaling. The aim of this study was to discover a serological biomarker for the aortic root dilatation in a mouse model of MFS, that was also responsive to losartan therapy.. Methods: Serum samples from mice heterozygous for a fibrillin-1 missense mutation (C1039G/+) and wild-type mice treated with losartan or placebo were obtained at 10 weeks, 6 months and 10 months of age. Total (acid activated) TGF-beta 1 serum concentrations were measured by ELISA. Echo measurements of the aortic root were obtained from a parasternal long axis view at 10 months of age.. Results: Mean TGF-beta serum concentrations were higher in C1039G/+ mice compared to wild-type mice (p=0.01; 80.0 ng/ml (n=5) vs. 58.3 ng/ml (n=4) at 10 weeks, 117.4 ng/ml (n=11) vs. 87.0 ng/ml (n=6) at 6 months, 137.5 ng/ml (n=3) vs. ...
Marfan Syndrome is a rare disease (1:5000)(MIM#154700) caused by mutations of the Fibrillin 1 (FBN1) gene. The major clinical problem is aortic aneurysm with risk of dissection when root diameter is 5 cm.. The investigators designed a clinical trial in which a new generation Beta-Blocker Nebivolol with expected effects on shear stress, heart rate and potential anti-stiffness benefits is compared to Losartan, and Angiotensin receptor blocker anti TGF-beta effects, and to the association of both molecules in patients with Marfan Syndrome. Nebivolol is a patented drug that differs chemically, pharmacologically and therapeutically from all other BBs. Nebivolol shows the highest selectivity for ß1 receptors among the currently available BBs, influences the arterial stiffness through an agonistic effect on ß2-receptors and preserves the arterial compliance. We expect a significantly lower progression of the Aortic Root Dilation in the arm of Nebivolol plus Losartan vs. single drug (primary ...
OBJECTIVE: To assess the clinical significance of transient ischemic dilation of the left ventricle during myocardial perfusion scintigraphy with stress/rest sestamibi. METHODS: The study retrospectively analyzed 378 patients who underwent myocardial perfusion scintigraphy with stress/rest sestamibi, 340 of whom had a low probability of having ischemia and 38 had significant transient defects. Transient ischemic dilation was automatically calculated using Autoquant software. Sensitivity, specificity, and the positive and negative predictive values were established for each value of transient ischemic dilation. RESULTS: The values of transient ischemic dilation for the groups of low probability and significant transient defects were, respectively, 1.01 ± 0.13 and 1.18 ± 0.17. The values of transient ischemic dilation for the group with significant transient defects were significantly greater than those obtained for the group with a low probability (P|0.001). Bicuspid aortic valve (BAV), the most common congenital heart defect, is associated with an increased prevalence of aortic dilation, aortic rupture and aortic valve calcification. Endothelial cells (ECs) play a major role in vessel wall integrity. Little is known regarding EC function in BAV patients due to lack of patient derived primary ECs. Endothelial colony forming cells (ECFCs) have been reported to be a valid surrogate model for several cardiovascular pathologies, thereby facilitating an in vitro system to assess patient-specific endothelial dysfunction. Therefore, the aim of this study was to investigate cellular functions in ECFCs isolated from BAV patients. Outgrowth and proliferation of ECFCs from patients with BAV (n = 34) and controls with a tricuspid aortic valve (TAV, n = 10) were determined and related to patient characteristics. Interestingly, we were only able to generate ECFCs from TAV and BAV patients without aortic dilation, and failed to isolate ECFC colonies from patients ...
In patients determined to have higher risk of dissection on the basis of family history or rapid aortic expansion (≥0.5 cm per year), surgical intervention is reasonable when the aortic diameter reaches ≥5.0 cm. Patients with BAV tend to present with aortic dissection at younger ages than patients with tricuspid aortic valves (7-9), and those with large asymptomatic aneurysms may benefit from prophylactic aortic repair at younger ages than patients with tricuspid aortic valves who have similarsized aneurysms. Observational data support operation for patients with aortic root diameter ≥5.0 cm (10). Therefore, repair or replacement of the aortic root or replacement of the ascending aorta is reasonable when the aortic diameter is ≥5.0 cm in patients with BAV who are at low surgical risk according to the "2014 AHA/ACC Guideline for the Management of Patients With Valvular Heart Disease" (Table 2) (2) when the operation is performed by experienced surgeons in centers with established ...
We read with great interest the paper by Hardikar and Marwick (1) published in a recent issue of iJACC. The authors should be congratulated for their efforts to address the controversial issue of bicuspid aortic valve (BAV)-associated aortopathy in their current meta-analysis. This report is based on a systematic review of the published literature on BAV aortopathy, with a special focus on aortic-related events during long-term follow-up. The most important finding of this meta-analysis is the very low risk of adverse aortic events in patients with BAV disease, which is in contrast to the widespread perception that exists in the cardiovascular-and in particular the cardiac surgery-community. Indeed, some have even incorrectly equated this risk with that observed in patients with connective tissue disorders (e.g., Marfan syndrome). Hardikar and Marwick (1) were able to identify BAV patients that were at higher risk of aortic events, however, according to patients age and clinical setting.. The ...

