Primary cutaneous aspergillosis (PCA) is an uncommon infection of the skin. There is a paucity of organized literature regarding this entity in regard to patient characteristics, associated Aspergillus species, and treatment modalities on outcome (disease recurrence, disease dissemination, and mortality). We reviewed all published reports of PCA from 1967 to 2015. Cases were deemed eligible if they included the following: patient baseline characteristics (age, sex, underlying condition), evidence of proven or probable PCA, primary treatment strategy, and outcome. We identified 130 eligible cases reported from 1967 to 2015. The patients were predominantly male (63.8%) with a mean age of 30.4 ± 22.1 years. Rates of PCA recurrence, dissemination, and mortality were 10.8%, 18.5%, and 31.5%, respectively. In half of the cases, there was an association with a foreign body. Seven different Aspergillus species were reported to cause PCA. Systemic antifungal therapy without surgery was the most common ...
In a 1984 report of necropsy findings in AIDS patients, Hui and colleagues (29) described cutaneous aspergillosis in a 30-year-old Hispanic homosexual man who died from pulmonary failure caused by Pneumocystis carinii, cytomegalovirus, and acid-fast bacilli. The investigators did not provide details regarding the numbers or locations of lesions, the proximity of lesions to an intravenous catheter site, or neutrophil count. We presume that this patient had primary cutaneous aspergillosis because the investigators did not describe any evidence of disseminated aspergillosis.. In 1992, Hunt and colleagues (30) described two men who developed foci of cutaneous aspergillosis beneath an adhesive dressing near a central venous catheter site. The first patient developed several pruritic umbilicated papules that resembled molluscum contagiosum. Prior to the onset of the lesions, the patient had received doxorubicin, bleomycin, vincristine, ganciclovir, and fluconazole. Neutrophil counts ranged between 1.8 ...

Ketoconazole is an oral or topical synthetic dioxolane imidazole compound that interferes with the biosynthesis of ergosterol, leading to alterations in certain membrane-associated cell functions. It has a high affinity for keratin and a broad spectrum of activity which includes both dermatophytes and yeasts. The risk of hepatitis, albeit rare (1 in 10 000 to 1 in 15 000 of patients treated with a medium time of onset of one month), makes ketoconazole a secondary choice for dermatophyte infections. However, ketoconazole has become the drug of choice for treating Malassezia infections and is an important adjunct in the treatment of AIDS patients with fluconazole-resistant Candida infections. Ketoconazole is not absorbed systemically following topical application, but is well absorbed orally under acid conditions. It is poorly absorbed in patients with achlorhydria and in those taking antacids or H2 receptor antagonists. The usual adult dose is 200-400 mg/day depending on the infection being ...

Many of the skin fungal infections in cancer patients may look similar to those infections in non-cancer patients. However, in some cases, they become more extensive and even life threatening. Prolonged and severe neutropenia is the main risk factor for the dramatic issue of fungal infections. The dermatomycoses in cancer patients can be classified in four broad groups: primary superficial dermatophytoses, primary superficial yeast infections, opportunistic mold infections with distinct potential for dissemination and secondary cutaneous manifestations of fungaemia. Occasionally, more than one fungus are found inside a given skin lesion. A special condition is represented by the mycotic colonization of mucosal ...

Abafungin has been shown to have fungicidal and fungistatic effects on a wide variety of pathogens, including dermatophytes, yeasts (Candida) and moulds. The drug acts on the infecting organisms in two ways: by interfering with the formation of a vital sterol in the fungal cell membrane, thereby preventing cell growth, and also by direct interaction with another membrane component resulting in membrane disruption, leakage of cellular contents and death of the cell and independent of whether it is in a non-metabolising (`resting) phase of development or actively growing. These features of abafungin, combined with the attainment of microbicidal concentrations in the skin from a 1% preparation, its long residence at the site of dermal application and the lack of significant systemic absorption, mean that Abasol(tm) is particularly attractive for the topical treatment of dermatomycoses, which is the first indication under review for marketing approval.. ...

The effect of treatment on outcomes of complex diseases such as PM and DM is challenging to study because of factors that include the rarity of these disorders, the heterogeneity of clinical phenotypes and the lack of sensitive outcome measures. There are no large-scale, randomized controlled trials of IIM confirming the efficacy of commonly recommended immunosuppressive drugs. In our study, we evaluated a relatively large cohort of 160 patients and offer a unique perspective on prognostic factors for patients with PM and DM.. Overall 5-year survival in our cohort was 77%, which is consistent with prior estimates of 60% to 80% [5-8], though a small British study reported 95% [13]. Of the 27 deaths, cardiovascular and respiratory disease were the leading causes (22% each), followed by infections and cancer. Autoimmune disorders, such as lupus, are recognized to increase risk of cardiovascular disease, which is not explained by traditional risk factors [14] but by rather adverse effects of ...
Malassezia pachydermatis is a zoophilic yeast in the division Basidiomycota. It was first isolated in 1925 by Fred Weidman, and has been named pachydermatis Greek for thick-skin after the original sample taken from an Indian rhinoceros (Rhinocerosus unicornis) with severe exfoliative dermatis. Within the genus Malassezia, M. pachydermatis is most closely related to the species M. furfur. A commensal fungus, it can be found within the microflora of healthy mammals such as humans, cats and dogs, However, it is capable of acting as an opportunistic pathogen under special circumstances and has been seen to cause skin and ear infections, most often occurring in canines. Malassezia pachydermatis is a bottle-shaped, non-lipid dependent lipophilic yeast in the genus Malassezia. Colonies are cream or yellowish in colour, smooth to wrinkled and convex with a margin possessing a slightly lobed appearance. Cells are ovoidal in shape and range in size from 3.0-6.5 x 2.5 μm. Colonies grow via unipolar ...

