Exposures in the period from conception to early childhood - including fetal growth, cell division, and organ functioning - may have long-lasting impact on health and disease susceptibility. To investigate these issues the Danish National Birth Cohort (Better health in generations) was established. A large cohort of pregnant women with long-term follow-up of the offspring was the obvious choice because many of the exposures of interest cannot be reconstructed with suffcient validity back in time. The study needed to be large, and the aim was to recruit 100,000 women early in pregnancy, and to continue follow-up for decades. Exposure information was collected by computer-assisted telephone interviews with the women twice during pregnancy and when their children were six and 18 months old. Participants were also asked to fill in a self-administered food frequency questionnaire in mid-pregnancy. The effect of random measurement error is to make the observed disease/exposure relation weaker than the true relation. Because the dietary assessment queries intake only on a single day or a few days, measures of usual intake from 24-hour recalls are prone to measurement error. Using a simple average of 2 days does not adequately represent usual intake, particularly for episodically-consumed foods and nutrients. Thus, more sophisticated methods based on statistical modeling are necessary. All of the statistical methods that have been developed to estimate usual intake from 24-hour recalls make the assumption that the 24-hour recall is prone to random, not systematic error (see Module 18, Task 2 for more details on these methods). Two statistical methods have been developed to model the distribution of usual intake of episodically-consumed foods, adjusting for measurement error. The Nurses Health Study II is a prospective cohort study of 116,671 female registered nurses who were 25 to 42 years of age in 1989. For the current analysis, we started follow-up from 1991 when diet was first measured. We excluded women who had an implausible dietary intake, who had reported a diagnosis of cancer, or who were postmenopausal at baseline. We censored women after they reached either natural or surgical menopause (including hysterectomy without bilateral oophorectomy) during follow-up (9)..