Marfan syndrome (MFS) is an inherited disorder of connective tissue with morbidity and mortality from aortic dilatation and dissection. The current standard of care is beta-blocker (BB) treatment and therapeutic target is heart rate. The degree of aortic dilatation and response to BB vary in adults with MFS. However, aortic stiffness is often present, and can be a predictor of aortic dilatation and cardiovascular complications. Aortic stiffness is a logical therapeutic target in adults with MFS.. Transforming growth factor beta(TGF-beta) mediates disease pathogenesis in MFS and contributes to aortic stiffness. Cross-talk between TGF-beta system and renin-angiotensin system (RAS) has been demonstrated. The angiotensin receptor blocker (ARB), losartan, inhibits TGF-beta activity and reverses aortic wall pathology in a Marfan mouse model. In a small cohort study, the use of ARB therapy (losartan or irbesartan) significantly slowed the rate of progressive aortic dilatation in patients with MFS, ...
BACKGROUND: Marfan syndrome (MFS) is a disorder of autosomal dominant inheritance, in which aortic root dilation is the main cause of morbidity and mortality. Fibrillin-1 (FBN-1) gene mutations are found in more than 90% of MFS cases. The aim of our study was to summarise variants in FBN-1 and establish the genotype-phenotype correlation, with particular interest in the onset of aortic events, in a broad population of patients with an initial clinical suspicion of MFS. MATERIAL AND METHODS: This single centre prospective cohort study included all patients presenting variants in the FBN-1 gene who visited a Hereditary Aortopathy clinic between September 2010 and October 2016 ...

Mammary duct ectasia represents a benign condition characterized by the dilatation of the mammary ducts. The diagnosis can be clinically challenging due to its complex and diversified symptoms, but also because in some cases it can mimic breast cancer.
Pregnancy places a physiologic load on the cardiovascular system. Plasma volume increases because of conservation of renal salt and water, cardiac output increases up to 50% above the prepregnant level because stroke volume and heart rate are augmented, pulmonary vascular resistance usually falls while PA pressures are maintained despite enhanced flow, and systemic arterial resistance usually decreases (14).. The hemodynamic burden of pregnancy combined with the residual cardiovascular lesions after TOF repair is potentially of concern because of the following: progression of RV dysfunction, atrial and ventricular dysrhythmias, thromboembolic phenomena, maternal or fetal death in the presence of severe PHT, progressive aortic root dilation, intrauterine growth retardation, and endocarditis. Also, offspring of mothers with TOF are more likely to have congenital heart disease, with a reported incidence of approximately 3.1% (15,16).. In our series, patients who had cardiovascular complications ...

In order to better understand the function of Raf-1 in mature cardiac muscle, we used the Cre-loxP system to specifically inactivate the c-raf-1 gene in cardiac muscle. Our data indicate that the c-raf-1 gene is not essential for mouse heart development or the regulation of developmental cardiomyocyte hypertrophic growth. Our data, however, do not exclude the role of the MEK/ERK pathway in cardiac development or hypertrophy, since MEK1/2 and ERK can be activated in Raf CKO hearts.. Raf CKO mice exhibited cardiac dysfunction and heart dilatation at 5 weeks of age. Raf CKO hearts demonstrated a heightened level of apoptosis at 3, 4, and 5 weeks but not at 2 weeks and after 6 weeks of age, even though cardiac contractility progressively decreased during the 10 weeks. These findings suggest that apoptosis is one of the early causes of cardiac dysfunction rather than a terminal event associated with the end stage. Mice are undergoing marked postnatal growth and hypertrophic stimulation during the ...

