The second part describes the management of an incomplete evacuation of the uterus after misoprostol use for first trimester miscarriage. Although women with an incomplete evacuation are often relatively asymptomatic, a curettage is often performed. We conducted a randomized controlled trial, comparing curettage to expectant management. A cohort study was conducted alongside this trial following women who received treatment of their preference. These studies show, that although curettage is slightly more effective, expectant management is safe and also effective in at least 5 out of 6 women. Quality of life did not differ between randomized women, and in the cohort, women who underwent expectant management reported a slightly better quality of life. Furthermore, curettage does not seem cost effective over expectant management in women with an incomplete evacuation. In these studies we found no differences in subsequent pregnancy rates or pregnancy outcomes ...

When I was going in for a D&C (dilatation and curettage procedure to remove retained tissue after a miscarriage) I brought my camera as a way to calm and distract myself from the procedure. My husband and I had been pregnant and the fetus had died. We are at the age where we need help to get pregnant. Like many couples today, we have focused on our careers until suddenly, we are on the edge of not being able to conceive. A Work in Progress is a meditation and a calming reprieve from the medical procedures that have become a rhythmic presence in my life. Ongoing visits to medical facilities create intimate spaces of waiting, wherein the stage is not only in the doctors office but in the spaces before and after, including garages, lobbies, bedrooms, and outdoor areas. Taken from the visual perspective of the patient, this project is at once a kind of ritual to cope with a traumatic process, a document of the continuous struggle of a couple trying to create a child, and a contemplation on medicine ...

A miscarriage is the loss of a pregnancy during the first 20 weeks. Miscarriages are very common. Most happen because the fertilized egg in the uterus does not develop normally. Stress, exercise, or sex does not cause a miscarriage.. While many miscarriages pass on their own, some do not. These are called incomplete miscarriages because all of the tissue related to pregnancy is not shed from the uterus. An incomplete miscarriage often requires treatment. Medicine or a procedure call dilation and curettage (D&C) is used to clear the tissue from the uterus.. If your blood type is Rh negative, ask your doctor or midwife if you need a shot of Rh immune globulin (WinRho) to prevent problems in future pregnancies.. ...

In essence, because the uterus is not dilated big enough to remove the entire baby all at once, the baby must be removed in pieces and once removed, a curettage procedure is performed. While this is a very effective and viable option, some women and their partners would like to see and hold their baby, possibly take pictures of and with their baby, receive handprints/footprints from their baby, wash their baby, dress their baby, provide dignified services such as baptisms or blessings, and bury or cremate their baby. Some of these options might be available with a D&E but not all of them.. If I were your bereavement doula, I would personally walk you through these options and discuss them at length so you can make the best decision for your family. Hiring a bereavement doula is highly recommended for miscarriage and stillbirth. It is nearly impossible to explain in such detail here all a bereavement doula would present and help you with in this scenario. Please reach out if you would like to ...

Curettage or root planing is the process of smoothening the root surfaces and removing any infected tooth structure.. The procedure is usually done under local anaesthetic so therefore it is painless.. ...
A Cervical pregnancy is an ectopic pregnancy that has implanted in the uterine endocervix. Such a pregnancy typically aborts within the first trimester, however, if it is implanted closer to the uterine cavity - a so-called cervico-isthmic pregnancy - it may continue longer. Placental removal in a cervical pregnancy may result in major hemorrhage. The incidence has been reported to be about 1:1,000 to 1: 16,000 pregnancies. Pregnancies involving the isthmus - the segment of the uterus between the cervix and the fundus - are more common than true cervical pregnancies. While in many situations the cause of the abnormal implantation remains unclear, there is evidence to link the development of cervical pregnancy to uterine instrumentation, specifically repeated D&Cs (dilatation and curettage). Cervical pregnancies are to be distinguished from pregnancies that start from an implantation in a scar of a previous cesarean section, so-called scar pregnancies. The diagnosis is made in asymptomatic ...

5 000 IU/L.8 Intralesional injection and/or other additional interventions are used when the levels are more than 5 000 IU/L.17 Following systemic administration of MTX, the gestational sac could bulge into the uterine cavity and this could help in performing an easier and uncomplicated dilatation and curettage procedure.18 Initial treatment with MTX followed by dilatation and curettage has shown results of a shorter time of treatment and a more favorable effect.19 Some studies used UAE to reduce the blood loss. Lower bleeding volumes and hospitalization time were noted in the UAE group compared to the MTX group. No major side effects were noted in both groups. UAE followed by suction evacuation were recommended as a priority option in the management of CSP.20 It is important to rule out uterine rupture when any form of non-surgical management is planned.21 During the treatment, the patient must be counseled for surgery in case medical treatment fails or the CSP ruptures.. Ok, folks…this is a hard entry to write. We have lost our baby. I had a miscarriage, and had a procedure called a Dilation & Curettage(D&C) today. I am doing ok-both physically and emotionally. It may sound crazy, but I am really at peace about this. I know that when miscarriages occur, its usually nature seeing a problem and taking care of it. That may sound kind of cold and harsh and clinical of me, but its really a blessing when you think about it. Had this child grown to term, he would have(more than likely) been fraught with chromosomal issues. And, especially after seeing and hearing about the many patients of Marshalls who have a variety of chromosomally-based problems, I realize that all children are blessings-but a whole new set of issues, and whole new level of parental responsibility come into play when a child is born with something like that. I know God gives me free will, and I gladly embrace that. But Im glad that he made this decision for me. Even amidst the pain of today, ...

Spontaneous abortion in the first trimester occurs in up to 20 percent of recognized pregnancies. However, the actual prevalence may be even higher, as many women are treated outside of a hospital setting or are unaware of the pregnancy. For the past 50 years, surgical evacuation by dilatation and curettage (D&C) has been the primary treatment of spontaneous abortion. This procedure is generally considered safe, but complications such as infection, bleeding, uterine perforation and decreased fertility occur in up to 10 percent of women. Recent studies have questioned the need for routine D&C, suggesting that expectant or medical management might be more appropriate. Geyman and colleagues performed a pooled quantitative literature evaluation to compare the outcomes of medical, surgical and expectant management of first trimester spontaneous abortions.. The literature search included MEDLINE citations on spontaneous abortion from 1966 to 1998, along with additional studies listed in the Cochrane ...
Preterm prelabour rupture of membranes (PPROM) complicates up to 2% of all pregnancies and is the cause of 40% of all preterm births. The optimal management of women with PPROM prior to 37 weeks, is not known. Furthermore, diversity in current clinical practice suggests uncertainty about the appropriate clinical management. There are two options for managing PPROM, expectant management (a wait and see approach) or early planned birth. Infection is the main risk for women in which management is expectant. This risk need to be balanced against the risk of iatrogenic prematurity if early delivery is planned. The different treatment options may also have different health care costs. Expectant management results in prolonged antenatal hospitalisation while planned early delivery may necessitate intensive care of the neonate for problems associated with prematurity. We aim to evaluate the effectiveness of early planned birth compared with expectant management for women with PPROM between 34 weeks and 366
