Implantable cardioverter defibrillator (ICD) devices with or without cardiac resynchronization therapy (CRT-D) have the ability to continuously monitor heart rate variability, daily heart rate, and patient activity. Changes in these measures have been associated with heart failure prognosis. In addition, ICD or CRT-D devices also have the ability to measure daily intrathoracic impedance. Previous data have demonstrated that device-based intrathoracic impedance measurements correlate with pulmonary capillary wedge pressure and fluid retention, both of which are common measures of heart failure status. The objective of this study is to evaluate whether the heart rate variability, daily heart rate, and/or intrathoracic impedance can be used in conjunction with other methods to determine if patients are ready for hospital discharge. This thesis is comprised of four studies. Study I was a systematic review performed according to the PRISMA statement and used qualitative methods to analyze data through content analy- sis. Studies II to IV were based on a randomized controlled trial CareHND (NCT03362983). Study II used descriptive statistics to describe patient diagnostic data, Charlson Comorbidity Index scores, and performed a comparison of care utilization patterns between integrated mul- tidisciplinary care and traditional care. The practice was also intelligent enough to offer retail scans too, so in other words if a patient, on their own with no insurance coverage here, wanted to have a full body scan, heart scan, virtual colonoscopy, etc. they could. Now as far as I know theres no law against wanting to have one and paying for it on your own account. If a practice is smart, they make the availability known to the patient, and if a scan is required, it can be done immediately. So without permission from my psychiatrist I stopped taking it because my gut feeling told me that Zoloft was only hurting me. It has not reached your full therapeutic level yet.