Malassezia spp. are lipophilic fungi, which have been recognised for over a century as members of the normal human cutaneous flora, and also as agents of certain skin diseases. In addition, since the 1980s, they have been reported as causing opportunistic systemic infections. These yeasts have frequently been found on a diverse range of other warm-blooded animals, and their wider distribution in nature is now being explored using molecular techniques. The focus of the present study was the species no lipid-dependent Malassezia pachydermatis, and its characterization through the enzymatic profile by the Api-Zym® system. Then 30 isolated samples of dogs had been used, with and without clinical signals, identified by the technique of Polymerase Chain Reaction (PCR). The samples were cultivated into Dixons modified medium, incubated at 37°C during 48 hours, for posterior quantification through spectrophotometry. An inocula of 65 μl was placed in each microwell of the commercial kit of enzymatic ...

Sir,. Phaeohyphomycetes are dematiaceous (brown pigmented) fungi which derive their pigmentation from melanin in cell walls. Cutaneous and subcutaneous infections due to this group of fungi are reported globally, usually following traumatic implantation of the fungal elements through contaminated soil, wood splinters or thorns. Bipolaris hawaiiensis is a phaeohyphomycete which belongs to division Deuteromycota and genus Cochliobolus. They have been occasionally found in humans, mainly as chronic colonizers and in patients with allergic sinusitis. Fungi usually make their homes in moist areas of the body where skin surfaces meet: between the toes, in the genital area, and under the breasts. Common fungal skin infections are caused by yeasts (such as or ) or , such as Epidermophyton, Microsporum, and Trichophyton . Many such fungi live only in the topmost layer of the epidermis (stratum corneum) and do not penetrate deeper. Obese people are more likely to get these infections because they have excessive skinfolds, especially if the skin within a skinfold becomes irritated and broken down ( ). People with tend to be more susceptible to fungal infections as well.

Background: In contrast to the increasing numbers of agents for the treatment of invasive fungal infections, discoveries of new antifungal agents with therapeutic value in dermatomycoses are reported only rarely. Methods: Abafungin (chemical abstracts service registry No. 129639-79/8) is the first member of a novel class of synthetic antifungal compounds, the arylguanidines. It was first synthesized at Bayer AG, Leverkusen, Germany, and its antifungal action was discovered during the screening of H-2-receptor antagonists based on the structure of famotidine. To obtain insight into its mode of action and antifungal activity, various tests were carried out with different fungal pathogens in vitro. Results: Abafungin was found to have potent antifungal activity. Furthermore, mode-of-action studies suggested that abafungin exerts its antifungal activity regardless of whether the pathogens are growing or in a resting state. One target of abafungin was found to be the inhibition of transmethylation at ...

Signs. The most common strain of yeast that causes infections in canines is Malassezia pachydermatis. If you suspect that your pup has a yeast infection, watch out for these telltale signs:. Itching. One indication that your pooch has a yeast infection is if hes scratching the affected region incessantly. This yeast strain mostly affects a dogs skin, paws, and ears. So, if you notice him scratching any of these areas, he probably has a yeast infection.. Foist smell. This is one the most indisputable signs of a yeast infection in dogs. Often, dog owners have reported their dogs smelling like an old loaf of sourdough bread from San Francisco or like a musty old attic.. Lethargy. Is your pooch spending more time napping than usual? KOH is a strong alkali. When specimen such as skin, hair, nails or sputum is mixed with 20% w/v KOH (preparation of KOH is posted at the end of this post), it softens, digests and clears the tissues (e.g., keratin present in skins) surrounding the fungi so that the hyphae and conidia (spores) of fungi can be seen under microscope.. Uses:. In diagnostic laboratories, KOH mount is one of the main methods of investigating fungal infections. It is used as a primary screening tool, it detects fungal elements present but may not necessarily identify the species of the fungi. (Note: To identify the fungal isolate, specimen must be cultured in either general purpose fungal culture media such as Sabouraud Dextrose Agar (SDA) or specific media based on the type of anticipated isolate.). My name is Dan. I was diagnosed in 1995 with melanoma. I had a wide excision, and two lymph nodes removed. No chemotherapy or other treatment was done at the time. Fast forward twenty years and my melanoma has returned. In October of 2014 I received the news after noticing two separate subcutaneous lesions. Many more have followed. After multiple scans it appears there are no major organ mets, some possible brain mets -(too small) and a possible met behind kidney in fatty tissue. there are several subcutaneous mets on head and torso. Otherwise I am a very healthy and active 50 year old. My oncologist had initially wanted to put me on zelboraf. I declined knowing the short term success and realizing that zelboraf would be a better choice at a later stage. He had asked me again to go on zelboraf. Since I am still very healthy, I asked about trials.. I was sent to Princess Margaret and seen by Dr. Marcus Butler. after reviewing my scans, he thought it best to begin Yervoy. I wasnt even aware of ...

Allergic asthma is a heterogeneous disorder that defies a unanimously acceptable definition, but is generally recognized through its highly characteristic clinical expression of dyspnea and cough accompanied by clinical data that document reversible or exaggerated airway constriction and obstruction. The generally rising prevalence of asthma in highly industrialized societies despite significant therapeutic advances suggests that the fundamental cause(s) of asthma remain poorly understood. Detailed analyses of both the indoor (built) and outdoor environments continue to support the concept that not only inhaled particulates, especially carbon-based particulate pollution, pollens, and fungal elements, but also many noxious gases and chemicals, especially biologically derived byproducts such as proteinases, are essential to asthma pathogenesis ...