a motion of the parts of a body by which it expands, or opens itself, so as to occupy a greater space.. Many authors confound dilatation with rarefaction; but the more accurate writers distinguish between them; defining dilatation as the expansion of a body into a greater bulk, by its own elastic power; and rarefaction, the like expansion produced by means of heat.. The moderns have observed, that bodies which, after being compressed, and again left at liberty, restore themselves perfectly, do endeavour to dilate themselves with the same force by which they are compressed; and accordingly they sustain a force, and raise a weight equal to that with which they are compressed.. Again, bodies, in dilating by their elastic power, exert a greater force at the beginning of their dilatation, than towards the end; as being at first more compressed; and the greater the compression, the greater the elastic power and endeavour to dilate. So that these three, the compressing power, the compression, and the ...

Let $P$ be a connected plane embedded graph on $n$ vertices. The {\em dilation} of $P$ on two vertices, $x$ and $y$, is defined to be the Euclidean length of the shortest path in $P$ that connects $x$ with $y$, divided by their Euclidean distance. We are interested in counting the number of vertex pairs that attain the maximum dilation on $P$. For paths we describe an $O(n^{3/2} \log n)$ algorithm for counting the number of maximum dilation pairs. Then we briefly sketch how to generalize this result to closed curves and trees. The normal range for aortic root diameters employing the echocardiographic continuous recording technique was determined in 159 adult subjects without aortic valve disease or hypertension. In order to evaluate the accuracy of this noninvasive technique, the aortic root diameters as measured by ultrasound before operation in 31 patients with aortic valve disease were compared with their respective aortic annulus diameters as determined at the time of valve replacement. The 159 normal subjects consisted of 78 men and 81 women whose ages ranged from 18 to 74 pears (average 38). The normal range of aortic root diameters in this group was 17 to 33 mm (mean 23.7). A significant difference (P is smaller than 0.001) in aortic root diameters existed between men and women which could not be explained by differences in body surface area. The mean diameter among the 81 women was 22.4 plus or minus 3.3 mm(I SD) compared to the mean diameter in the 78 men of 25 plus or minus 3.5 mm. These results indicate a ...
OBJECTIVES: To determine whether coronary artery lesions (ectasia and aneurysm) are commonly observed on the initial echocardiogram of patients with acute Kawasaki syndrome, whether coronary artery ectasia and/or aneurysms occur more frequently in patients with incomplete Kawasaki syndrome than in those patients with complete findings, and whether earlier diagnosis and treatment of Kawasaki syndrome are associated with less frequent occurrence of coronary artery ectasia and/or aneurysm.. DESIGN: A retrospective medical record review.. SETTING: A tertiary care pediatric hospital.. PARTICIPANTS: One hundred patients treated for Kawasaki syndrome between July 1, 1998, and June 30, 2003, who were identified by a medical record search.. MAIN OUTCOME MEASURE: Prevalence of coronary artery lesions (ectasia and aneurysm) on the initial and subsequent echocardiograms.. RESULTS: Forty-four percent of patients had a coronary artery lesion (31% with ectasia, 13% with aneurysm) on the initial echocardiogram. ...
The present epidemiologic community-based echocardiographic study shows that patients with a bicuspid aortic valve without hemodynamically significant stenosis or regurgitation have a larger aortic anulus, aortic sinus, and proximal ascending aorta compared with controls with normal tricuspid aortic valves. The two groups were matched with respect to age, sex, and body surface area to eliminate confounding, because these variables do influence aortic dimensions.25,26⇓ In addition, none of the BAV cases or controls had Marfan syndrome or a family history of Marfan syndrome, and the prevalence of hypertension in each group was low. Both cases and controls are from a geographically defined population of Olmsted County, Minnesota, which helps eliminate geographic variation and reduce referral bias inherent in studies from tertiary medical centers. Importantly, none of the BAV patients were referred to the echocardiography laboratory because of aortic dilatation.. There is biologic rationale and ...
